Many sex partners of persons with gonorrhea or chlamydial infections are not treated, which leads to frequent reinfections and further transmission.
We randomly assigned women and heterosexual men ...with gonorrhea or chlamydial infection to have their partners receive expedited treatment or standard referral. Patients in the expedited-treatment group were offered medication to give to their sex partners, or if they preferred, study staff members contacted partners and provided them with medication without a clinical examination. Patients assigned to standard partner referral were advised to refer their partners for treatment and were offered assistance notifying partners. The primary outcome was persistent or recurrent gonorrhea or chlamydial infection in patients 3 to 19 weeks after treatment.
Persistent or recurrent gonorrhea or chlamydial infection occurred in 121 of 931 patients (13 percent) assigned to standard partner referral and 92 of 929 (10 percent) assigned to expedited treatment of sexual partners (relative risk, 0.76; 95 percent confidence interval, 0.59 to 0.98). Expedited treatment was more effective than standard referral of partners in reducing persistent or recurrent infection among patients with gonorrhea (3 percent vs. 11 percent, P=0.01) than in those with chlamydial infection (11 percent vs. 13 percent, P=0.17) (P=0.05 for the comparison of treatment effects) and remained independently associated with a reduced risk of persistent or recurrent infection after adjustment for other predictors of infection at follow-up (relative risk, 0.75; 95 percent confidence interval, 0.57 to 0.97). Patients assigned to expedited treatment of sexual partners were significantly more likely than those assigned to standard referral of partners to report that all of their partners were treated and significantly less likely to report having sex with an untreated partner.
Expedited treatment of sex partners reduces the rates of persistent or recurrent gonorrhea or chlamydial infection.
Since the discovery of the secondary preventive benefits of antiretroviral therapy, national and international governing bodies have called for countries to reach 90% diagnosis, ART engagement and ...viral suppression among people living with HIV/AIDS. The US HIV epidemic is dispersed primarily across large urban centers, each with different underlying epidemiological and structural features. We selected six US cities, including Atlanta, Baltimore, Los Angeles, Miami, New York, and Seattle, with the objective of demonstrating the breadth of epidemiological and structural differences affecting the HIV/AIDS response across the US. We synthesized current and publicly-available surveillance, legal statutes, entitlement and discretionary funding, and service location data for each city. The vast differences we observed in each domain reinforce disparities in access to HIV treatment and prevention, and necessitate targeted, localized strategies to optimize the limited resources available for each city’s HIV/AIDS response.
The Golden (British Columbia, Canada) meteorite fall occurred on October 4, 2021 at 0534 UT with the first recovered fragment (1.3 kg) landing on an occupied bed. The associated fireball was recorded ...by numerous cameras permitting reconstruction of its trajectory and orbit. The fireball entered the atmosphere at a 54° angle from the horizontal at a speed of 18 km s−1. The fireball reached a peak brightness of −14, having first become luminous at a height of >84 km and ending at 18 km altitude. Analysis of the infrasonic record of the bolide produced an estimated mass of 78−65+157 kg while modeling of the fireball light curve suggests an initial mass near 70 kg. The fireball experienced a major flare near 31 km altitude where more than half its mass was lost in the form of dust and gram‐sized fragments under a dynamic pressure of 3.3 MPa. The strength and fragmentation behavior of the fireball were similar to those reported for other meteorite‐producing fireballs (Borovička et al., 2020). Seven days after the fireball occurred, an additional 0.9 kg fragment was recovered during the second day of dedicated searching guided by initial trajectory and dark flight calculations. Additional searching in the fall and spring of 2021–2022 located no additional fragments. The meteorite is an unbrecciated, low‐shock (S2) ordinary chondrite of intermediate composition, typed as an L/LL5 with a grain density of ~3530 k gm−3, an average bulk density of 3150 kg m−3 and calculated porosity of ~10%. From noble gas measurements, the cosmic ray exposure age is 25 ± 4 Ma while gas retention ages are all >2 Ga. Short‐lived radionuclides and noble gas measurements of the pre‐atmospheric size overlap with estimates from infrasound and light curve modeling producing a preferred pre‐atmospheric mass of 70–200 kg. The orbit of Golden has a high inclination (23.5°) and is consistent with delivery from the inner main belt. The highest probability (60%) of an origin is from the Hungaria group. We propose that Golden may originate among the background S‐type asteroids found interspersed in the Hungaria region. The current collection of 18 L/LL—chondrite orbits shows a strong preference for origins in the inner main belt, suggesting multiple parent bodies may be required to explain the diversity in CRE ages and shock states.
