Introduction Les enregistrements glycémiques interstitiels continus (CGMS) sont la référence pour l'analyse de la variabilité glycémique, mais pas toujours accessibles. Les glycémies capillaires de ...l'Auto Surveillance Glycémique (ASG) peuvent-elles apporter un renseignement équivalent ? Matériels et Méthodes Des diabétiques de type 1 (DT1), et des témoins ont bénéficié d'un CGMS sur 5 jours à domicile, comparé aux données de l'ASG. Les variabilités Interstitielles et Capillaires ont été calculées et comparées : écarts-type (ET) et coefficient de variation (CV) : tests t pour valeurs appariées et corrélations ; et entre diabétiques et contrôles : ANOVA. Résultats Nous avons étudié 13 sujets DT1 (âge : 53 ± 13 ans, durée de diabète : 19 ± 15 ans, HbA1c : 7,5 ± 1,1 %) et 6 témoins (âge : 30 ± 7 ans). Les glycémies des DT1 étaient supérieures (8H00 : Gcap 171 ± 52 mg/dL et Gint 136 ± 38) à celles des témoins (8H00 : Gcap 82 ± 7 mg/dL et Gint 76 ± 15, p < 0,05 vs DT1 pour chaque). Les Glycémies interstitielles étaient inférieures aux capillaires (8H00 : Gcap 143 ± 66 mg/dL vs Gint 117 ± 43, p < 0,005), avec une bonne corrélation entre elles (R = 0,85, p < 0,001). Les ET capillaires étaient supérieurs aux interstitiels (8H00 : ETcap 37 ± 32 mg/dL, ETint 26 ± 14 ; p = 0,061 ; différence significative à 14H : p = 0,029 et 22H : p = 0,010), avec une bonne corrélation entre les deux (8H00 : R = 0,75, p < 0,001). Les CV capillaires (8H00 : 24 ± 16 %) et interstitiels (22 ± 8 %) ne différaient pas et étaient bien corrélés entre eux (8H00 : R = 0,66, p = 0,002). Les CV ne variaient pas significativement durant la journée. La variabilité capillaire était supérieure chez les DT1 (8H00 : ETcap 50 ± 30 mg/dL et CVcap 29 ± 14 %) par rapport aux témoins (8H00 : ET cap 8 ± 10 mg/dL et CV cap : 11 ± 15 %, p < 0,05 pour les 2) ; avec les valeurs interstitielles la différence était présente pour l'ET ( p < 0,005) mais non significative pour le CV ( p = 0,10). Conclusions Le Coefficient de Variabilité ne diffère pas selon qu'il soit estimé par l'ASG ou par le CGMS, avec une bonne corrélation entre ces deux valeurs.
Le dumping syndrome (DS) est une complication fréquente du bypass gastrique (BPG). Le dumping précoce se caractérise par des symptômes digestifs et vasomoteurs, le dumping tardif par des ...hypoglycémies, survenant 1 à 3 heures après le repas, sans signes de neuroglucopénie. Les analogues de la somatostatine ont été proposés dans les DS après chirurgie gastrique carcinologique. Leur place dans la prise en charge du DS post BPG est moins connue. Madame M, 54 ans présente un DS sévère post BPG. Après échec des mesures diététiques et de l’acarbose sur les hypoglycémies, un traitement par analogue de la somatostatine est introduit : Sandostatine® 100 μg × 3 par jour par voie sous-cutanée puis Sandostatine LP 30® tous les 28 jours par voie IM. Un profil de mesure continue du glucose interstitiel par capteur est réalisé avant et après traitement. Après 3 injections IM, Madame M rapporte une diminution des symptômes. Les pourcentages de valeurs de glucose interstitiel inférieures ou égales à 60 mg/dl passent de 5 % à 3 %. Les pourcentages de valeurs de glucose interstitiel comprises entre 60 et 80 mg/dl diminuent de 49 % à 7 %. Les pourcentages des valeurs de glucose interstitiel post-prandiales inférieures à 80 mg/dl passent de 70 à 0, de 32 à 0, de 27 à 16 pour le petit-déjeuner, le déjeuner et le dîner respectivement. La tolérance est bonne. Les analogues de la somatostatine semble un traitement intéressant dans les DS après BPG. La mesure continue du glucose interstitiel peut aider à la prise en charge de cette symptomatologie.
