In order to evaluate the main adverse effects of drug protocols using bortezomib and/or thalidomide for the treatment of multiple myeloma, we conducted a prospective study. Data were collected ...through interviews, clinical observation, and from hospital records. A total of 59 patients were included. There was a predominance of females, 36 (61%) vs 23 (39%) males, and of whites, 49 (83.1%) vs 10 (16.9%) blacks. Age ranged from 40 to 94 years, with a median of 65 years (SD=11.6). Regarding staging at diagnosis, 27 (45.7%) patients were in stage III-A, with 12 (20.3%) patients having serum creatinine ≥2 mg/dL. The main adverse effects in the bortezomib treatment group (n=40) were: neutropenia (42.5%), diarrhea (47.5%), and peripheral neuropathy in 60% of cases, with no difference between the iv (n=26) and sc (n=14) administration routes (P=0.343). In the group treated with thalidomide (n=19), 31.6% had neutropenia, 47.4% constipation, and 68.4% peripheral neuropathy. Neutropenia was associated with the use of alkylating agents (P=0.038). Of the 3 patients who received bortezomib in combination with thalidomide, only 1 presented peripheral neuropathy (33.3%). Peripheral neuropathy was the main adverse effect of the protocols that used bortezomib or thalidomide, with a higher risk of neutropenia in those using alkylating agents. Improving the identification of adverse effects is critical in multiple myeloma patient care, as the patient shows improvements during treatment, and requires a rational and safe use of medicines.
p63 gene is a p53 homologue that encodes proteins with transactivation, DNA-binding and tetramerisation domains. The isoforms TAp63 and TAp73 transactivate p53 target genes and induce apoptosis, ...whereas the isoforms DeltaNp63 and DeltaNp73 lack transactivation and might have dominant-negative effects in p53 family members. p63 is expressed in germinal centre lymphocytes and can be related to the development of the lymphoma, but the prognostic significance of its expression in the survival of patients with diffuse large B-cell lymphoma (DLBCL) remains unclear.
To determine whether quantitative immunohistochemical (IHC) analysis of p63 protein expression correlates with CD10 antigen, Bcl-6 antigen and IRF4 antigen expression and to determine whether p63 is a surrogate predictor of overall survival in high-intermediate and high risk DLBCL populations.
CD10, Bcl-6 and IRF4 expression were retrospectively evaluated by IHC in 73 samples of high-intermediate and high risk DLBCL and were used to divide the lymphomas into subgroups of germinal centre B-cell-like (GCB) and activate B-cell-like (ABC) DLBCL. Similarly, p63 expression was evaluated by IHC and the results were compared with subgroups of DLBCL origin and with the survival rates for these patients.
p63 was expressed in more than 50% of malignant cells in 11 patients and did not show correlation with subgroups of GCB-like DLBCL or ABC-like DLBCL, but p63(+) patients had better disease-free survival (DFS) than those who were negative (p = 0.01).
p63(+) high-intermediate and high risk DLBCL patients have a better DFS than negative cases.
Objectives: T-cell Brazil project started in April 2017 focusing to collecting epidemiological and clinical data from the most frequent subtypes of PTCL, as an ambispective study collected diagnosis ...data from January 2015 to December 2022. Our goals were to obtain the frequency of subtypes and the clinical and biology characteristic from five Brazilian regions; besides to have a routine pathological revision and to evaluate the Overall Survival (OS), Progression-Free Survival (PFS) in 5 years of follow-up. Method: After 39 centers were approved by their Ethical Committee, they registered their cases using the REDcap Platform by Vanderbilt, being 63% public centers. Here, It was just analyzed a Prospective Cohort (PC), with diagnosis date from April 2017 to December 2022, a total of 461 cases. Descriptive analyses, Kaplan-Meier method, Log-Rank test to compare groups and Cox Regression to identify risk factor for OS using IBM-SPSS software v.24. Results: The median age was 52 years (18-92); 58% male; Almost 72% had advanced stages, 25% ECOG ≥ 2; the distribution of main subtypes was: 30% PTCL-NOS; 18% ALCL, ALK-; 14% ATL; 13% ENKTL-NT; 10.5% AITL; 5% ALCL, ALK+; 9.5% others. A median of follow up of 22 months (0-73), 55% pts were alive and 24-month PFS and OS were 36% and 49%, respectively. OS by main subtypes was 69% ALCL, ALK+; 61% ALCL, ALK; 50% PTCL-NOS; 44% ENKTL-NT; 41% AITL; 27% ATL. Cox regression showed ATL (HR 1.6, p = 0.008), ECOG ≥ 2 (HR 1.86, p < 0.0001) and Ann Arbor Stage III-IV (HR 2.06, p < 0.0001) as bad prognosis for OS; whereas for PFS were ATL (HR 1.9, p < 0.0001); ECOG≥ 2 (HR 1.53, p = 0.002); Ann Arbor Stage III-IV (HR 1.75, p = 0.001) and B symptoms (HR 1.36, p = 0.02). Discussion: PTCL make up approximately 10-15% of lymphoid malignancies and its frequency varies geographically. In Brazil, mainly due to its vast dimension, data collection has limitations and is subject to bias. This is the first experience cover all over the country, focusing also an educational and of interchanging experience network among the multidisciplinary health team in Brazil. Conclusions: Our initial target of 500 pts exceeded, hence the number of prospective cohort is getting close, because we are in a consistency data phase and probably more cases will be available after that. So, it is possible to make analyzes with PC. ATL and ENKTL-NT subtype presented more frequent of all, different of the Occidental countries, but similar with recent data from American Latin.
