Introduction La transplantation rénale améliore la survie et la qualité de vie des patients obèses en insuffisance rénale terminale (IRT). Peu de données sont disponibles sur l'impact de l'obésité ...sur l'accès à la transplantation rénale. L'objectif de cette étude est de décrire l'association entre le statut pondéral au moment de l'initiation de la dialyse (ou la variation d'indice de masse corporelle, IMC) et l'accès à la transplantation rénale chez des patients en IRT. L'objectif secondaire et d'analyser l'association entre l'IMC et la mortalité. Matériels et Méthodes 19 524 patients en IRT et dialysés en France issus du Réseau Épidémiologie et Information en Néphrologie (REIN) ont été inclus entre 2002 and 2011. L'IMC était collecté à l'inclusion et tous les ans lors du suivi. Résultats Les moyennes ± SD de l'âge et de l'IMC étaient de 54,9 ± 11,9 ans et 26,2 ± 6,1 kg/m2 , respectivement. La médiane de suivi était de 20,3 mois durant laquelle 6 634 patients ont été transplantés. 63 % des patients étaient des hommes, 23 % avaient débuté la dialyse avec un IMC ≥ 30 kg/m2 ; enfin, 1/3 étaient diabétiques. Les patients avec les IMC les plus élevés étaient plus âgés, diabétiques, avaient une insuffisance cardiaque congestive et étaient hémodialysés ( vs dialyse péritonéale). Un IMC > 32 kg/m2 à l'initiation de la dialyse était associé à une moindre chance d'être transplanté, avec un risque croissant avec l'élévation de l'IMC au-dessus de ce seuil. Cher les sujets obèses, pour chaque élévation d'1 kg/m2 durant le suivi, les chances d'accès à la greffe rénale étaient diminuées de 8 à 10 %. Pour un IMC entre 23-38 kg/m2 la mortalité était diminuée. Si l'IMC était > 38 kg/m2 , la mortalité augmentait mais seulement de l'ordre d'1 % pour 1 kg/m2. Conclusions Les patients obèses en IRT et dialysés ont une moindre chance d'accès à la greffe rénale en France, en comparaison à ceux de corpulence standard.
Abstract Background and aims Benefits of Mediterranean diet on MetS risk have been suggested, but overall prospective evidence in the general population is limited. For the first time, the ...prospective association of adherence to Mediterranean diet with the 6-y risk of MetS and its components was evaluated in a large cohort in Europe. Methods and results Subjects included were participants from the Supplémentation en Vitamines et Minéraux AntioXydants (SU.VI.MAX) study. Adherence to Mediterranean diet was assessed using traditional Mediterranean diet score (MDS), an updated Mediterranean score (MED) and Mediterranean style-dietary pattern score (MSDPS) calculated from at least three 24-h records. In 3232 subjects, the association between Mediterranean diet scores and 6-y risk of MetS was evaluated. The association between Mediterranean scores and MetS components was also estimated. A lower risk of MetS was observed with increasing MED score ( P -trend = 0.001) and MDS ( P -trend = 0.03) in multivariate models. The adjusted odds ratios (95% Confidence Interval) for MetS risk were 0.47 (0.32–0.69) and 0.50 (0.32–0.77) in subjects in the highest versus lowest tertile of MED score and MDS, respectively. The MED score was inversely associated with waist circumference, systolic blood pressure and triglycerides, and directly associated with HDL-cholesterol. The MDS was negatively associated with waist circumference and triglycerides, and MSDPS was positively associated with HDL-cholesterol. Conclusions All Mediterranean diet scores were associated in a potentially beneficial direction with components of MetS or MetS incidence. Our findings support that individuals should be encouraged to follow a Mediterranean dietary pattern for reduction of MetS risk. Trial Registration: clinicaltrials.gov Identifier: NCT00272428.
