Purpose
To determine diagnostic performance of MRI radiomics-based machine learning for classification of deep-seated lipoma and atypical lipomatous tumor (ALT) of the extremities.
Material and ...methods
This retrospective study was performed at three tertiary sarcoma centers and included 150 patients with surgically treated and histology-proven lesions. The training-validation cohort consisted of 114 patients from centers 1 and 2 (n = 64 lipoma, n = 50 ALT). The external test cohort consisted of 36 patients from center 3 (n = 24 lipoma, n = 12 ALT). 3D segmentation was manually performed on T1- and T2-weighted MRI. After extraction and selection of radiomic features, three machine learning classifiers were trained and validated using nested fivefold cross-validation. The best-performing classifier according to previous analysis was evaluated and compared to an experienced musculoskeletal radiologist in the external test cohort.
Results
Eight features passed feature selection and were incorporated into the machine learning models. After training and validation (74% ROC-AUC), the best-performing classifier (Random Forest) showed 92% sensitivity and 33% specificity in the external test cohort with no statistical difference compared to the radiologist (

= 0.474).
Conclusion
MRI radiomics-based machine learning may classify deep-seated lipoma and ALT of the extremities with high sensitivity and negative predictive value, thus potentially serving as a non-invasive screening tool to reduce unnecessary referral to tertiary tumor centers.
Background:
Although most breast masses in children are benign, breast cancer must be considered in the differential diagnosis. The majority are represented by sarcomas and secondary lesions. ...Literature reports only four cases of neuroblastoma breast metastasis, with no emphasis on radiologic features. Our work aims to furnish a description of radiologic and sonographic features of neuroblastoma metastasis in the breast.
Case description:
A 15-year-old girl had a round nodular mass in the outer upper quadrant of the left breast that had rapidly enlarged over the last month. An ultrasound showed two subcutaneous nodules (3.8 cm and 1.3 cm in maximum diameter), with an irregular shape, heterogeneous echogenicity (isohypoechoic), and hyperechoic foci with a posterior acoustic shadow inside. Overall, the features were highly suspicious of secondary malignant lesions. Computed tomographic scan was performed and found a large retroperitoneal mass and multiple mixed secondary lesions to the spine and hip. A 14G core needle biopsy of breast masses was performed and showed a secondary localization of neuroblastoma.
Conclusions:
In adolescents, metastases are the most frequent cause of malignant breast masses. Ultrasound examination should be preferred as the first imaging tool. For the differential diagnosis of breast metastasis with benign masses, a rapid enlargement, a heterogeneous echogenicity, and intralesional hyperechogenic foci could be considered features of malignancy.
Segmentation and image intensity discretization impact on radiomics workflow. The aim of this study is to investigate the influence of interobserver segmentation variability and intensity ...discretization methods on the reproducibility of MRI-based radiomic features in lipoma and atypical lipomatous tumor (ALT). Thirty patients with lipoma or ALT were retrospectively included. Three readers independently performed manual contour-focused segmentation on T1-weighted and T2-weighted sequences, including the whole tumor volume. Additionally, a marginal erosion was applied to segmentations to evaluate its influence on feature reproducibility. After image pre-processing, with included intensity discretization employing both fixed bin number and width approaches, 1106 radiomic features were extracted from each sequence. Intraclass correlation coefficient (ICC) 95% confidence interval lower bound ≥ 0.75 defined feature stability. In contour-focused vs. margin shrinkage segmentation, the rates of stable features extracted from T1-weighted and T2-weighted images ranged from 92.68 to 95.21% vs. 90.69 to 95.66% after fixed bin number discretization and from 95.75 to 97.65% vs. 95.39 to 96.47% after fixed bin width discretization, respectively, with no difference between the two segmentation approaches (
p
≥ 0.175). Higher stable feature rates and higher feature ICC values were found when implementing discretization with fixed bin width compared to fixed bin number, regardless of the segmentation approach (

< 0.001). In conclusion, MRI radiomic features of lipoma and ALT are reproducible regardless of the segmentation approach and intensity discretization method, although a certain degree of interobserver variability highlights the need for a preliminary reliability analysis in future studies.