This study examined factors impacting the stability of biodiesel (B100) samples collected as part of a 2004 nationwide fuel quality survey in the United States. Biodiesel is significantly less stable ...than petroleum diesel, so an understanding of the chemical and environmental factors affecting its degradation is required. The survey included samples produced from soy, waste oils, and tallow. The 27 samples were assessed for stability using the ASTM D2274 test for insoluble formation and the OSI method (via Rancimat apparatus) for induction time. Additionally, the samples were characterized for fatty acid make up, relative antioxidant content, metals content, and total glycerin content (free glycerin plus glycerin bound as mono-, di-, and triglycerides). For the samples examined here the polyunsaturated content (or oxidizability) has the largest impact on both increasing insoluble formation and reducing induction time. However, the formation of insolubles is also measurably decreased by increasing relative antioxidant content and increased by increasing total glycerin content. The OSI or Rancimat induction time is also increased by increasing relative antioxidant content, as expected.
This roadmap on Nanotechnology for Catalysis and Solar Energy Conversion focuses on the application of nanotechnology in addressing the current challenges of energy conversion: 'high efficiency, ...stability, safety, and the potential for low-cost/scalable manufacturing' to quote from the contributed article by Nathan Lewis. This roadmap focuses on solar-to-fuel conversion, solar water splitting, solar photovoltaics and bio-catalysis. It includes dye-sensitized solar cells (DSSCs), perovskite solar cells, and organic photovoltaics. Smart engineering of colloidal quantum materials and nanostructured electrodes will improve solar-to-fuel conversion efficiency, as described in the articles by Waiskopf and Banin and Meyer. Semiconductor nanoparticles will also improve solar energy conversion efficiency, as discussed by Boschloo et al in their article on DSSCs. Perovskite solar cells have advanced rapidly in recent years, including new ideas on 2D and 3D hybrid halide perovskites, as described by Spanopoulos et al 'Next generation' solar cells using multiple exciton generation (MEG) from hot carriers, described in the article by Nozik and Beard, could lead to remarkable improvement in photovoltaic efficiency by using quantization effects in semiconductor nanostructures (quantum dots, wires or wells). These challenges will not be met without simultaneous improvement in nanoscale characterization methods. Terahertz spectroscopy, discussed in the article by Milot et al is one example of a method that is overcoming the difficulties associated with nanoscale materials characterization by avoiding electrical contacts to nanoparticles, allowing characterization during device operation, and enabling characterization of a single nanoparticle. Besides experimental advances, computational science is also meeting the challenges of nanomaterials synthesis. The article by Kohlstedt and Schatz discusses the computational frameworks being used to predict structure-property relationships in materials and devices, including machine learning methods, with an emphasis on organic photovoltaics. The contribution by Megarity and Armstrong presents the 'electrochemical leaf' for improvements in electrochemistry and beyond. In addition, biohybrid approaches can take advantage of efficient and specific enzyme catalysts. These articles present the nanoscience and technology at the forefront of renewable energy development that will have significant benefits to society.

Production of pure and high-yield client proteins is an important technology that addresses the need for industrial applications of enzymes as well as scientific experiments in protein chemistry and crystallization. Fungi are utilized in industrial protein production because of their ability to secrete large quantities of proteins. In this study, we engineered a high-expression-secretion vector, pEXPYR that directs proteins towards the extracellular medium in two Aspergillii host strains, examine the effect of maltose-induced over-expression and protein secretion as well as time and pH-dependent protein stability in the medium. We describe five client proteins representing a core set of hemicellulose degrading enzymes that accumulated up to 50–100mg/L of protein. Using a recyclable genetic marker that allows serial insertion of multiple genes, simultaneous hyper-secretion of three client proteins in a single host strain was accomplished.

We explore the relation between diffuse intracluster light (central galaxy included) and the galaxy cluster (baryonic and dark) matter distribution using a sample of 528 clusters at 0.2 ≤ z ...≤ 0.35 found in the Dark Energy Survey (DES) Year 1 data. The surface brightness of the diffuse light shows an increasing dependence on cluster total mass at larger radius, and appears to be self-similar with a universal radial dependence after scaling by cluster radius. We also compare the diffuse light radial profiles to the cluster (baryonic and dark) matter distribution measured through weak lensing and find them to be comparable. The IllustrisTNG galaxy formation simulation, TNG300, offers further insight into the connection between diffuse stellar mass and cluster matter distributions – the simulation radial profile of the diffuse stellar component does not have a similar slope with the total cluster matter content, although that of the cluster satellite galaxies does. Regardless of the radial trends, the amount of diffuse stellar mass has a low-scatter scaling relation with cluster’s total mass in the simulation, out-performing the total stellar mass of cluster satellite galaxies. We conclude that there is no consistent evidence yet on whether or not diffuse light is a faithful radial tracer of the cluster matter distribution. Nevertheless, both observational and simulation results reveal that diffuse light is an excellent indicator of the cluster’s total mass.
