Modern weapons cause severe damage, accompanied by high rates of complications and mortality. The investigation of such kinds of weapons is in high demand considering the ongoing active phase of ...russia's war against Ukraine since February 2022. In order to understand the pathological processes that occur in and outside the gunshot wound, we conducted an experimental study using mathematical simulation. The results presented in the article will help to choose the appropriate surgical management and improve the results of treatment. The aim of this study was to investigate and evaluate the damaging effect of a 5.45 mm 7N6M bullet and a 5.45 mm V-max expansive bullet using numerical modeling of wound canals in ballistic plasticine. The Ansys Explicit Dynamics engineering complex was used to simulate the dynamics of the bullet’s motion. The basic equations, solved by the explicit dynamic analysis, express the conservation of mass, momentum, and energy in Lagrange coordinates. Together with the material model and the set of initial and boundary conditions, they determine the complete solution to the problem. Taking into account that the initial velocity of the bullet is 1185 m/s at a mass of 3.9 g, we obtain energy 2740 J. All this energy acts at the area of the wound canal with a depth of 150 mm. Injury with a conventional 7N6M bullet is characterized by the fact that it passes through the block and loses only part of the kinetic energy. The simulation results showed that the velocity of the bullet at the outlet is 220 m/s. Taking into account the initial velocity of the bullet 918 m/s with a mass of 3.4 g, we obtain the kinetic energy acting on the walls of the wound canal with a depth of 200 mm of about 830 J. Mathematic analyses showed that the expansive bullet has a soft core that deforms and transfers all the kinetic energy to the tissues immediately after penetration into the tissues. The loss of kinetic energy of the bullet (ΔE, J) is defined as the difference between the kinetic energy at the time of injury (Ec, J) and the residual energy of the bullet when leaving the material (Er, J). Numerical modeling of wound ballistics in biological tissue simulators allows us to determine with high accuracy the features of wound canal formation and tissue response to damage of bullets having different kinetic energy, which contributes to the choice of adequate surgical management during surgery for gunshot wounds.
Актуальність. Сучасний стан військово-лікарської експертизи осіб, що призиваються до лав Збройних Сил України (ЗС України) не відповідає вимогам воєнного часу і ґрунтується на наказі Міністерства ...Оборони України № 402 від 14.08.2008. Проведення даного виду експертизи на даний час проводиться дерматологом, без використання актуальних методів діагностики, що потенційно збільшує ризик виявлення серйозних захворювань шкіри в лавах ЗС України і суттєво зменшує боєздатність та підвищує матеріальні витрати.
Метою даної роботи є пошук у сучасній літературі та порівняння сучасних рекомендацій із подібними рекомендаціями розвинутих країн, що містять прості та досить відтворювані інструкції щодо проведення військово-лікарської експертизи і мають більш широкий допуск до військової служби пацієнтів з новоутвореннями шкіри.
Матеріали та методи. З використанням методів систематичного, бібліосемантичного та ретроспективного аналізу вивчено наукові публікації та керівні документи з залученням електронних баз даних MEDLINE/PubMed, EMBASE/Excerpta Medica, Cochrane Library, PubMed та Google Scholar. Глибиною в 15 років було проаналізовано 10 публікацій, які і увійшли в наш огляд щодо переліку захворювань та станів шкіри, методів обстеження і діагностики, регламентуючі документи (накази) медичної служби ЗС України та ЗС Німеччини з метою визначення ступеню придатності до військової служби, носіння військового одягу, взуття та спеціального спорядження.
Результати. Проаналізовані регламентуючі документи не дозволяють провести ефективну експертизу дерматологічних захворювань, і ґрунтуються на застарілих даних. Констатовано відсутність номенклатури умов клінічних, лабораторних та морфологічних ознак які мають враховуватись кожним спеціалістом, що проводять дерматологічну експертизу конкретної особи. Проведено попередній аналіз літературних даних щодо визначення професійної придатності призовників та військовослужбовців ЗС України з доброякісними та злоякісними новоутвореннями шкіри. Визначено комплекс обстежень, застосування яких забезпечує військово-лікарську експертизу кандидатів на військову службу та допуск до служби вже діючих військовослужбовців.