Antarctic Bottom Water (AABW) supplies the lower limb of the global overturning circulation and ventilates the abyssal ocean. In recent decades, AABW has warmed, freshened and reduced in volume. Ross ...Sea Bottom Water (RSBW), the second largest source of AABW, has experienced the largest freshening. Here we use 23 years of summer measurements to document temporal variability in the salinity of the Ross Sea High Salinity Shelf Water (HSSW), a precursor to RSBW. HSSW salinity decreased between 1995 and 2014, consistent with freshening observed between 1958 and 2008. However, HSSW salinity rebounded sharply after 2014, with values in 2018 similar to those observed in the mid-late 1990s. Near-synchronous interannual fluctuations in salinity observed at five locations on the continental shelf suggest that upstream preconditioning and large-scale forcing influence HSSW salinity. The rate, magnitude and duration of the recent salinity increase are unusual in the context of the (sparse) observational record.
Summary
Objectives The role of androgen decline in the sexual activity of adult males is controversial. To clarify whether sexual function would benefit from testosterone (T) treatment in men with ...partially or severely reduced serum T levels, we conducted a systematic review and meta‐analysis of placebo‐controlled studies published in the past 30 years. The aim of this study was to assess and compare the effects of T on the different domains of sexual life.
Data source A comprehensive search of all published randomized clinical trials was performed in MEDLINE, the Cochrane Library, EMBASE and Current Contents databases.
Review methods Guided by prespecified criteria, software‐assisted data ion and quality assessed by two independent reviewers, a total of 17 randomized placebo‐controlled trials were found to be eligible. For each domain of sexual function we calculated the standardized mean difference relative to T and reported the results of pooled estimates of T treatment using the random effect model of meta‐analysis. Heterogeneity, reproducibility and consistency of the findings across studies were explored using sensitivity and meta‐regression analysis.
Results Overall, 656 subjects were evaluated: 284 were randomized to T, 284 to placebo (P) and 88 treated in cross‐over. The median study length was 3 months (range 1–36 months). Our meta‐analysis showed that in men with an average T level at baseline below 12 nmol/l, T treatment moderately improved the number of nocturnal erections, sexual thoughts and motivation, number of successful intercourses, scores of erectile function and overall sexual satisfaction, whereas T had no effect on erectile function in eugonadal men compared to placebo. Heterogeneity was explored by grouping studies according to the characteristics of the study population. A cut‐off value of 10 nmol/l for the mean T of the study population failed to predict the effect of treatment, whereas the presence of risk factors for vasculogenic erectile dysfunction (ED), comorbidities and shorter evaluation periods were associated with greater treatment effects in the studies performed in hypogonadal, but not in eugonadal, men. Meta‐regression analysis showed that the effects of T on erectile function, but not libido, were inversely related to the mean baseline T concentration. The meta‐analysis of available studies indicates that T treatment might be useful for improving vasculogenic ED in selected subjects with low or low‐normal T levels. The evidence for a beneficial effect of T treatment on erectile function should be tempered with the caveats that the effect tends to decline over time, is progressively smaller with increasing baseline T levels, and long‐term safety data are not available. The present meta‐analysis highlights the need, and pitfalls, for large‐scale, long‐term, randomized controlled trials to formally investigate the efficacy of T replacement in symptomatic middle‐aged and elderly men with reduced T levels and ED.
Summary
Objectives Ageing in men is associated with a gradual decline in serum testosterone levels and a concomitant loss of muscle mass, accumulation of central adiposity, impaired mobility and ...increased risk of bone fractures. Whether androgen treatment might be beneficial in these subjects is still under debate. We have carried out a systematic review of randomized controlled trials (RCTs) evaluating the effects of testosterone (T) administration to middle‐aged and ageing men on body composition, muscle strength, bone density, markers of bone metabolism and serum lipid profile.
Data source A comprehensive search of all published randomized clinical trials was performed using the MEDLINE, Cochrane Library, EMBASE and Current Contents databases.
Review methods Guided by prespecified criteria, software‐assisted data ion and quality assessed by two independent reviewers, 29 RCTs were found to be eligible. For each investigated variable, we reported the results of pooled estimates of testosterone treatment using the random effect model of meta‐analysis. Heterogeneity, reproducibility and consistency of the findings across studies were explored using sensitivity and meta‐regression analysis.
