Background: Oral anticoagulants, such as dabigatran etexilate, an oral, direct thrombin inhibitor, that do not require monitoring or dose adjustment offer potential for prophylaxis against venous ...thromboembolism (VTE) after total knee replacement surgery. Methods: In this randomized, double‐blind study, 2076 patients undergoing total knee replacement received dabigatran etexilate, 150 mg or 220 mg once‐daily, starting with a half‐dose 1–4 h after surgery, or subcutaneous enoxaparin 40 mg once‐daily, starting the evening before surgery, for 6–10 days. Patients were followed up for 3 months. The primary efficacy outcome was a composite of total VTE (venographic or symptomatic) and mortality during treatment, and the primary safety outcome was the incidence of bleeding events. Results: The primary efficacy outcome occurred in 37.7% (193 of 512) of the enoxaparin group vs. 36.4% (183 of 503) of the dabigatran etexilate 220‐mg group (absolute difference, −1.3%; 95% CI, −7.3 to 4.6) and 40.5% (213 of 526) of the 150‐mg group (2.8%; 95% CI,−3.1 to 8.7). Both doses were non‐inferior to enoxaparin on the basis of the prespecified non‐inferiority criterion. The incidence of major bleeding did not differ significantly between the three groups (1.3% vs. 1.5% and 1.3% respectively). No significant differences in the incidences of liver enzyme elevation and acute coronary events were observed during treatment or follow‐up. Conclusions: Dabigatran etexilate (220 mg or 150 mg) was at least as effective as enoxaparin and had a similar safety profile for prevention of VTE after total knee replacement surgery.

Aim
To investigate the safety, tolerability, pharmacokinetics and pharmacodynamics of empagliflozin in patients with type 2 diabetes following oral administration of 10, 25 or 100 mg doses ...once daily over 28 days.
Methods
A total of 78 patients were assigned to empagliflozin 10 mg (n = 16), 25 mg (n = 16) or 100 mg (n = 30) or placebo (n = 16) for 28 days. Assessments included adverse events (AEs) and pharmacokinetic and pharmacodynamic endpoints.
Results
Empagliflozin exposure increased dose‐proportionally over the dose range 10–100 mg and showed linear pharmacokinetics with respect to time. Urinary glucose excretion (UGE) increased from baseline to day 1 by 74, 90 and 81 g with empagliflozin 10, 25 and 100 mg, respectively. The increases in UGE were maintained over 28 days with multiple dosing. Virtually no change in UGE was observed in the placebo group. Significant reductions from baseline in mean daily plasma glucose and fasting plasma glucose were observed with empagliflozin compared with placebo. The incidence of AEs was similar in the empagliflozin and placebo groups (50.0, 56.3 and 66.7% with empagliflozin rising doses and 62.5% with placebo). The most frequently reported AEs were pollakiuria (10.3%), nasopharyngitis (9.0%), constipation (9.0%) and headache (7.7%).
Conclusions
Oral administration of empagliflozin at doses of 10, 25 or 100 mg once daily over 28 days resulted in significant increases in UGE and reductions in blood glucose compared with placebo, and were well tolerated in patients with type 2 diabetes.
Aims
To evaluate the effects of the sodium glucose cotransporter 2 (SGLT2) inhibitor empagliflozin added to metformin for 12 weeks in patients with type 2 diabetes.
Methods
This dose‐ranging, ...double‐blind, placebo‐controlled trial randomized 495 participants with type 2 diabetes inadequately controlled on metformin haemoglobin A1c (HbA1c) >7 to ≤10% to receive 1, 5, 10, 25, or 50 mg empagliflozin once daily (QD), or placebo, or open‐label sitagliptin (100 mg QD), added to metformin for 12 weeks. The primary endpoint was change in HbA1c from baseline to week 12 (empagliflozin groups versus placebo).
Results
Reductions in HbA1c of −0.09 to −0.56% were observed with empagliflozin after 12 weeks, versus an increase of 0.15% with placebo (baseline: 7.8–8.1%). Compared with placebo, empagliflozin doses from 5 to 50 mg resulted in reductions in fasting plasma glucose (−2 to −28 mg/dl vs. 5 mg/dl with placebo; p < 0.0001) and body weight (−2.3 to −2.9 kg vs. −1.2 kg; p < 0.01). Frequency of adverse events was generally similar with empagliflozin (29.6–48.6%), placebo (36.6%) and sitagliptin (35.2%). Hypoglycaemia rates were very low and balanced among groups. Most frequent adverse events with empagliflozin were urinary tract infections (4.0% vs. 2.8% with placebo) and pollakiuria (2.5% vs. 1.4% with placebo). Genital infections were reported only with empagliflozin (4.0%).
