Introduction
Given the high variability and fragility of the thoracic duct, good knowledge of its anatomy is essential for its repair or to prevent iatrogenic postoperative chylothorax. The objective ...of this study was to define a site where the thoracic duct is consistently found for its ligation. The second objective was to define an anatomically safe surgical pathway to prevent iatrogenic chylothorax in surgery for aortic arch anomalies with vascular ring, through better knowledge of the anatomical relationships of the thoracic duct.
Methods
Seventy adult formalin-fixed cadavers were dissected. The anatomical relationships of the thoracic duct were reported at the postero-inferior mediastinum, at levels T3 and T4.
Results
The thoracic duct was consistently situated between the left anterolateral border of the azygos vein and the right border of the aorta between levels T9 and T10, whether it was simple, double, or plexiform. It was located medially, anteromedially, or posteriorly to the left subclavian artery in 51%, 21%, and 28% of the cases, respectively, at the level of T3. At T4, it was posteromedial in 27% of the cases or had no direct relationship with the aortic arch.
Conclusion
These results favor mass ligation of the thoracic duct at levels T9–T10 between the right border of the aorta and the azygos vein, eventually including the latter. To prevent iatrogenic postoperative chylothorax in aortic arch anomalies with vascular ring surgery, we recommend remaining strictly lateral to the left subclavian artery at the level of T3 to reach the aortic arch anomalies with vascular ring at T4.
Inferior vena cava (IVC) agenesis is an uncommon congenital vascular anomaly stemming from aberrant development during embryogenesis. It results from the failure of one or more of the supracardinal ...veins, subcardinal veins, vitelline veins or postcardinal veins to connect. The symptomatology resulting from this vascular malformation can be either absent or extremely rich and varied.
Thoracoabdominal-pelvic CT scan projections following iodine-based contrast product injection were analyzed and a three-dimensional model of vascularization constructed.
Herein, an asymptomatic case of IVC agenesis with absence of the suprarenal and renal segments, with azygos continuation, presenting an accessory hepatorenal vein is reported. The presence of this type of accessory vein has never been described in the literature to date. The etiology of this case of IVC agenesis is explored in depth. We also analyzed the morphometric parameters of the IVC remnant segments and the azygos vein in order to quantify the dilatation of the collateral venous pathway overdeveloped to handle blood return.
Using the findings from this case and those reported in the literature, we provide general recommendations that should be taken into account before managing a patient, symptomatic or asymptomatic, admitted to the hospital with IVC agenesis.
Purpose
To determine the position of the lower loop of the ansa cervicalis (AC) compared to the inferior edge of the omohyoid muscle to guide minimally invasive surgery for the lateral oblique ...approach of parathyroid glands.
Methods
Authors performed 36 anatomical dissections in the laboratory of anatomy (Laboratoire d’Anatomie Des Alpes Françaises) of the Grenoble medical school in 2012 on human cadavers. They independently measured the distance between the caudal extremity of the AC and the lower edge of the superior belly of the omohyoid muscle. Then, they controlled this measure on pictures.
Results
The study shows a majority of long AC (under the omohyoid muscle) in 66.7 % of cases. In addition, the AC was located on an average value of 0.1 cm below the lower edge of the omohyoid muscle (median −0.5 cm). Thus, two-thirds of AC are between 0 and −2 cm under the omohyoid muscle. Furthermore, the AC is generally non-symmetrical: there is a mean difference of 1.3 cm between the left and right AC. In this series, there are as many long AC on the right side as on the left side.
Conclusions
These results are in contradiction with literature data. To preserve the ansa cervicalis and its phonatory functions, it is necessary for the surgeon to perform a systematic per operative identification of the AC because the position of the AC is mainly under the omohyoid muscle and because of an asymmetry. Per operative neurostimulation and/or magnified lenses might be helpful during the surgical approach.