Results Overall, 1083 subjects were evaluated, 625 randomized to T, 427 to placebo and 31 to observation (control group). Weighted mean age was 64·5 years (range 49·9–77·6) and mean serum testosterone was 10·9 nmol/l (range 7·8–19). Testosterone treatment produced: (i) a reduction of 1·6 kg (CI: 2·5–0·6) of total body fat, corresponding to −6·2% (CI: 9·2–3·3) variation of initial body fat, (ii) an increase in fat free mass of 1·6 kg (CI: 0·6–2·6), corresponding to +2·7% (CI: 1·1–4·4) increase over baseline and (iii) no change in body weight. The effects of T on muscle strength were heterogeneous, showing a tendency towards improvement only at the leg/knee extension and handgrip of the dominant arm (pooled effect size = 0·3 standard mean difference (SMD), CI: −0·0 to 0·6). Testosterone improved bone mineral density (BMD) at the lumbar spine by +3·7% (CI: 1·0–6·4%) compared to placebo, but not at the femoral neck, and produced a consistent reduction in bone resorption markers (pooled effect size = −0·6 SMD, CI: −1·0 to −0·2). Testosterone also reduced total cholesterol by 0·23 mmol/l (CI: −0·37 to −0·10), especially in men with lower baseline T concentrations, with no change in low density lipoprotein (LDL)‐cholesterol. A significant reduction of high density lipoprotein (HDL)‐cholesterol was found only in studies with higher mean T‐values at baseline (−0·085 mmol/l, CI: −0·017 to −0·003). Sensitivity and meta‐regression analysis revealed that the dose/type of T used, in particular the possibility of aromatization, explained the heterogeneity in findings observed on bone density and HDL‐cholesterol among studies.
Conclusion The present analysis provides an estimate of the average treatment effects of testosterone therapy in middle‐aged men. Our findings are sufficiently strong to justify further interventional studies focused on alternative targets of androgenic treatment carrying more stringent clinical implications, in particular the cardiovascular, metabolic and neurological systems.
Assessing the uncertainty of precipitation measurements is a challenging problem because precipitation estimates are inevitably influenced by various errors and environmental conditions. A way to ...characterize the error structure of coincident measurements is to use the triple colocation (TC) statistical method. Unlike more typical approaches, where measures are compared in pairs and one of the two is assumed error‐free, TC has the enviable advantage to succeed in characterizing the uncertainties of co‐located measurements being compared to each other, without requiring the knowledge of the true value which is often unknown. However, TC requires to have at least three co‐located measuring systems and the compliance with several initial assumptions. In this work, for the first time, TC is applied to in‐situ measurements of rain precipitation acquired by three co‐located devices: a weighing rain gauge, a laser disdrometer and a bidimensional video disdrometer. Both parametric and nonparametric formulations of TC are implemented to derive the rainfall product precision associated with the three devices. While the parametric TC technique requires tighter constraints and explicit assumptions which may be violated causing some artifacts, the nonparametric formulation is more flexible and requires less strict constrains. For this reason, a comparison between the two TC formulations is also presented to investigate the impact of TC constrains and their possible violations. The results are obtained using a statistically robust dataset spanning a 1.5 year period collected in Switzerland and presented in terms of traditional metrics. According to triple colocation analysis, the two disdrometers outperform the classical weighing rain gauge and they have similar measurement error structure regardless of the integration time intervals.
This work aims to provide error estimates of three in‐situ precipitation sources applying the triple colocation statistical method. The latter has the enviable advantage to succeed in characterizing the uncertainties of co‐located measurements being compared each other, without requiring the knowledge of the true value, which is often unknown. This analysis suggests that the actual error assigned to the devices might be much larger than that estimated by the traditional dual colocation method.
