Summary Objectives To determine the ability of radiographic bone texture (BTX) parameters to quantify subchondral tibia sclerosis and to examine clinical relevance for assessing osteoarthritis (OA) ...progression. We examined the relationship between BTX parameters and each of (1) location-specific joint space width (JSW) JSW( x ) and minimum JSW (mJSW) of the affected compartment, and (2) knee alignment (KA) angle in knee radiographs of participants undergoing total knee arthroplasty (TKA). Design Digitized fixed-flexion knee radiographs were analyzed for run-length and topological BTX parameters in a subchondral region using an algorithm. Medial JSW( x ) was computed at x = 0.200, 0.225, 0.250 and 0.275 according to a coordinate system defined by anatomic landmarks. mJSW was determined for medial and lateral compartment lesions. KA angles were determined from radiographs using an anatomic landmark-guided algorithm. JSW measures and the magnitude of knee malalignment were each correlated with BTX parameters. Reproducibility of BTX parameters was measured by root-mean square coefficients of variation (RMSCV%). Results Run-length BTX parameters were highly reproducible (RMSCV% < 1%) while topological parameters showed poorer reproducibility (>5%). In TKA participants (17 women, 13 men; age: 66 ± 9 years; body mass index (BMI): 31 ± 6 kg m−2 ; WOMAC: 41.5 ± 16.1; Kellgren–Lawrence score mode: 4), reduced trabecular spacing (Tb.Sp) and increased free ends (FE) were correlated with decreased JSW after accounting for BMI, gender and knee malalignment. These relationships were dependent on site of JSW measurement. Conclusion High reproducibility in quantifying bone sclerosis using Tb.Sp and its significant relationship with JSW demonstrated potential for assessing OA progression. Increased trabecular FE and reduced porosity observed with smaller JSW suggest collapsing subchondral bone or trabecular plate perforation in advanced knee OA.
Summary Objectives (1) To investigate the reproducibility of computer-assisted measurements of knee alignment angle (KA) from digitized radiographs of osteoarthritis (OA) participants requiring total ...knee arthroplasty (TKA) and (2) to determine whether landmark choice affects the precision of KA measurements on radiographs. Methods Using a custom algorithm, femoral, central, and tibial measurement-guiding rules were interactively placed on digitized posteroanterior fixed-flexion knee radiographs by mouse control and positioned according to different anatomic landmarks. The angle subtended by lines connecting these guiding rules was measured by three readers to assess interobserver, intraobserver and experience–inexperience reproducibility. Test–retest reproducibility was evaluated with duplicate radiographs from a healthy cohort. Reproducibility was assessed using root-mean square coefficients of variation (RMSCV%). The Bland–Altman method was performed on data obtained from varying anatomic landmarks (confidence interval, CI = 95%). Results From 16 healthy and 30 TKA participants, reproducibility analyses revealed a high degree of intraobserver ( n = 38, RMSCV = 0.56%), interobserver ( n = 38, RMSCV = 0.72%), test–retest ( n = 16, RMSCV = 0.87%) and experience–inexperience ( n = 38, RMSCV = 0.73%) reproducibility with variances below 1%. Varying the orientation of tibial and femoral rules according to anatomic landmarks produced a difference that exceeded an a priori limit of agreement of −1.11° to +1.67°. Conclusion Our custom-designed software provides a robust method for measuring KAs within digitized knee radiographs. Although test–retest analyses were only performed in a healthy cohort, we anticipate a similar degree of reproducibility in an OA sample. A standardized set of anatomic landmarks employed for KA measurement is recommended since arbitrary selection of landmarks resulted in imprecise KA measurement even with a computer-assisted technique.
Мета: встановити причини високого рівня захворюваності на інфекційні хвороби з фекально-оральним механізмом на півдні України.
Методи дослідження: епідеміологічного аналізу, описово-аналітичний, ...вірусологічні, бактеріологічні, серологічні та статистичні.