Introduction L’hormone antidiurétique (ou vasopressine) est élevée dans le diabète. Or, l’administration de vasopressine chez l’homme sain augmente transitoirement la glycémie, probablement par la ...stimulation des récepteurs V1a hépatiques. Nous avons montré, chez 3615 sujets initialement normoglycémiques de la cohorte française D.E.S.I.R. (Données Epidémiologiques sur le Syndrome d’Insulino-Résistance), une association négative entre consommation d’eau (qui freine la sécrétion de vasopressine), et survenue d’une hyperglycémie dans les 9 années de suivi, indépendante des facteurs de risque classiques et de la consommation des autres sortes de boissons (Roussel et al, Diabetes Care, 2011). Patients et méthodes Nous avons maintenant mesuré la concentration plasmatique de vasopressine (PVP) à l’inclusion chez 500 sujets de la cohorte D.E.S.I.R., tirés au sort. Des associations ont été recherchées entre PVP (log-transformée) et survenue dans les 9 années suivantes d’une hyperglycémie (HGly, glycémie à jeûn ≥ 6,1 mmol/L) ou d’un diabète (DM, glycémie à jeûn ≥ 7,0 mmol/L ou traitement du DM) par régression logistique tenant compte du sexe, de l’âge, et des facteurs de risque classiques à l’inclusion (IMC, glycémie, tabac, alcool, activité physique, triglycérides, cholesterol, HOMA-IR, gamma-GT, histoire familiale de diabète). Résultats Log-PVP était positivement corrélé à la densité urinaire (p = 0,002) et négativement à la consommation d’eau (p = 0,031) mais pas à la glycémie à l’inclusion. La survenue ultérieure d’HGly (n = 60) ou DM (n = 13) était significativement associée à Log-PVP en analyse univariée (p < 0,0001 pour HGly, p < 0,015 pour DM), et après ajustements: Odds Ratio IC 95 % pour une unité de Log-PVP = 1,85 1,26–2,70, p = 0,002 pour HGly, et 3,25 1,20–8,77, p = 0,02 pour DM. Conclusion Ces résultats confortent l’hypothèse d’une contribution de la vasopressine à l’élévation de la glycémie. Une faible consommation d’eau constitue donc probablement un nouveau facteur de risque pour le diabète, via l’augmentation de la sécrétion de vasopressine qui en résulte.
Abstract Background and aims The prevention of the metabolic syndrome (MetS) is of major concern and nutrition has been shown to modulate at least partly MetS risk. Our objective was to investigate ...whether a pro-inflammatory diet was associated with a higher risk of MetS and its components in a large cohort of French adults. Methods and results A total of 3726 participants from the Supplémentation en Vitamines et Minéraux AntioXydants (SU.VI.MAX) cohort were included in this study. The MetS status was identified at baseline and after 13 years of follow-up using self-reported medication, data from clinical investigations and biological measurements. The dietary inflammatory index (DII) was computed using repeated 24 h-dietary records (n = 10.1 ± 3.1). Logistic and linear regression analyses were conducted to assess the prospective association of the DII (as Q, quartiles) with the incidence of MetS and with the traits contributing to the MetS-definition (blood pressure, glycaemia, triglycerides, HDL-cholesterol, waist circumference). A diet with pro-inflammatory properties, as expressed by higher DII scores, was significantly associated with a higher risk of developing the MetS (OR comparing Q4 to Q1: 1.39, 95% confidence interval 1.01–1.92, P = 0.047). Moreover, higher DII scores were associated with higher systolic and diastolic blood pressure ( Ptrend across quartiles = 0.03 and 0.05, respectively) and triglycerides ( Ptrend = 0.01), and with lower HDL-cholesterol ( Ptrend = 0.03). Conclusion A higher DII score was prospectively associated with a higher risk of MetS, with associations with blood pressure, triglycerides and HDL-cholesterol. Promotion of a healthy diet exhibiting anti-inflammatory properties may contribute to prevent cardio-metabolic disorders.

Un système d’information nutritionnelle simplifiée à cinq couleurs (5-C) fondé sur le score des aliments de la Food Standards Agency (score FSA) a récemment été proposé dans le cadre ...des nouvelles propositions de santé publique nutritionnelle. Les objectifs de ce travail ont été d’évaluer la transférabilité du score FSA en cinq catégories au contexte français, ainsi que la performance, l’acceptabilité, la perception et la compréhension objective du système 5-C.
Matériel et méthodes
Le score FSA des aliments composant la table de composition alimentaire de l’étude Nutrinetsanté (N=3741) ainsi que pour les céréales du petit déjeuner, présentes dans une base de données ad hoc (N=380) a été calculé. Les aliments ont été catégorisés selon les groupes alimentaires du Programme National Nutrition Santé (PNNS). Un échantillon représentatif de la population française a été tiré au sort parmi les participants à l’étude Nutrinet-Santé (N=4225). L’apport énergétique de chaque aliment consommé par l’individu était utilisé pour calculer un score agrégé au niveau individuel pondéré par l’apport énergétique global (score FSA-individu). Un questionnaire spécifique portant sur l’acceptabilité, la perception et la compréhension objective de cinq formes de système d’information nutritionnelle, dont le 5-C a été développé et posé à un échantillon de la cohorte Nutrinet-Santé (N=13.578).