Висновки. Встановлено, що сучасна військово-лікарська експертиза дерматологічних хворих потребує реформування із врахуванням досягнень дерматології і тенденцій у військовій медицині розвинутих країн. Показано, що дійсні на сьогодні накази та регламентуючі документи не відповідають сучасним вимогам щодо проведення якісної військово-лікарської експертизи пацієнтів з хворобами шкіри та підшкірної клітковини. Наявні регламентуючі документи перш за все вимагають корекції, яка полягає у чіткому визначенні станів та захворювань у осіб, яким проводиться військово-лікарська експертиза з приводу хвороб шкіри та підшкірної клітковини, зокрема онкодерматології.
Background. Malignant neoplasms of the skin are fairly common tumors in the world population and among the population of Ukraine. The main method of diagnosing skintumors is a biopsy, which allows ...establishing a diagnosis at an early stage and ensures the cure of most patients.
Purpose. To evaluate modern recommendations for skin biopsy in the diagnosis of dermato-onсological diseases.
Materials and methods. The search for sources of information was conducted using the MEDLINE/PubMed, EMBASE/ExcerptaMedica, CochraneLibrary, PubMed та Google Scholar databases using the following keywords: skin biopsy, skin tumors, diagnosis, melanoma. Among the identified sources, works without statistical analysis, descriptions of individual cases, articles without conclusions, and sources with duplicate results were excluded. The search depth was 10 years.
Results. As a result of the conducted search, 57 publications were found that corresponded to the declared purpose. The most common methods are: puncture, shaving, excisional and incisional biopsy. Most guidelines recommend full-thickness excisional biopsy as the preferred procedure for the diagnosis of suspected melanoma. It is indicated that a statistically significant mortality rate was found in the puncture biopsy group. Most observations showed no significant differences in melanoma recurrence between excisional biopsy and puncture groups. Given the clinical diversity of melanoma, there is no uniformity in the types of biopsies performed to diagnose melanoma. The most inaccurate method turned out to be the punch biopsy method, which is associated with an increased risk of underdiagnosis of melanoma.
Conclusions. A skin biopsy is a mandatory first step to establish a definitive diagnosis of a skin tumor. Excisional complete biopsy is the most justified in most cases of diagnosis. Rational biopsy technique remains an issue that needs further study.
There was made the analysis of wound ballistics of modern expansive bullets in comparison with shell bullets on 25 blocks of ballistic plasticine (ROMA PLASTILINA No. 1, Ballistic Testing Backing ...Material), made in the USA, in which one shot from an AKS-74 automatic firearm and carbine ZBROYAR Z-10 with an optical sight was fired. The bullet speed was the highest in 5.45x39 caliber cartridge with a V-max bullet. – 1185 m/s, low in the bullet caliber cartridge .308 Win with a bullet SP, – 664 m/s. The difference is significant at the level of significance α=0.05. In terms of the size of the entrance hole, the largest is from the bullet of cartridge .308 Win with a bullet SP – 10.0 cm, the smallest - from the bullet "PS" with a steel core 5.45x39 mm, cartridge sample of the year 1974 (7H6) – 1.2 cm. The difference is significant at the level of significance α=0.05. In the expansive bullet of type "V-Max" of shortgun cartridge of caliber 5.45x39 mm, the size of the entrance hole was 9.1 cm, with asterial shape having radial gaps and turned edges of ballistic plasticine on the outside. All cartridges with expansive bullets did not have an exit hole after the shot. The dimensions of the formed residual cavity were the largest after firing with .308 Win caliber cartridge with a bullet SP – 25.0x5.0 cm., the smallest – bullet of a military caliber cartridge of 5.45x39 mm (7H6) – 6.0x4.0 cm. The difference is significant at significance level α=0.05. The shape and character of the residual cavity in ballistic plasticine was significant for all expansive bullets, in contrast to the bullet of a military cartridge 5.45x39 mm (7H6), where no such changes are detected. The considerable signs of expansion properties and deformation of the bullet in the form of a "flower of death" were identified in the bullet of caliber cartridge .308 Win with a bullet SP, other bullets with expansive properties showed significant fragmentation, with the location of fragments both in the residual cavity and outside its borders at different distances. Expansive bullets differ significantly at the level of significance α=0.05. The low flight speed of bullets (m/s) of .308 Win caliber cartridges with bullets BTHP and SP is due to their structure, weight, and caliber. Bullet of type V-max with cartridge 5.45x39 mm has the highest speed – 1185 m/s, and due to its design has significant expansive properties. Common to expansive bullets is a entrance hole, the blind nature of the lesion with the presence of a large residual cavity, which is due to kinetic energy return 114.37 E, J/mm2 inside the object of lesion. Fragmentation of expansion bullets occurs inside an object with fragments located at different distances. A .308 Win caliber cartridge with SP bullet causes deformation of s bullet by the type of "death flower" causing significant damage.