Результати досліджень. При порівняльному вивченні проявів епідемічного процесу групи інфекційних хвороб з фекально-оральним механізмом передачі установлено еволюційні зміни та високий рівень його інтенсивності, як в Україні, так і в регіоні Північно-західного Причорномор’я, до якого відносять Миколаївську, Одеську та Херсонську області. Загалом на території України відбулось зменшення питомої ваги гострих кишкових інфекцій зі встановленим збудником у 1,2 рази, дизентерії –у 3 рази, збільшення – гострих кишкових інфекцій з невстановленим збудником та ротавірусної інфекції у 1,2 та у 1,8 разів відповідно. Відзначено вірогідно вищий (t=13,61; p <0,001) рівень захворюваності на ГКІ серед населення регіону (506,63±29,64) о/оооо у порівнянні із показниками у країні (268,03±11,39) о/оооо. Встановлено виражену тенденцію до спаду захворюваності на шигельоз, як у державі (з 4,31 о/оооо у 2010 р. до 1,97 о/оооо у 2018 р.), так і в регіоні (з 9,02 о/оооо у 2010 р. до 2,39 о/оооо у 2018 р.). У регіоні відзначено тенденцію до зросту захворюваності на сальмонельоз (з 17,60 о/оооо у 2010 р. до 20,28 о/оооо у 2018 р.), та стабільно низький рівень захворюваності на ієрсиніоз, як серед населення України (0,22±0,20) о/оооо, так і – регіону (0,31±0,04) о/оооо. Відбувався постійний зріст захворюваності на РВІ, як на території країни (з 14,71 о/оооо у 2010 р. до 31,30 о/оооо у 2018 р.), так і регіону (з 20,92 о/оооо у 2010 р. до 36,36 о/оооо у 2018 р.) з двома періодичними підйомами (у 2014 та 2017 рр.). В цей час в Одеській області відбулась зміна домінуючого генотипу ротавірусу (у 2013 р. (G4P8 – (58,80±4,87) %, у 2014 р. циркулювали три генетичних варіанти: G2P4 (32,0±4,59) %, G4P8 (31,0±4,56) % і G1P8 (30,1±4,52) %, у 2017 р. домінував G1P8 (66,7±4,91) %. На вказаній території постійно відбувається зміна домінуючих геноваріантів ротавірусів: G1P8, G3P8 та G4P8, що супроводжується циклічним (2-3 роки) підйомом захворюваності на РВІ серед дітей до 5 років (r=0,68; р<0,01), що пояснюється природним формуванням «сприйнятливого прошарку».
Висновки. У період 2010-2018 рр. відзначено зростання інтенсивності ЕП ГКІ як в Україні, так і у регіоні Північно-західного Причорномор’я, де питома вага захворюваності склала майже 1/5 частину від загальнодержавної і була достовірно вищою (p<0,001). На прикладі Одеській області показано етіологічну структуру ГКІ зі встановленим збудником, у дітей виявлено «витіснення» патогенних бактерійних збудників умовно-патогенними та ротавірусами. Комплексний підхід у діагностиці, а також застосування сучасних ПЛР-методів дослідження підвищує ефективність етіологічної діагностики збудників ГКІ. На прикладі регіону показано, що при застосуванні і бактеріологічного методу, і мультиплексної ПЛР етіологію ГКІ розшифрували у (89,78±2,36) % випадків, у тому числі у (18,66±1,74) % зразків були виявлені 2-3 збудники. Протягом періоду проведення дозорного епідеміологічного нагляду за РВІ на території Одеської області у хворих на ГКІ дітей виявлено 15 різних GP-комбінацій ротавірусів, з них чотири основних: G1P8, G3P8, G4P8 та G2P4. При домінуванні генотипу G1P8 епідемічний процес РВІ характеризувався стабільним перебігом, тоді як збільшення питомої частки G4P8 супроводжувалось вагомим підвищенням його інтенсивності. Для оптимізації епідеміологічного нагляду за інфекційними хворобами з фекально-оральним механізмом передачі вважаємо за потрібне введення на окремих територіях країни дозорного епідеміологічного нагляду з виділенням індикаторних популяцій (діти, особи похилого віку) та ділянок (територій), зі створенням оснащених референс-центрів.