Conclusions
Once daily empagliflozin as add‐on therapy to metformin was well tolerated except for increased genital infections and resulted in reductions in HbA1c, fasting plasma glucose and body weight in patients with type 2 diabetes inadequately controlled on metformin monotherapy.
Aim
This Phase IIb, randomized, double‐blind, placebo‐controlled trial evaluated the efficacy, safety, tolerability and pharmacokinetics of empagliflozin in patients with type 2 diabetes.
Methods
...Four hundred and eight patients (treatment‐naïve or after a 4‐week wash‐out period) were randomized to receive empagliflozin 5, 10 or 25 mg once daily, placebo or open‐label metformin for 12 weeks. The primary endpoint was change in haemoglobin A1c (HbA1c) after 12 weeks.
Results
After 12 weeks' treatment, empagliflozin showed dose‐dependent reductions in HbA1c from baseline 5 mg: −0.4%, 10 mg: −0.5%, 25 mg: −0.6%; all doses p < 0.0001 vs. placebo (+0.09%). Fasting plasma glucose (FPG) decreased with empagliflozin 5 mg: −1.29 mmol/l, 10 mg: −1.61 mmol/l, 25 mg: −1.72 mmol/l; all doses p < 0.0001 vs. placebo (+0.04 mmol/l). Body weight decreased in all empagliflozin groups (all doses p < 0.001 vs. placebo). The incidence of adverse events (AEs) was similar in the placebo (32.9%) and empagliflozin (29.1%) groups. The most frequently reported AEs on empagliflozin were pollakiuria (3.3% vs. 0% for placebo), thirst (3.3% vs. 0% for placebo) and nasopharyngitis (2.0% vs. 1.2% for placebo). AEs consistent with urinary tract infections (UTIs) were reported in four (1.6%) patients on empagliflozin vs. one (1.2%) on placebo. Genital infections were reported in five (2%) patients on empagliflozin vs. 0% on placebo. No UTIs or genital infections led to premature discontinuation.
Conclusions
In patients with type 2 diabetes, empagliflozin resulted in dose‐dependent, clinically meaningful reductions in HbA1c and FPG, and reductions in body weight compared with placebo. Empagliflozin was well‐tolerated with a favourable safety profile.
Aim
To assess the effect of empagliflozin on cardiovascular (CV) risk in patients with type 2 diabetes (T2DM) through a meta‐analysis of data from eight placebo‐controlled trials.
Methods
Data were ...analysed from eight randomized placebo‐controlled trials undertaken to investigate the efficacy and safety of empagliflozin 10 and 25 mg once daily in patients with T2DM, comprising patients at low/medium and high CV risk. Suspected CV events were prospectively adjudicated. The empagliflozin 10 and 25 mg groups were pooled for the primary analysis. The primary endpoint was a composite of CV death, non‐fatal myocardial infarction (MI), non‐fatal stroke and hospitalization for unstable angina 4‐point major adverse CV events (MACE). The secondary endpoint was a composite of CV death, non‐fatal MI and non‐fatal stroke (3‐point MACE). Risk estimates were calculated using Cox regression analysis.
Results
A total of 3835 patients received placebo and 7457 received empagliflozin. Total exposure was 7448.3 years for placebo and 15482.1 years for empagliflozin. Four‐point MACE occurred in 365 (9.5%) patients receiving placebo and 635 (8.5%) patients receiving empagliflozin hazard ratio for empagliflozin vs. placebo 0.86 (95% CI 0.76, 0.98). Three‐point MACE occurred in 307 (8.0%) patients receiving placebo and 522 (7.0%) patients receiving empagliflozin hazard ratio for empagliflozin vs. placebo 0.84 (95% CI 0.73, 0.96).
Conclusions
In a meta‐analysis of data from eight randomized trials involving 11 292 patients with T2DM at low/medium or high CV risk, empagliflozin was associated with a reduced risk of 4‐point MACE and 3‐point MACE compared with placebo.