Psoriasis is a chronic inflammatory disease of the skin and joints that may also have systemic inflammatory effects, including the development of cardiovascular disease (CVD). Multiple epidemiologic ...studies have demonstrated increased rates of CVD in psoriasis patients, although a causal link has not been established. A growing body of evidence suggests that sub-clinical systemic inflammation may develop in psoriasis patients, even from a young age. We aimed to evaluate the prevalence of atherosclerosis and identify specific clinical risk factors associated with early vascular inflammation.
We conducted a cross-sectional study of a tertiary care cohort of psoriasis patients using coronary artery calcium (CAC) score and carotid intima-media thickness (CIMT) to detect atherosclerosis, along with high sensitivity C-reactive protein (hsCRP) to measure inflammation. Psoriasis patients and controls were recruited from our tertiary care dermatology clinic. Presence of atherosclerosis was defined using validated numeric values within CAC and CIMT imaging. Descriptive data comparing groups was analyzed using Welch's t test and Pearson Chi square tests. Logistic regression was used to analyze clinical factors associated with atherosclerosis, and linear regression to evaluate the relationship between psoriasis and hsCRP.
296 patients were enrolled, with 283 (207 psoriatic and 76 controls) having all data for the hsCRP and atherosclerosis analysis. Atherosclerosis was found in 67.6 % of psoriasis subjects versus 52.6 % of controls; Psoriasis patients were found to have a 2.67-fold higher odds of having atherosclerosis compared to controls 95 % CI (1.2, 5.92); p = 0.016, after adjusting for age, gender, race, BMI, smoking, HDL and hsCRP. In addition, a non-significant trend was found between HsCRP and psoriasis severity, as measured by PASI, PGA, or BSA, again after adjusting for confounders.
A tertiary care cohort of psoriasis patients have a high prevalence of early atherosclerosis, increased hsCRP, and psoriasis remains a risk factor for the presence of atherosclerosis even after adjustment of key confounding clinical factors. Psoriasis may contribute to an accelerated systemic inflammatory cascade resulting in increased risk of CVD and CV events.
The effect of saturated monoglyceride (SMG) content of four B100s on the cloud point (CP) of blends with four diesel fuels was examined. Detecting CP with a more sensitive light-scattering method ...allowed observation of an early (higher temperature) CP in blends containing approximately 0.01 wt.% to 0.03 wt.% SMG. Blend samples with SMG content in this range may be particularly prone to unexpected filter clogging above the measured CP. Results for a 140 blend sample matrix revealed that SMG content had a larger effect on CP than other blend properties. An increase of 0.01 wt.% SMG in a biodiesel blend increased CP by as much as 4 degree C. At a constant SMG level, increasing biodiesel content lowered CP, as did increasing the diesel fuel aromatic content, by improving the solubility of SMG. This implies that lowering the SMG content of a B100 allows preparation of higher biodiesel content blends having the same or lower CP. Increasing the unsaturated monoglyceride-to-SMG ratio by blending in monoolein lowered CP, presumably because monoolein inhibits nucleation of SMG. In most blends with SMG content above 0.01 wt.%, polymorphic phase transformation of crystallized SMG (converting from the metastable alpha -form to the less soluble, stable beta -form) was observed.

We characterize the properties and evolution of bright central galaxies (BCGs) and the surrounding intracluster light (ICL) in galaxy clusters identified in the Dark Energy Survey and ...Atacama Cosmology Telescope Survey (DES-ACT) overlapping regions, covering the redshift range 0.20 < z < 0.80. Over this redshift range, we measure no change in the ICL’s stellar content (between 50 and 300 kpc) in clusters with log10(M200m,SZ/M⊙) >14.4. We also measure the stellar mass–halo mass (SMHM) relation for the BCG+ICL system and find that the slope, β, which characterizes the dependence of M200m,SZ on the BCG+ICL stellar mass, increases with radius. The outskirts are more strongly correlated with the halo than the core, which supports that the BCG+ICL system follows a two-phase growth, where recent growth (z < 2) occurs beyond the BCG’s core. Additionally, we compare our observed SMHM relation results to the IllustrisTNG300-1 cosmological hydrodynamic simulations and find moderate qualitative agreement in the amount of diffuse light. However, the SMHM relation’s slope is steeper in TNG300-1 and the intrinsic scatter is lower, likely from the absence of projection effects in TNG300-1. Additionally, we find that the ICL exhibits a colour gradient such that the outskirts are bluer than the core. Moreover, for the lower halo mass clusters (log10(M200m,SZ/M⊙) < 14.59), we detect a modest change in the colour gradient’s slope with lookback time, which combined with the absence of stellar mass growth may suggest that lower mass clusters have been involved in growth via tidal stripping more recently than their higher mass counterparts.
