Atypical hemolytic uremic syndrome (aHUS) is a disease of complement dysregulation. In approximately 50% of patients, mutations have been described in the genes encoding the complement regulators ...factor H, MCP, and factor I or the activator factor B. We report here mutations in the central component of the complement cascade, C3, in association with aHUS. We describe 9 novel C3 mutations in 14 aHUS patients with a persistently low serum C3 level. We have demonstrated that 5 of these mutations are gain-of-function and 2 are inactivating. This establishes C3 as a susceptibility factor for aHUS.
Healthcare is a fundamental human right irrespective of gender, race, religion, caste, social status or the community in which they live. But transgender people are mostly victims of such barriers ...which eventually lead to compromised oral health. This narrative review emphasise on the oral health needs including unmet dental needs among transgender population. Search strategies were devised for each database for easy identification of the studies to be included in this review. The search strategy used a combination of controlled vocabulary and free text terms. The main electronic database used to access the studies were PubMed, PubMed Central, Cochrane Review, Embase and Google Scholar using keywords such as transgender, eunuch, dental health, oral health, etc., were used as the main electronic databases to access the studies. Exact 20 articles fulfilled this criteria and were selected for review. A high prevalence of oromucosal lesions were seen among the transgenders. High prevalence of gutkha chewing habit might be a reason for an increase in number of oral mucosal lesions. Furthermore, the review also showed that Oral Submucous Fibrosis (OSMF), leukoplakia and lichen planus were the commonly found oral mucosal lesions in the surveyed population. Awareness of brushing habits among transgenders and care towards their oral hygiene was found to be low. In the present review, it has been observed that the dental health is poor among the studied population. There is a need for oral hygiene and health awareness among transgenders as well as improvement and participation by policy makers for better delivery of health and dental facilities to them. Thus, more emphasis should be laid on research in this population group.

Chronic rhino sinusitis (CRS) is a common disease. Maxillary sinusitis not cured by the medicines was addressed by the open surgical procedure namely Caldwell Luc operation. Thereafter introduction ...of nasal endoscopes in 1970’s led to the minimally invasive surgery FESS which preserved the physiology of the nose and sinuses. In the year 2002 balloon sinuplasty was introduced in the western world and subsequently in India. Due to various logistics it was not performed and reintroduced in the year 2015 in India. It can be termed as micro minimally invasive surgery wherein anatomy as well as physiology of the nose and sinuses were preserved in cases of medically non responsive mild to moderate sinusitis. 20 cases were selected for exclusive balloon sinuplasty of maxillary sinus. Balloon sinuplasty is a relatively new procedure which can be termed as micro minimally invasive surgery addressing the CRS without the traditional forms for surgery like incision, cutting or microdebriding. The principle is causing microfractures by inflating the sinus opening and thus facilitating the drainage of the sinus contents. No immediate or late post operative complications were noted. Sinus patency 6 months later was present in 90% of the cases. Balloon sinuplasty is an excellent procedure for medically nonresponsive CRS without polyposis. The success rate is spectacular matching the FESS with almost no immediate post operative and late complication. The recurrence rate of sinusitis is low. We conclude that balloon sinuplasty is a micro invasive procedure which saves operating time, time of hospital stay of the patient and delivers excellent result with almost no complications. We hope it could be incorporated as a routine surgery for mild to moderate sinusitis not responding to medicines.
Rhinosinusitis is a prevalent condition worldwide affecting the developed and the third world countries alike. SNOT-22 is a validated disease-specific questionnaire designed to use in Chronic Rhino ...Sinusitis (CRS), which means to quantify patients perception of their disease burden. SNOT 22 questionnaire can chart patients nasal as well as psychological symptoms arising due to CRS and very well become marker of individual symptomatic improvement. To study the correlation between disease specific quality of life and endoscopy score after 1 month and 6 months of Endoscopic Sinus Surgery in patients with Chronic Rhinosinusitis. In the present study, ESS surgery showed improvement in Endoscopy score and all the symptoms assessed by SNOT-22. The total percentage of improvement in the SNOT-22 score was 70% after 1 month and 89% at the end of 6 months. We conclude that after the procedure of ESS not only endoscopic evaluation is necessary but also psychological and symptomatic assessment by SNOT 22 is very necessary as nasal and psychological factors were equally decisive for the seeking of the treatment (pharmacological/surgical) by the patient.
Uncontrolled disposal of feathers from the poultry industry and slaughterhouses is environmentally undesirable. The feathers are composed of approximately 90% of keratin which is an important ...ingredient of cosmetics, shampoos and hair treatment creams. This study aimed to determine the optimum conditions for the extraction of keratin from chicken feathers. The extraction of keratin using various reducing agents was studied using statistical experimental design. In the extraction process, pH, temperature, ratio of reducing agents, mass of chicken feathers and incubation time were analyzed. The keratin in the total extracted protein was purified by size exclusion chromatography, sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE) and further characterized using amino acids profile analysis. The surface morphology and chemical composition were studied by scanning electron microscopy (SEM) and Fourier transform infrared spectroscopy (FTIR) analysis. Sodium sulfide (Na