L’hyperthyroïdie gestationnelle transitoire est un trouble fréquent dans les hyperémesis gravidarum. L’hyperthyroïdie non contrôlée qui perdure pendant la grossesse est associée à plus de ...pré-éclampsie, prématurité, retard de croissance intra-utérin (RCIU) et faibles poids de naissance. Le risque lié à la sévérité de la thyréotoxicose transitoire a été peu évalué et l’intérêt du traitement par antithyroïdien de synthèse (ATS) dans cette indication est réservé aux formes sévères.
Étude rétrospective monocentrique sur 202 patientes hospitalisées pour vomissements gravidiques avec une hyperthyroïdie biologique. Comparaison du déroulement de la grossesse, de la période néonatale et du développement des enfants, entre un groupe en thyréotoxicose transitoire et un groupe témoin en hyperthyroïdie infraclinique.
Parmi les complications étudiées, le groupe thyréotoxicose (n=156) n’a pas présenté plus de fausses couches spontanées (FCS), évènements hypertensifs, RCIU, prématurité ou petit poids de naissance que le groupe infraclinique (n=46). Les 7 FCS sont survenues dans le groupe thyréotoxicose non traitée. Pas de différence retrouvée de développement staturopondéral et psychomoteur entre les enfants des 2 groupes. Dix-huit patientes en thyréotoxicose ont été traitées par ATS : pas de dysthyroïdie néonatale, ni de modification du déroulement de la grossesse. Parmi les malformations néonatales décrites, 7 sur 8 sont dans le groupe thyréotoxicose, dont certaines très rares (trigonocéphalie et encéphalocèle).
La thyréotoxicose gestationnelle transitoire n’est pas associée à plus de complications de la grossesse dans notre étude. Cependant, des malformations rares sont décrites et les FCS sont toutes survenues dans le groupe thyréotoxicose non traitée.
In 2009–2011, 113 adult in- and outpatients with measles were referred to the University Hospital of Clermont-Ferrand (centre of France): 71 (62.8 %) needed hospitalisation, 31 had pneumonia, 29 ...diarrhoea, 47 liver enzymes elevation, 38 thrombopaenia, one encephalitis and there were no deaths. Nineteen cases occurred among healthcare workers and five of them were hospital-acquired. There were 92 unvaccinated patients. The 2011 peak of that measles re-emerging epidemic occurred when non-immunised adults were affected.
Human echovirus types 6 (E-6) and 30 (E-30) cause seasonal epidemics of aseptic meningitis. These two enteroviruses are frequently observed in co-circulation, an epidemiological pattern that is ...prerequisite for the occurrence of dual infections, which can lead to recombination between co-infecting virus strains. Viral sequences were determined at loci 1D (VP1 capsid protein) and 3CD (non structural proteins) in 49 E-6 strains recovered in a single geographical region in France from 1999 to 2007, during the epidemiological survey of enterovirus infections. They were compared with previously recorded sequences of E-30 strains to investigate their evolutionary histories and possible recombination patterns. Phylogenetic analyses identified two distinct E-6 populations and different subpopulations. Assuming a relaxed molecular clock model and a Bayesian skyline demographic model in coalescent analyses with the BEAST program, the substitution rate in E-6 was estimated at 8.597×10−3 and 6.252×10−3 substitution/site/year for loci 1D and 3CD respectively. Consistent estimates of divergence times (tMRCA) were obtained for loci 1D and 3CD indicating that two distinct E-6 populations originated in 1997 and 1999. Incongruent phylogenetic patterns inferred for the two loci were indicative of recombination events between the two populations. Phylogenies including the E-30 3CD sequences showed close genetic relationships between E-6 and discrete E-30 subpopulations. Recombination breakpoints were located with statistical significance in E-6 and E-30 genomes. Estimates of tMRCA of phylogenetic recombinant clades indicated directional genetic transfers from E-30 to E-6 populations and their co-divergence over the time period studied.
Earlier studies indicated that density-arrested cancer cells released an unidentified growth inhibitor whose secretion was prevented by overexpression of the lysosomal protease cathepsin D (cath D). ...In this study, this growth inhibitor was purified by affinity chromatography and identified as the heat shock cognate 70 protein (hsc70) based on its peptide microsequencing and specific antibody recognition. Among intracellular proteins, including other heat shock proteins, only constitutive hsc70 was secreted in response to the high-cell density. Moreover, hsc70 secretion from cancer cells was generated by serum deprivation, whereas its cellular concentration did not change. Prevention of Hsc70 secretion by cath D overexpression was associated with the formation of multilayer cell cultures, thus indicating a loss of contact inhibition. In addition, we showed that supplementing the culture medium with purified hsc70 inhibited cell proliferation in the nanomolar range. Conversely, removal of this extracellular hsc70 from the medium by either retention on ADP-agarose or competition at the Hsc70 binding site restored cell proliferation. Hsc70 appears active in human breast cancer cells and hypersecreted by direct cath D inhibition. These results suggest a new role of this secreted hsc70 chaperone in cell proliferation that might account for the higher tumor growth of cancer cells overexpressing cath D.

