It summarizes the science questions driving the Ultra-High-Energy Cosmic-Ray (UHECR) community and provides recommendations on the strategy to answer them in the next two decades.
Background: Transmyocardial laser revascularization was used as the sole therapy for patients with ischemic heart disease not amenable to percutaneous transluminal coronary angioplasty or coronary ...artery bypass grafting. This technique uses a carbon dioxide laser to create transmyocardial channels for direct perfusion of the ischemic heart.
Methods: Since 1992, 200 patients, at eight hospitals in the United States, have undergone transmyocardial laser revascularization. The patients have a combined 1560 months of follow-up for an average of 10 ± 3 months per patient. Their age was 63 ± 10 years and their ejection fraction was 47% ± 12%. Eighty-two percent had at least one previous bypass graft operation and 38% had a prior angioplasty. Preoperatively, the patients underwent nuclear single photon emission computed tomography perfusion scans to identify the extent and severity of their ischemia. These scans were repeated at 3, 6, and 12 months. Angina class, admissions for angina, and medications were recorded.
Results: The perioperative mortality was 9%. Angina class decreased significantly from before treatment to 3, 6, and 12 months (
p < 0.001). Likewise, there was a significant decrease in the number of perfusion defects in the treated left ventricular free wall. Concomitantly, there was a significant decrease in the number of admissions for angina in the year after the procedure when compared with the year before treatment (2.5 vs 0.5 admissions per patient-year).
Conclusion: These combined results indicate that transmyocardial laser revascularization provides angina relief, decreases hospital admissions, and improves perfusion in patients with severe coronary artery disease. (J Thorac Cardiovasc Surg 1997;113:645-54)

Effluent toxicity testing methods have been well defined, but for the most part, these methods do not attempt to segregate the effects of active ionic concentrations and ion imbalances upon test and ...species performances. The role of various total dissolved solids in effluents on regulatory compliance has emerged during the last few years and has caused confusion in technical assessment and in permitting and compliance issues. This paper assesses the issue of ionic strength and ion imbalance, provides a brief summary of applicable data, presents several case studies demonstrating successful tools to address toxicity resulting from salinity and ion imbalance, and provides recommendations for regulatory and compliance options to manage discharges with salinity/ion imbalance issues. Effluent toxicity resulting from inorganic ion imbalance and the ion concentration of the effluent is pervasive in permitted discharge from many industrial process and municipal discharges where process streams are concentrated, adjusted, or modified. This paper discusses procedures that use weight‐of‐evidence approaches to identify ion imbalance toxicity, including direct measurement, predictive toxicity models for freshwater, exchange resins, mock effluents, and ion imbalance toxicity with tolerant/susceptible text species. Cost‐effective waste treatment control options for a facility whose effluent is toxic because of total dissolved solids (TDS) or because of specific ion(s) are scarce at best. Depending on the discharge situation, TDS toxicity may not be viewed with the same level of concern as other, more traditional, toxicants. These discharge situations often do not require the conservative safety factors required by other toxicants. Selection of the alternative regulatory solutions discussed in this paper may be beneficial, especially because they do not require potentially expensive or high‐energy–using treatment options that may be ineffective control options. The information presented is intended to provide a better understanding of the role of ion imbalance in aquatic toxicity testing and to provide various recommendations that should be considered in addressing these issues.
Regulatory T cells (T(regs)) are crucial in the maintenance of the immune tolerance and seem to have an important role in systemic sclerosis (SSc). The interleukin 2 receptor α (IL2RA) is an ...important T(reg) marker, and polymorphisms of IL2RA gene are associated with a number of autoimmune diseases. Therefore, we aimed to investigate for the first time the association of the IL2RA locus in SSc. For this purpose, a total of 3023 SSc patients and 2735 matched healthy controls, from six European Caucasian cohorts, were genotyped for the IL2RA gene variants rs11594656, rs2104286 and rs12722495 using the TaqMan allelic discrimination technology. The overall meta-analysis reached statistical significance when the three polymorphisms were tested for association with SSc, the limited subtype (lcSSc) and anti-centromere auto-antibodies (ACAs). However, no significant P-values were obtained when the ACA-positive patients were removed from the SSc and lcSSc groups, suggesting that these associations rely on ACA positivity. The strongest association signal with ACA production was detected for rs2104286 (P(FDR)=2.07 × 10(-4), odds ratio=1.30 (1.14-1.47)). The associations of rs11594656 and rs12722495 were lost after conditioning to rs2104286, and allelic combination tests did not evidence a combined effect, indicating that rs2104286 best described the association between IL2RA and ACA presence in SSc.