Background: Metabolic syndrome is characterized by hypertension, dyslipidemia, central obesity, glucose intolerance, insulin resistance. Thyroid hormone acts as general pacemaker, accelerating ...metabolic process and may be associated with metabolic syndrome. There is no information available in literature regarding the prevalence and association of thyroid dysfunction in metabolic syndrome in this central region of the country.
Aims and Objective: To estimate the prevalence of thyroid dysfunction in patients of metabolic syndrome.
Materials and Methods: It is a duration based prospective cross sectional study including 200 patients of metabolic syndrome. A detailed history, clinical examination and relevant investigations including serum Free T4 (FT4), Free T3 (FT3), Thyroid Stimulating Hormone (TSH) were done. Range, frequencies, percentage, mean, standard deviation and P value were calculated. P value of < 0.05 was taken as significant.
Results: Prevalence of thyroid dysfunction in metabolic syndrome patients was 28.5%. Prevalence of subclinical and overt hypothyroidism was 18.5% and 8.5% respectively. In patients with both metabolic syndrome and thyroid dysfunction, most common components associated are diabetes mellitus and hypertriglyceridemia.
Conclusion: Thyroid dysfunction is significantly common in metabolic syndrome patients. It should be aggressively detected and treated in these patients for better outcome.
Precise measurements of the thermodynamic critical field (B_{c}) in the type-I noncentrosymmetric superconductor BeAu were performed by means of the muon-spin rotation and relaxation technique. The ...temperature evolution of B_{c} cannot be described within the single-gap scenario and it requires the presence of at least two different types of superconducting order parameters. The self-consistent two-gap approach, adapted for analysis of B_{c}(T) behavior, suggests the presence of two superconducting energy gaps with the gap-to-T_{c} ratios 2Δ/k_{B}T_{c}≃4.52 and ≃2.37 for the big gap and the small gap, respectively Δ=Δ(T=0) is the zero-temperature value of the gap and k_{B} is the Boltzmann constant. This implies that the superconductivity in BeAu is unconventional and that the supercarrier pairing occurs at various energy bands.

The present review aims to analyse the correlation of altmetrics with the traditional citations in medical research.
The search strategy used a combination of controlled vocabulary and free text terms. The main database searched were PubMed, PubMed Central, Cochrane Review, Embase and Google Scholar from 2010 to 2016. Out of the total 78 titles appeared seven articles fulfilled the criteria and were selected for the review.
The positive correlation between altmetrics and traditional citations indicates that the two are not entirely different from each other and are familiar with each other. Altmetrics are usually accessible earlier and enable us to evaluate the social impact of scholarly research, almost at the actual time.
Much work is needed to develop this research which will focus on the clarity of the impact signal. Thus newer dimensions, such as altmetrics and article-level metrics are an effort to explore the influence of research across the worldwide population.
There is a scarcity of data from India on the impact of cell of origin (COO) on outcomes of diffuse large B-cell lymphoma (DLBCL). This study was conducted to evaluate the impact of COO on outcomes ...of DLBCL patients treated with uniform rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisolone (RCHOP) protocol.
This retrospective analysis included patients who received uniform RCHOP chemoimmunotherapy during the study period (2014-2020) at the Department of Medical Oncology at All India Institute of Medical Sciences (AIIMS), New Delhi, India. The patients were classified as germinal center B-cell like (GCB) or activated B-cell (ABC) type using the Hans classification.
Four hundred seventeen patients with median age of 48 years (range, 18-76) and a male-female ratio of 2:1 were included in the analysis. B symptoms and bulky disease were seen in 42.9% and 35.5%. Extranodal involvement was seen in 50.8% of cases. ECOG performance status (0-2) was present in 65%, and 51% presented with advanced disease. GCB subtype was seen in 43%, and 47% were ABC type. Low- and intermediate-risk international prognostic index (IPI) score was seen in 76% of cases. The overall response rate to RCHOP was 85.8%, including a complete response rate of 74.8%. After a median follow-up of 30 months, the 3-year event-free survival (EFS) and overall survival (OS) were 80% and 88%, respectively. The presence of B symptoms and poor ECOG performance status (3-4) was associated with inferior CR rate. Low albumin (
< 0.001), age >60 years (
= 0.001), bulky disease (
< 0.001), and extranodal involvement (
= 0.001) were associated with inferior EFS, whereas a high IPI risk score was associated with an inferior OS (
< 0.001). EFS and OS were not significantly different between the GCB and ABC subtypes. Grade III/IV anemia, neutropenia, and thrombocytopenia were seen in 7.6%, 13.6%, and 2.7% of patients, respectively. Febrile neutropenia was seen in 8.9% of patients, and there were four treatment-related deaths.
Cell of origin for DLBCL has no impact on CR, EFS, and OS if patients are appropriately treated with standard doses and frequency of RCHOP. RCHOP is well tolerated in our patients, and results are comparable with the Western data.