La catégorisation des groupes alimentaires du PNNS dans 5 classes de score FSA pour les aliments solides a été analysée. Les associations entre score FSA-individu et consommations alimentaires et catégories sociodémographiques et de mode de vie ont été étudiées. La performance du système 5-C a été étudiée dans la base de données des céréales du petit déjeuner. La modification des catégories de couleurs a été étudiée selon des scénarios de reformulation. La compréhension objective a été étudiée au travers de la capacité des individus à classer trois produits alimentaires selon leur qualité nutritionnelle, à partir des systèmes d’information nutritionnelle proposés.
Résultats
Le score FSA permettait de classer les groupes alimentaires en cinq classes cohérentes par rapport aux recommandations du PNNS. Par ailleurs, les cinq classes identifiées permettaient de différentier la qualité nutritionnelle des aliments au sein des grands groupes alimentaires. Le score FSA-individu permettait de caractériser la qualité nutritionnelle globale de l’alimentation. Enfin, l’application du système 5-C aux céréales du petit déjeuner affichait une bonne performance puisqu’elles étaient réparties dans au moins trois catégories de couleurs. La reformulation permettait des modifications significatives dans les catégories de couleur. Enfin, le 5-C apparaissait comme le système d’information nutritionnelle le plus simple à identifier, et le mieux compris parmi ceux étudiés.
Conclusion
Le système à cinq couleurs (5-C) fondé sur le score FSA semble adapté au contexte français, puisqu’il permet une bonne différentiation de la qualité nutritionnelle des aliments. Il semble perçu favorablement et bien compris par les utilisateurs.

Individual intakes of 264 different polyphenols (mg day
) were estimated using the Phenol-Explorer database and the mean of 6-17 24-h dietary records collected in 1994-1996. BMI in kg m
and WC in cm were measured in 1995-1996, 1998-1999 and 2001-2002. Linear mixed-effect models allowed for the assessment of longitudinal associations between energy-adjusted quartiles of total polyphenol intake as well as intake of 15 polyphenol classes and changes of these respective polyphenol classes in anthropometry over the 6 years of follow-up. Adjustment variables included sex, age, socio-economic status, lifestyle, dietary intakes and health status.
Participants in the highest quartile of intake of flavanones (BMI change: -0.28 (-0.43; -0.13), P=0.009), flavones (BMI change: -0.29 (-0.44; -0.14), P=0.008) and lignans (BMI change: -0.28 (-1.63; -0.09), P=0.01) experienced a less notable increase in BMI over time compared with their counterparts in the bottom quartile of intake of the respective polyphenol classes. Participants in the highest quartile of intake of flavanones (WC change: -1.39 (-2.02; -0.92), P=0.001), flavones (WC change: -1.57 (-2.32; -0.92), P=0.001), hydroxycinnamic acids (WC change: -1.27 (-1.92; -0.63), P=0.01), lignans (WC change: -1.16 (-1.80; -0.51), P=0.006) and total polyphenol intake (WC change: -1.39 (-2.05; -0.74), P=0.001) experienced a less notable increase in WC over time compared with their counterparts in the bottom quartile of intake of the respective polyphenols.
Dietary polyphenol intakes may help reduce weight gain over time in the general population. This could have important public health implications because moderate increases in BMI and WC over time have been shown to increase disease risk.
Early-life growth characteristics and in particular age at adiposity rebound (AR), have been shown to impact nutritional status later in life but studies investigating the association with long-term ...health remain scarce. Our aims were to identify determinants of age at AR and its relationship with nutritional status and cardiometabolic risk factors at adulthood.
A total of 1465 subjects aged 20-60 years participated in this retrospective cohort study. Height, weight, waist circumference, blood glucose, lipids and blood pressure were measured at adulthood. Childhood weight, height, gestational age, birth weight and early nutrition were collected retrospectively from health booklets and age at AR was assessed. Participants self-reported parental silhouettes. Associations were assessed using multiple linear and logistic regression.
An earlier AR was associated with higher body mass index and waist circumference at adulthood in both men and women (P<0.0001). In addition, women with an earlier occurrence of AR had higher triglyceride (P=0.001), low-density lipoprotein-cholesterol (P=0.001), systolic (P=0.02) and diastolic blood pressure (P=0.04) at adulthood. Both men (odds ratio (OR) (95% confidence interval (CI)): 0.82 (0.70-0.95)) and women (OR (95% CI): 0.84 (0.73-0.96) with an AR occurring earlier were more likely to develop a metabolic syndrome. Larger parental silhouette was associated with an earlier AR.
This long-term study showed that age at AR was associated with nutritional status and metabolic syndrome at adulthood. These results highlight the importance of monitoring childhood growth so as to help identify children at risk of developing an adverse cardiometabolic profile in adulthood. AR determinants for use in overweight surveillance were identified.