Мета: розробити шляхи покращення підготовки військових хірургів за рахунок аналізу проблемних питань системи медичного забезпечення в умовах повномасштабної агресії росії проти України.

Результати. З 19 серпня 1993 року для потреб медичної служби ЗС України підготовлено близько 600 військових хірургів, проведено професійну підготовку лікарів за хірургічними спеціальностями для більш ніж 2400 слухачів Академії. У стінах кафедри пройшли перепідготовку та підвищення кваліфікації понад 7000 фахівців лікувальних установ МО України та інших силових відомств, в тому числі на виїзних циклах 1400 лікарів і медичних сестер. Щорічно на кафедрі військової хірургії навчаються від 30 до 50 хірургів і понад 120 слухачів інших спеціальностей. Викладається 25 навчальних дисциплін для слухачів факультетів підготовки і підвищення кваліфікації військових лікарів. З урахуванням сучасних потреб військово-медичної служби України були відкриті цикли спеціальних клінічних дисциплін по діагностиці та лікуванню захворювань і травм з використанням високотехнологічного обладнання і малоінвазивних технологій: ендоскопії, лапароскопії в хірургії, урології та гінекології, ультразвукової діагностики, доплерографії, інтервенційної сонографії, рентгенології, СКТ та МРТ. У навчальному процесі задіяні 16 докторів і 31 кандидат медичних наук. З початком повномасштабної агресії росії проти Держави Україна колективом кафедри проведено шість циклів ТУ «Актуальні питання бойової хірургічної травми» з лікарями-хірургами військових мобільних госпіталів і передових хірургічних груп та з військовослужбовцями ЗС України на першому рівні з TCCC. З моменту початку повномасштабної агресії колектив кафедри провів 25 Вебінарів з міжнародною участю з «Актуальних аспектів бойової хірургічної травми». В них прийняли участь понад 3000 слухачів лікарів-хірургів, травматологів та інших спеціальностей хірургічного напрямку, зі всіх рівнів медичного забезпечення. В обговоренні проблеми виступили найкращі спікери з 10 країн партнерів Держави Україна в боротьбі з повномасштабною агресією росії.
Висновки. З метою покращення підготовки військових хірургів для Сил оборони колективу кафедри військової хірургії необхідно створити навчальну платформу «Військові хірурги України», провести 21-22 жовтня 2023 року предметно-методичний збір з завідувачами кафедр хірургії, травматології та ортопедії вищих медичних навчальних закладів МОЗ України з питань покращення викладання військової хірургії в Україні, провести XVII з’їзд військових хірургів України 19-20 вересня 2023 року та присвятити 30-річному Ювілею кафедри. Покращити взаємодію та співпрацю кафедри військової хірургії УВМА з хірургічними службами всіх лікувальних закладів України, які приймають поранених військовослужбовців з метою методичної підтримки, навчання лікарів з питань воєнно-польової хірургії та підвищення якості надання допомоги при бойовій хірургічній травмі.
