Background Patients with asthma and healthy controls differ in bacterial colonization of the respiratory tract. The upper airways have been shown to reflect colonization of the lower airways, the ...actual site of inflammation in asthma, which is hardly accessible in population studies. Objective We sought to characterize the bacterial communities at 2 sites of the upper respiratory tract obtained from children from a rural area and to relate these to asthma. Methods The microbiota of 327 throat and 68 nasal samples from school-age farm and nonfarm children were analyzed by 454-pyrosequencing of the bacterial 16S ribosomal RNA gene. Results Alterations in nasal microbiota but not of throat microbiota were associated with asthma. Children with asthma had lower α- and β-diversity of the nasal microbiota as compared with healthy control children. Furthermore, asthma presence was positively associated with a specific operational taxonomic unit from the genus Moraxella in children not exposed to farming, whereas in farm children Moraxella colonization was unrelated to asthma. In nonfarm children, Moraxella colonization explained the association between bacterial diversity and asthma to a large extent. Conclusions Asthma was mainly associated with an altered nasal microbiota characterized by lower diversity and Moraxella abundance. Children living on farms might not be susceptible to the disadvantageous effect of Moraxella . Prospective studies may clarify whether Moraxella outgrowth is a cause or a consequence of loss in diversity.
Hintergrund und Ziel: Die im Rahmen der COVID-19-Pandemie erlassenen Maßnahmen zum Infektionsschutz führten zu tiefgreifenden Einschränkungen und Veränderungen im sozialen, (vor-)schulischen, ...familiären und Freizeitbereich. Die vorliegende Studie untersucht das Ausmaß an psychischer Belastung von Kindern, Jugendlichen und ihren Familien während der COVID-19-Pandemie. Mögliche Einflussfaktoren sollen identifiziert werden. Material und Methoden: Die Untersuchungen erfolgten zwischen Herbst 2020 und Frühjahr 2021 in einer klinischen Inanspruchnahmestichprobe (n = 280 Patient:innen zwischen 4–17 Jahren) und einer Feldstichprobe (n = 1958 Kinder und Jugendliche zwischen 4–19 Jahren, über Schulen und vorschulische Einrichtungen rekrutiert). Dabei wurden Urteile der Eltern sowie Selbsturteile der Kinder und Jugendlichen mittels Fragebögen erfasst. Ergebnisse: Die psychische Belastung der Kinder und Jugendlichen im Zusammenhang mit der Pandemie wird über beide Beurteilungsperspektiven und Stichproben hinweg als leicht bis moderat erhöht eingeschätzt. Rund 60–70 % der Eltern- und Selbsturteile beschreiben eine Zunahme dieser Belastung, während Entlastungen von bis zu 12 % sowohl im Eltern- als auch im Selbsturteil angegeben werden. Beim Vergleich der beiden Stichproben zeigt sich eine leicht höhere Belastung der Kinder und Jugendlichen nur im Selbsturteil der Klinikstichprobe. Die untersuchten soziodemografischen Faktoren haben keinen Einfluss auf die Belastung. Allerdings zeigen sich in beiden Stichproben leichte bis moderate Zusammenhänge zwischen der subjektiv erlebten Verschlechterung der familiären und sozialen Situation und einem erhöhten Belastungserleben. Background
The Borna disease virus (BoDV-1) is an emerging zoonotic virus causing severe and mostly fatal encephalitis in humans.
Methods and Results
A local cluster of fatal BoDV-1 encephalitis ...cases was detected in the same village three years apart affecting two children. While the first case was diagnosed late in the course of disease, a very early diagnosis and treatment attempt facilitated by heightened awareness was achieved in the second case. Therapy started as early as day 12 of disease. Antiviral therapy encompassed favipiravir and ribavirin, and, after bioinformatic modelling, also remdesivir. As the disease is immunopathogenetically mediated, an intensified anti-inflammatory therapy was administered. Following initial impressive clinical improvement, the course was also fatal, although clearly prolonged. Viral RNA was detected by qPCR in tear fluid and saliva, constituting a possible transmission risk for health care professionals. Highest viral loads were found
post mortem
in the olfactory nerve and the limbic system, possibly reflecting the portal of entry for BoDV-1. Whole exome sequencing in both patients yielded no hint for underlying immunodeficiency. Full virus genomes belonging to the same cluster were obtained in both cases by next-generation sequencing. Sequences were not identical, indicating viral diversity in natural reservoirs. Specific transmission events or a common source of infection were not found by structured interviews. Patients lived 750m apart from each other and on the fringe of the settlement, a recently shown relevant risk factor.
Conclusion
Our report highlights the urgent necessity of effective treatment strategies, heightened awareness and early diagnosis. Gaps of knowledge regarding risk factors, transmission events, and tailored prevention methods become apparent. Whether this case cluster reflects endemicity or a geographical hot spot needs further investigation.