The relationship between diet quality and development of obesity is complex and unresolved. The aim of this study was to assess and compare the predictive value of six different dietary scores on ...both relative weight change and the risk of obesity after 13 years of follow-up in adults aged 45 years and older.
Six scores reflecting adherence to different nutritional recommendations (the French Programme National Nutrition Santé-Guideline Score (PNNS-GS), the Dietary Guidelines for Americans Index (DGAI), the Diet Quality Index-International (DQI-I), the Mediterranean Diet Scale (MDS), the relative Mediterranean Diet Score (rMED) and the Mediterranean Style Dietary Pattern Score (MSDPS)) were estimated in 3151 participants in the French SU.VI.MAX (SUpplémentation en VItamines et Minéraux AntioXydants) study. Associations of dietary scores with 13-year weight change were assessed through multivariate linear regression models, and obesity risk was analyzed with logistic regression, providing odds ratios (OR) and 95% confidence intervals (CI).
Except for the MSDPS, higher scores, that is, better adherence to nutritional guidelines or to a Mediterranean diet, were associated with lower weight gain in men (all P-value for trend <0.05). In addition, among men, ORs for becoming obese after 13 years associated with a 1 s.d. increase in dietary scores ranged from 0.63, 95% CI: 0.51, 0.78 for DGAI to 0.72, 95% CI: 0.59, 0.88 for MDS. These associations were weaker or not statistically significant in women.
Overall, the six dietary scores predicted obesity risk equally well. Among French adults, strong adherence to dietary guidelines appears to be protective with regard to weight gain and obesity, especially in men.
Objectif 1) Étudier les relations entre les biomarqueurs d’adiposité, d’inflammation et de fonction endothéliale et le développement du diabète de type 2, 2) Évaluer la pertinence d’inclure ces ...biomarqueurs dans un modèle de prédiction du diabète de type 2. Patients et méthodes 1 287 subjets participants à l’étude « SUpplémentation en VItamines et Minéraux AntioXydants », non diabétiques à l’inclusion et suivis pendant 13 ans ont été inclus dans cette étude. Les odds ratios (OR) et leurs intervalle de confiance à 95 % (IC 95 %) évaluant le risque de diabète incident associé à une augmentation de 1-SD des valeurs plasmatiques d’adiponectine, de leptine, de CRP, de sICAM-1, de sVCAM-1, de sélectine E et de MCP-1 ont été estimés. Les capacités de prédiction des modèles incluant les biomarqueurs individuellement ou en combinaison ont été comparés à un modèle comportant les facteurs de risque classiques par le biais de l’aire sous la courbe roc (AUC) et des IDI. Résultats 82 participants sont devenus diabétiques au terme du suivi. Le risque de développer le diabète diminuait avec l’augmentation de l’adiponectinémie (0.72 0.54–0.96) et augmentait en même temps que les valeurs plasmatiques de leptine (1.98 1.26–3.11), sICAM-1 (1.31 1.04–1.66) et CRP (1.28 1.01–1.63). Ces biomarqueurs prédisaient indépendamment la survenue du diabète lorsqu’ils étaient introduits simultanémente dans le modèle. Le modèle incluant les biomarqueurs améliorait significativement la prédiction de la survenue du diabète de type 2 en comparaison à un modèle comportant les facteurs de risque classiques (P < 0.05 pour les comparaisons de IDI). Conclusion Les biomarqueurs de l’adiposité, de l’inflammation et de la fonction endothéliale apparaissent comme des prédicteurs potentiels de la survenue du diabète de type 2. Leur inclusion au sein des scores de risque du diabète de type 2 doivent être considérés.

Knowledge about concurrent substance use and other risk behaviors - as well as their determinants - in the general population is insufficient.
Objectives
To investigated ...socio-demographic, health status, and lifestyle determinants of habit-forming risk behaviors among French men and women.
Methods
We analyzed data collected in 2021–2022 from 32,622 participants (74.5% female; mean age=57.9±14.2 years) of the NutriNet-Santé web-cohort who had completed the Alcohol Use Disorders Identification Test, the 12-item Cigarette Dependence Scale, the modified Yale Food Addiction Scale 2.0, and the Internet Addiction Test. Using established cutoff values, participants were first split into 2 groups (Yes/No) for each risk behavior (alcohol use disorders, nicotine dependence, food addiction, and Internet addiction) and then placed into 3 groups (no risk behavior, 1 risk behavior (reference), and ≥2 risk behaviors) before fitting polytomous logistic regression models according to sex.

To our knowledge, this is the first study to explore determinants of concurrent habit-forming risk behaviors among men and women in a large, population-based study. The findings could serve as impetus for future research in this domain and ultimately help guide addiction prevention efforts.