The Brazilian pepper tree ( Schinus terebinthifolius ) is an invasive species that requires significant disturbance to eradicate. Previous studies have identified associations between the Brazilian ...pepper tree and ectomycorrhizal fungi (EMF). However, limited research has explored the connection between disturbance from removal and the effect on EMF. This study investigated the sensitivity of EMF and the broader fungal community to the full and selective removal of the Brazilian pepper tree. During the selective removal of Brazilian pepper tree, we examined the mycorrhizal community of the Arroyo willow ( Salix lasiolepis ) to assess the influence of the restoration disturbance on native species. We used ITS2 sequencing to identify the EMF present during the restoration. Our expectation was that both removal efforts would reduce the presence of EMF. Contrary to our predictions, full removal increased EMF richness and relative abundance in the soil. As anticipated, selective removal reduced the richness and relative abundance of EMF associated with soil. Selective removal led to a decrease in the richness of EMF in arroyo willow roots with no effect on relative abundance. Moreover, fungal community composition in soil and roots shifted significantly during selective and full removal. However, the community composition of EMF, specifically, remained constant across treatment types. During full removal efforts, the application of organic soil amendments may have contributed to the increase in the diversity and relative abundance of EMF. Selective removal will require additional measures, such as soil amendments, to curtail the loss of EMF.
La synovite dans la polyarthrite (PR) conduisant à l’apparition de dommages structuraux se caractérise par la présence d’une hypervascularisation qui peut être mise en évidence en échographie par le ...doppler puissance. Actuellement, il est d’usage de quantifier la synovite en échographie par des scores semi-quantitatifs allant de 0 à 3. L’objectif de cette étude était de déterminer la reproductibilité inter- et intra-observateur d’une approche de mesure quantitative par échographie de la synovite en mode B et par doppler puissance chez des patients atteints de polyarthrite rhumatoïde (PR).
Les articulations superficielles et profondes de patients répondant aux critères ACR/EULAR 2010 de la PR ont été analysées en échographie (MyLab9, Esaote) en mode B et doppler puissance. Toutes les mesures ont été effectuées indépendamment par deux échographistes (CF, PG). Le « Quantification Curves Environment (QPack, Esaote) » a été utilisé pour déterminer manuellement la surface de la synovite de chaque articulation comme région d’intérêt permettant au logiciel de déterminer la surface de la synovite (cm2) et le pourcentage de pixels colorés en Doppler puissance dans cette région. La fiabilité a été quantifiée par le coefficient de corrélation intraclasse (CCIC) intra- et inter-observateur. L’autorisation de conduire cette étude a été obtenue auprès la Direction de recherche clinique et de l’innovation du CHU Grenoble Alpes.
Soixante et une articulations de sept patients atteints de PR ont été analysées. Les 32 premières articulations présentant une synovite ont été évaluées quatre fois (deux fois par opérateur) afin de calculer la reproductibilité intra-opérateur. La durée de l’examen était d’environ 1 à 3minutes par articulation. La reproductibilité inter-opérateur était excellente pour la surface de la synovite en mode B (CCIC 0,98 IC95 % : 0,97 ; 0,99) et bonne pour la quantification du signal doppler (CCIC=0,77 IC95 % : 0,65 ; 0,85). La reproductibilité intra-opérateur était excellente pour la surface de la synovite en mode B (évaluateur 1 : 0,98 IC95 % : 0,97 ; 0,99, évaluateur 2 : 0,96 IC95 % : 0,92 ; 0,98) et bonne (0,71 IC95 % : 0,48 ; 0,84) à excellente (0,94 IC95 % : 0,90 ; 0,97) pour la quantification du signal doppler.
La quantification précise de la synovite vascularisée ouvre la possibilité d’évaluer et de quantifier au cours du suivi du patient le niveau des flux vasculaires dans les mêmes zones cibles de façon plus précise qu’un score semi-quantitatif. Cette méthode de quantification pourrait renforcer la valeur prédictive de l’échographie et l’adhésion thérapeutique des patients en fournissant une démonstration chiffrée du niveau d’inflammation articulaire. Les progrès dans le domaine de l’intelligence artificielle et du traitement d’images laissent entrevoir la possibilité d’automatiser l’analyse des images pour favoriser l’implémentation en pratique clinique.
La reproductibilité intra-observateur et inter-observateur de la quantification de la surface en mode B et du signal doppler de la synovite de la PR est bonne à excellente. De futures études sont nécessaires pour démontrer la valeur ajoutée de cette méthode de quantification.