A total of 53 patients aged 18-60 years with high-intermediate or high-risk diffuse large B-cell lymphoma (DLBCL) were evaluated to analyze the impact of the cell of origin. Of 53 patients, 16 ...underwent autologous SCT (ASCT) in first remission and the rest received conventional chemotherapy. Immunohistochemistry was evaluated in 47 cases: 17 were of germinal center (GC) origin and 30 were of non-GC origin. There was no survival difference between the two groups. Overall survival (OS) and disease-free survival (DFS) at 3 years were 93 and 83%, respectively, for the 14 patients who underwent ASCT. Their DFS was significantly better than that of patients who achieved CR but did not undergo ASCT. We conclude that ASCT is safe and improves the DFS of high-intermediate and high-risk DLBCL, regardless of the cell of origin. This observation should be confirmed in a larger study.
O papel da terapia de altas doses (TAD) seguida pelo transplante de células tronco periférico (TCTP) autólogo como consolidação é ainda muito discutido para os pacientes com Linfoma de células T ...periféricas (LCTP), ou seja, não há consenso atualmente sobre o gerenciamento ideal para o tratamento de primeira linha, principalmente devido ao viés de seleção de pacientes e das amostras pequenas nos estudos. Da mesma forma, com a disponibilidade de novos medicamentos, o uso TAD seguida de TCTP na consolidação ou salvamento deixou de serem as únicas opções disponíveis. Assim, nosso objetivo é descrever e verificar os resultados desses procedimentos e compreender o papel da TAD seguida de TCTP nestes pacientes.
Entre janeiro de 2015 e julho de 2021, 414 pacientes foram registrados por 34 centros brasileiros; destes 374 tinha informação disponível das terapias usadas. Um total de 70 (18.5%) casos foi submetido ao TAD seguido TCTP e este grupo é o foco deste estudo. Sobrevida global (SG) foi calculada a partir da data do diagnóstico até a data da última visita ou data de óbito. A sobrevida livre de progressão (SLP) da data do diagnóstico até data do 1° evento ou última consulta. Para o grupo de salvamento (TCTP-S) foi calculado a SLP considerando a data do TCTP até a data da última consulta ou 1° evento, sendo o evento progressão ou morte.
A mediana de idade dos pacientes transplantados foi de 51 anos (20-75), 37 (53%) sexo masculino, 20% teve performance status > 1; 60% apresentaram sintomas B e 67% teve algum sintoma relacionado à doença. 73% apresentou estadio III/IV; 16% dos casos tinham envolvimento extranodal > 1 e 17% tinham doença com bulky; Os subtipos mais frequentes foram 39% Linfoma de células T periféricas não especificado; 28% Linfoma de grandes células T anaplásicas, ALK- e 17% Linfoma angioimunoblástico. O grupo TCTP-C teve 54 pacientes e o status da doença antes do procedimento foi 50 (92,5%) casos com RC e 4 RP (7,5%); o TCTP-S foi realizado em 18 casos e 2 pacientes foram transplantados em ambos os momentos. Em relação ao tipo de TCTP, na consolidação, foram realizados 48 autólogos e 6 alogênicos, enquanto no salvamento foram 13 autólogos e 5 alogênicos. Após uma mediana de seguimento de 23 meses (4-77) para o grupo TCTP-C e 25 meses (6-43) para TCTP-S, a SG do grupo com TCTP-C foi de 81% (IC 95% 69-93%) e 71% (IC 95% 24-88%) para o TCTP-S, P = 0.16; A SLP foi de 61% (IC 95% 45-77%) e 7% (IC 95% 0-19%), respectivamente, p < 0,0001. Analisando o grupo TCTP-S, a resposta após o TCTP foi 4 (22%) RC, 1 (5,5%) RP, 3 (16,5%) doença estável, 5 (28%) progressão e 5 (28%) morreram. A mediana de seguimento foi 10 meses (1-23), e a SG-S foi 54% e 56% SLP-S em dois anos. Conclusões: De acordo com os dados da literatura, o TCTP pode indicar um controle em longo prazo para o LCTP alcançando uma estabilização após 5 anos e nossos dados apresentam curvas de 3 anos para o grupo consolidação, sugerindo que este procedimento é uma boa opção para melhorar os resultados finais.