Our neonatal intensive care unit (NICU) was designed with a web-camera system in eight private rooms and four open wards. This equipment was installed to both guarantee children's safety in the NICU ...and promote the bond between the child and his/her family through a viewing service permitted by an Internet access.
We evaluated the web-camera system in its 5th year with two types of users. The nursing staff was asked about use of the video and its impact on their management of NICU patients. Questionnaires for parents sought to determine how they used the system and their feelings about it.
Ninety-three percent of the nursing staff used the web-camera system to provide medical supervision or to comply with developmental care, mainly to respect the baby's natural rhythm by initiating care only when the baby showed signs of awakening. The web-camera system allowed them to observe the baby, verify his/her good body position, and spot discomfort situations. It helped provide a faster and personalized answer that was adjusted to the baby's needs. Sixty-one percent of the parents used the remote connection, half of them to present the child to the family. Only 17% of parents were embarrassed by the cameras; 88% of parents thought that the video was an additional safety device in NICU management.

It helped staff react faster to situations that created an inappropriate stimulation for the baby. It also allowed to them to respect the baby's natural rhythm. It reassured nursing staff and parents and it facilitated the baby's integration into the family.

Après 5 ans d’implantation, nous avons évalué à l’aide de questionnaires l’utilisation de la vidéo et son impact sur la prise en charge des nouveau-nés par les soignants, l’utilisation de cet outil par les parents et leur ressenti.
Ce système était utilisé par 93 % des professionnels dans le cadre d’une surveillance médicale ou des soins de développement, principalement pour respecter le rythme de l’enfant et réaliser les soins lors de phases d’éveil. Il avait permis d’observer le bébé, de vérifier son positionnement et repérer les situations d’inconfort, ce qui avait facilité une prise en charge plus rapide, personnalisée et adaptée à ses besoins. La connexion à distance avait été utilisée par 61 % des parents dont près de la moitié pour présenter le bébé à la famille. Seulement 17 % des parents avaient été gênés par la présence des caméras. La plupart d’entre eux (88 %) estimait que la vidéo était un élément de sécurité supplémentaire dans la prise en charge.
La vidéo apparaît comme une technologie intéressante pour faciliter les soins de développement lorsque les enfants sont installés en chambre. Elle permettrait de réagir plus rapidement à des situations dystimulantes et d’intervenir en respectant le rythme propre de chaque nouveau-né. Elle rassurerait les professionnels et les parents, faciliterait l’accueil de l’enfant dans la famille.

Enteroviruses (EV) are the main etiological agents of aseptic meningitis. Diagnosis is made by detecting the genome using RT-PCR. The aim of the study was to evaluate the impact of a positive ...diagnosis on the management of infants, children, and adults. During 2005, 442 patients were admitted to hospital with suspected meningitis. Clinical and laboratory data and initial treatment were recorded for all patients with enteroviral meningitis. The turnaround time of tests and the length of hospital stay were analyzed. The results showed that EV-PCR detected EV in 69 patients (16%), 23% (16/69) were adults. About 18% of CSF samples had no pleocytosis. After positive PCR results, 63% of children were discharged immediately (mean 2 hr 30 min) and 95% within 24 hr. Infants and adults were discharged later (after 1.8 and 2 days, respectively). The use of antibiotics was significantly lower in children than in infants and adults. The PCR results allowed discontinuation of antibiotics in 50-60% of all patients treated. Patients received acyclovir in 16% of cases (7% children vs. 50% adults) and 23% (11% vs. 69%) underwent a CT scan. Clinical data were compared between patients whose positive EV-PCR results were available within 24 hr (n = 32) and those whose results were available > 24 hr after collection of CSF (n = 14). Duration of antibiotic treatment (difference: 2.3 days; P = 0.05) was reduced between the two groups. No statistical difference in the length of stay was observed. The EV-PCR assay should be performed daily in hospital laboratory practice and considered as part of the initial management of meningitis. J. Med. Virol. 81:42-48, 2009.