On March 16, 2020, the French Society of Pharmacology and Therapeutics put online a national Question and Answer (Q&A) website, https://sfpt-fr.org/covid19 on the proper use of drugs during the ...COVID‐19 pandemic. The working group ‘Drugs and COVID‐19’ was composed of a scientific council, an editorial team, and experts in the field. The first questions were posted online during the first evening of home‐confinement in France, March 17, 2020. Six weeks later, 140 Q&As have been posted. Questions on the controversial use of hydroxychloroquine and to a lesser extent concerning azithromycin have been the most consulted Q&As. Q&As have been consulted 226 014 times in 41 days. This large visibility was obtained through an early communication on Twitter, Facebook, traditional print, and web media. In addition, an early communication through the French Ministry of Health and the French National Agency for Medicines and Health Products Safety ANSM had a large impact in terms of daily number of views. There is a pressing need to sustain a public drug information service combining the expertise of scholarly pharmacology societies, pharmacovigilance network, and the Ministry of Health to quickly provide understandable, clear, expert answers to the general population’s concerns regarding COVID‐19 and drug use and to counter fake news.
Les patients atteints de rhumatisme inflammatoire chronique (RIC) ont un risque augmenté de comorbidités et donc une mortalité plus élevée. D’autre part, les patients atteints de comorbidités ont ...moins de probabilité d’atteindre la rémission.
Notre objectif était d’étudier la relation entre le maintien thérapeutique des bDMARDS ou ts DMARDS et l’adhésion aux recommandations de prise en charge des comorbidités selon l’EULAR chez des patients atteints de RIC.
Nous avons conduit une étude de cohorte prospective mono centrique dans le service de rhumatologie de Grenoble. Les critères d’inclusion étaient un âge supérieur à 18 ans, un diagnostic de spondyloarthrite, rhumatisme psoriasique ou polyarthrite rhumatoïde fait par un rhumatologue expert et une initiation de bDMARDS ou ts DMARDS. 2 groupes ont été créés en fonction du pourcentage de recommandations suivies (plus ou moins de 80 % des recommandations émises). Le critère de jugement principal était le taux d’arrêt de la biothérapie en fonction du groupe de suivi des recommandations (adhérent et non adhérent aux recommandations de dépistage de comorbidités) sur les 18 mois de suivi. Les critères de jugement secondaire étaient les taux d’arrêt en fonction de la cause (intolérance ou inefficacité).
Un total de 180 patients a été inclus dans notre étude, 108 femmes (60 %) pour 72 hommes (40 %), d’âge moyen de 49 ans. Parmi ceux-ci, 86 étaient adhérents aux recommandations au seuil de 80 % contre 94 patients non-adhérents. 74 patients (58,7 %) ont arrêté le bDMARD prescrit à 18 mois dont 49/74 patients (66,2 %) pour manque d’efficacité et 25/74 patients (33,8 %) pour intolérance. L’arrêt des bDMARDS ou tsDMARDS pour intolérance était significativement associé à un moindre suivi des recommandations (OR=0.362 IC95 % : 0.127 ;0.941, p<0.05), contrairement à l’arrêt toutes causes (OR=0,5810,288 ;1,144, p=0.121) ou l’arrêt pour inefficacité ((OR=0.980 IC95 % :0.461 ;2.066, p=0.958).
Le rôle des comorbidités dans l’arrêt des bDMARDS en raison d’événements indésirables a déjà été suggéré dans d’autres études. Nous fournissons ici des données montrant que l’arrêt de bDMARDS ou ts DMARDS en raison d’événements indésirables était plus probable chez les patients qui n’observaient pas les recommandations pour une prise en charge optimale des comorbidités, sans que cela affecte l’arrêt pour inefficacité. Aucun des paramètres associés à l’arrêt du bDMARD, tels que la prescription concomitante de sDMARDS, la durée de la maladie, le nombre de précédents bDMARDS, le sexe féminin n’influençait l’interaction entre l’arrêt du bDMARDS et le respect des recommandations. Cela pourrait s’expliquer par le fait que ces facteurs prédictifs de l’arrêt du bDMARD influencent principalement son efficacité.
L’adhésion aux recommandations pour une prise en charge optimale des comorbidités pourrait avoir un impact sur l’adhésion au bDMARD ou ts DMARDS, ce qui soutient la nécessité de la mise en œuvre d’un dépistage systématique dans la pratique quotidienne.