Membrane permeability and P-glycoprotein (Pgp) can be limiting factors for blood-brain barrier penetration. The objectives of this study were to determine whether there are differences in the in ...vitro permeability, Pgp substrate profiles, and physicochemical properties of drugs for central nervous system (CNS) and non-CNS indications, and whether these differences are useful criteria in selecting compounds for drug development. Apparent permeability (P(app)) and Pgp substrate profiles for 93 CNS (n = 48) and non-CNS (n = 45) drugs were determined by monolayer efflux. Calcein-AM inhibition assays were used to supplement the efflux results. The CNS set (2 of 48, 4.2%) had a 7-fold lower incidence of passive permeability values <150 nm/s compared with the non-CNS set (13 of 45, 28.9%). The majority of drugs (72.0%, 67 of 93) were not Pgp substrates; however, 49.5% (46 of 93) were positive in the calcein-AM assay when tested at 100 microM. The CNS drug set (n = 7 of 48, 14.6%) had a 3-fold lower incidence of Pgp-mediated efflux than the non-CNS drug set (n = 19 of 45, 42.2%). Analysis of 18 physicochemical properties revealed that the CNS drug set had fewer hydrogen bond donors, fewer positive charges, greater lipophilicity, lower polar surface area, and reduced flexibility compared with the non-CNS group (p < 0.05), properties that enhance membrane permeability. This study on a large, diverse set of marketed compounds clearly demonstrates that permeability, Pgp-mediated efflux, and certain physicochemical properties are factors that differentiate CNS and non-CNS drugs. For CNS delivery, a drug should ideally have an in vitro passive permeability >150 nm/s and not be a good (B --> A/A --> B ratio <2.5) Pgp substrate.
The US Health and Human Services Pain Management Best Practices Inter-Agency Task Force initiated a public-private partnership which led to the publication of its report in 2019. The report ...emphasized the need for individualized, multimodal, and multidisciplinary approaches to pain management that decrease the over-reliance on opioids, increase access to care, and promote widespread education on pain and substance use disorders. The Task Force specifically called on specialty organizations to work together to develop evidence-based guidelines. In response to this report's recommendations, a consortium of 14 professional healthcare societies committed to a 2-year project to advance pain management for the surgical patient and improve opioid safety. The modified Delphi process included two rounds of electronic voting and culminated in a live virtual event in February 2021, during which seven common guiding principles were established for acute perioperative pain management. These principles should help to inform local action and future development of clinical practice recommendations.
Nivolumab plus ipilimumab produced objective responses in patients with advanced renal-cell carcinoma in a pilot study. This phase 3 trial compared nivolumab plus ipilimumab with sunitinib for ...previously untreated clear-cell advanced renal-cell carcinoma.
We randomly assigned adults in a 1:1 ratio to receive either nivolumab (3 mg per kilogram of body weight) plus ipilimumab (1 mg per kilogram) intravenously every 3 weeks for four doses, followed by nivolumab (3 mg per kilogram) every 2 weeks, or sunitinib (50 mg) orally once daily for 4 weeks (6-week cycle). The coprimary end points were overall survival (alpha level, 0.04), objective response rate (alpha level, 0.001), and progression-free survival (alpha level, 0.009) among patients with intermediate or poor prognostic risk.