It is widely known that precipitation is a key variable of the hydrological cycle that is strongly affected by recent climate changes. Therefore, there is a growing interest in research activities ...focused on alteration of rainfall regime, as it conditions the planning of countermeasures against flood and landslide hazards. The available literature about precipitation tendencies over Italian peninsula offers a limited number of studies about recent changes of extreme events and precipitation intensity. This work aims at adding a contribution to fill this research gap, investigating the changes in rainfall regime observed over the 2002–2021 period in the Campania region (southern Italy). To pursue this aim, a dataset including daily precipitation records collected at 107 stations was analysed both through 11 indices developed by the Expert Team on Climate Change Detection and Indices and through the Standardized Precipitation Index in order to detect signals of changes in extreme events and to assess tendencies towards drier or wetter conditions. The Theil‐Sen method and the Mann–Kendall non‐parametric test were employed to evaluate the trends and their statistical significance. The main results emerging from this work are (i) an increasing tendency in precipitation intensity and in the frequency of occurrence of heavy rainfall events in autumn, mainly in the northern part of the region and in the mountainous areas, (ii) an upward trend of the duration of the longest wet spell in the coastal areas and (iii) an increasing trend of dry spells in spring and in summer in the Gulf of Salerno.
Recent changes in Campania (southern Italy) rainfall regime were analyzed through 11 extreme precipitation indices. The main results are:
an increasing trend in precipitation intensity and in the frequency of occurrence of heavy rainfall events in autumn season in the northern sector of region and in the mountainous areas,
a positive tendency of the duration of the longest wet spell on the coastal areas, and
an increasing trend of dry spells in spring and in summer in the Gulf of Salerno.
Nowadays, laser disdrometers constitute a very appealing tool for measuring surface precipitation properties, by virtue of their capability to estimate not only the rainfall amount and intensity, but ...also the number, the size and the velocity of falling drops. However, disdrometric measures are affected by various sources of error being some of them related to environmental conditions. This work presents an assessment of Thies Clima laser disdrometer performance with a focus on the relationship between wind and the accuracy of the disdrometer output products. The 10-min average rainfall rate and total rainfall accumulation obtained by the disdrometer are systematically compared with the collocated measures of a standard tipping bucket rain gauge, the FAK010AA sensor, in terms of familiar statistical scores. A total of 42 rainy events, collected in a mountainous site of Southern Italy (Montevergine observatory), are used to support our analysis. The results show that the introduction of a new adaptive filtering in the disdrometric data processing can reduce the impact of sampling errors due to strong winds and heavy rain conditions. From a quantitative perspective, the novel filtering procedure improves by 8% the precipitation estimates with respect to the standard approach widely used in the literature. A deeper examination revealed that the signature of wind speed on raw velocity-diameter spectrographs gradually emerges with the rise of wind strength, thus causing a progressive increase of the wrongly allocated hydrometeors (which reaches 70% for wind speed greater than 8 m s−1). With the aid of reference rain-gauge rainfall data, we designed a second simple methodology that makes use of a correction factor to mitigate the wind-induced bias in disdrometric rainfall estimates. The resulting correction factor could be applied as an alternative to the adaptive filtering suggested by this study and may be of practical use when dealing with disdrometric data processing.
The transport sector and road infrastructures are very sensitive to the issues connected to the atmospheric conditions. The latter constitute a source of relevant risk, especially for roads running ...in mountainous areas, where a wide spectrum of meteorological phenomena, such as rain showers, snow, hail, wind gusts and ice, threatens drivers’ safety. In such contexts, to face out critical situations it is essential to develop a monitoring system that is able to capillary surveil specific sectors or very small basins, providing real time information that may be crucial to preserve lives and assets. In this work, we present the results of the “Campania Region Meteorological Radar Network”, which is focused on the development of X-band radar-based meteorological products that can support highway traffic management and maintenance. The X-band measurements provided by two single-polarization systems, properly integrated with the observations supplied by disdrometers and conventional automatic weather stations, were involved in the following main tasks: (i) the development of a radar composite product; (ii) the devise of a probability of hail index; (iii) the real time discrimination of precipitation type (rain, mixed and snow); (iv) the development of a snowfall rate estimator. The performance of these products was assessed for two case studies, related to a relevant summer hailstorm (which occurred on 1 August 2020) and to a winter precipitation event (which occurred on 13 February 2021). In both cases, the X-band radar-based tools proved to be useful for the stakeholders involved in the management of highway traffic, providing a reliable characterization of precipitation events and of the fast-changing vertical structure of convective cells.