A total of 1096 patients were assigned to receive nivolumab plus ipilimumab (550 patients) or sunitinib (546 patients); 425 and 422, respectively, had intermediate or poor risk. At a median follow-up of 25.2 months in intermediate- and poor-risk patients, the 18-month overall survival rate was 75% (95% confidence interval CI, 70 to 78) with nivolumab plus ipilimumab and 60% (95% CI, 55 to 65) with sunitinib; the median overall survival was not reached with nivolumab plus ipilimumab versus 26.0 months with sunitinib (hazard ratio for death, 0.63; P<0.001). The objective response rate was 42% versus 27% (P<0.001), and the complete response rate was 9% versus 1%. The median progression-free survival was 11.6 months and 8.4 months, respectively (hazard ratio for disease progression or death, 0.82; P=0.03, not significant per the prespecified 0.009 threshold). Treatment-related adverse events occurred in 509 of 547 patients (93%) in the nivolumab-plus-ipilimumab group and 521 of 535 patients (97%) in the sunitinib group; grade 3 or 4 events occurred in 250 patients (46%) and 335 patients (63%), respectively. Treatment-related adverse events leading to discontinuation occurred in 22% and 12% of the patients in the respective groups.
Overall survival and objective response rates were significantly higher with nivolumab plus ipilimumab than with sunitinib among intermediate- and poor-risk patients with previously untreated advanced renal-cell carcinoma. Summary Background Alemtuzumab is a humanised monoclonal antibody that depletes lymphocytes, causing long-term immunomodulation. In a 3-year, rater-blinded phase 2 study (the CAMMS223 study) in ...patients with relapsing-remitting multiple sclerosis (RRMS), alemtuzumab reduced relapse rate and the risk of sustained accumulation of disability compared with subcutaneous interferon beta-1a, and the mean expanded disability status scale (EDSS) score of the alemtuzumab cohort improved compared with baseline. Adverse events included infusion-associated reactions, predominantly mild to moderate infections, thyroid disorders, and immune thrombocytopenia. In this study, we further analysed the CAMMS223 data with the aim of determining whether demographic and baseline disease-related characteristics affect the beneficial effects of alemtuzumab. Additionally, we aimed to describe a new outcome measure in multiple sclerosis research: sustained reduction in disability. Methods 334 treatment-naive patients with active, early RRMS were randomly assigned in a 1:1:1 ratio to receive interferon beta-1a (44 μg subcutaneously three times per week), or 24 mg per day or 12 mg per day alemtuzumab intravenously for 2 or 3 annual cycles. We analysed freedom from clinical disease activity (CDA; defined as no relapses and no sustained accumulation of disability) and occurrence of sustained reduction in disability (SRD; a ≥1 point decrease on the EDSS sustained for 6 consecutive months for patients with a baseline EDSS ≥2), and analysed efficacy outcomes for subgroups based on age, sex, geographic region, MRI-T1 brain volume, MRI-T2 lesion volume, disease duration, number of previous relapses within 2 years, and EDSS. Findings 322 patients were analysed. 161 of 215 patients treated with alemtuzumab were free of CDA at 36 months (Kaplan-Meier estimate 71·8%, 95% CI 63·1–78·8%) compared with 52 of 107 patients treated with interferon beta-1a (42·6%, 32·4–52·4%; hazard ratio HR=0·31, 0·20–0·46; p<0·0001). For the 199 patients with a baseline EDSS score greater than or equal to 2, SRD was more likely (HR=2·61, 1·54–4·43; p=0·0004) among patients treated with alemtuzumab (66 of 133 patients, Kaplan-Meier estimate 51·6%, 95% CI 43·2–60·7%) than patients treated with interferon beta-1a (15 of 66 patients, 27·2%, 17·2–41·4%). All disability and relapse outcomes showed evidence of beneficial effects of alemtuzumab compared with interferon beta-1a across all analysed patient subsets, and no subgroup of patients consistently responded better than others to alemtuzumab. Interpretation Alemtuzumab reduced disease activity compared with interferon beta-1a in most of the analysed subgroups. Significantly greater numbers of patients experienced sustained improvement in disability after treatment with alemtuzumab than interferon beta-1a. The efficacy offered by alemtuzumab is a substantial advance in the treatment of multiple sclerosis. Funding Genzyme and Bayer Schering Pharma.