Abstract Background Three randomized, double-blind trials compared dabigatran, an oral direct thrombin inhibitor, with enoxaparin for the primary prevention of venous thromboembolism (VTE) in ...patients undergoing elective total hip and knee arthroplasty. Objectives and Methods We conducted a pre-specified pooled analysis of these trials. 8,210 patients were randomized, of whom 8,135 were treated (evaluable for safety) with dabigatran 220 mg or 150 mg once-daily, or subcutaneous enoxaparin (40 mg once-daily or 30 mg twice-daily). Efficacy analyses were based on the modified intention-to-treat population of 6,200 patients with an evaluable outcome. The common risk difference (RD) of treatment effect between each dabigatran dose and enoxaparin was estimated using fixed-effects models, and statistical heterogeneity was estimated using the I2 statistic. Results The composite outcome of major VTE (proximal deep vein thrombosis and/or pulmonary embolism) and VTE-related mortality occurred in 3.3% of the enoxaparin group versus 3.0% of the dabigatran 220 mg group (RD vs. enoxaparin -0.2%, 95% CI -1.3% to 0.9%, I2 = 37%) and 3.8% of the 150 mg group (RD vs. enoxaparin 0.5%, -0.6% to 1.6%, I2 = 0%). Major bleeding occurred in 1.4% of the enoxaparin group versus 1.4% of the dabigatran 220 mg group (RD vs. enoxaparin -0.2%, -0.8% to 0.5%, I2 = 40%) and 1.1% of the 150 mg group (RD vs. enoxaparin -0.4%, -1.0% to 0.2%, I2 = 0%). Conclusions Oral dabigatran was as effective as subcutaneous enoxaparin in reducing the risk of major VTE and VTE-related mortality after hip or knee arthroplasty and has a similar bleeding profile.
Introduction Une analyse groupée des résultats de quatre études randomisées (phase III) chez des diabétiques de type 2 (âge > 18 ans) avec un IMC ≤ 45 kg/ m² et 7 % ≤ HbA1c ≤ 10 % (n = 2 477) a ...montré que l’empagliflozine 10 mg (E10) et 25 mg (E25), en monothérapie ou traitement associé pendant 24 semaines, a plus réduit vs placebo l’HbA1c et le poids de patients jeunes avec une HbA1c initialement élevée. Patients et méthodes Dans une analyse a posteriori de patients de moins de 65 ans avec un 25 ≤ IMC < 35 kg/m² et une HbA1c ≥ 8 % à l’inclusion nous avons comparé vs placebo (n = 188), l’efficacité de E10 (n = 193) ou E25 (n = 179). Résultats L’empagliflozine E10 et E25 a, vs placebo, significativement et respectivement réduit (%) l’HbA1c (–0,89*** ; –0,99***), le poids (kg) corporel (– 1,6*** ; –2,0***) et la pression (mm Hg) artérielle systolique (–2,6* ; – 2,5*). Des événements indésirables (EI) furent rapportés chez respectivement 58,7 % ; 60,1 % et 56,4 % des patients sous E10, E25 et placebo. Les patients avec des hypoglycémies (≤ 70 mg/dl et/ou nécessitant assistance) ont été plus nombreux sous EMPA 10 (3,9 %) et EMPA 25 (4,7 %) que sous placebo (3,2 %) ; aucun épisode n’a nécessité assistance. Des infections urinaires ont été rapportées de façon similaire sous EMPA 10 (4,5 %), EMPA 25 (8,3 %) et placebo (5,9 %) . Des infections génitales ont été plus nombreuses sous EMPA 10 (3,6 %) et EMPA 25 (4,5 %) que sous placebo (1,1 %). Conclusion Chez des patients jeunes en surpoids ou obèses avec initialement une HbA1c ≥ 8 %, l’empagliflozine à 10 ou 25 mg a significativement réduit l’HbA1c, le poids corporel et la pression artérielle et a été bien tolérée. * p < 0,05 ; *** p < 0,001: vs placebo. Introduction Les inhibiteurs du SGLT2 augmentent la glycosurie. Cela favorise- t-il les Infections Urinaires et les Infections Génitales ? Quatre études (Phase III) randomisées contrôlées vs placebo ...ont évalué chez des DT2 l’effet de 24 semaines d’EMPA en monothérapie, en addition à metformine (Met) +/-Sulfamides (SU) ou en addition à pioglitazone ± Met Patients et méthodes Ces études ont inclus 2 477 patients (âge moyen 55,6 ± 10,5 ans) dont 830 sous EMPA 10 mg, 822 sous EMPA 25 mg et 825 prenant un placebo (PBO). Résultats Le pourcentage (%) de patients présentant une IU a été similaire entre PBO (8,2 %) et EMPA 10 mg et 25 mg (9,3 % et 7,5 %). Par rapport au PBO (0,7 %) plus de patients traités par EMPA (4,2 % et 3,6 %) ont rapporté des IG. Les événements ont été plus fréquents chez les femmes (* : IU : 13 % 18,5 % et 15,9 % ; IG : 1,0 %, 6,3 % et 7,0 %) que chez les hommes (* :IU : 3,8 % ; 1,9 % et 1,1 % ; IG : 0,5 % ; 2,6 % et 1,1 %), et plus fréquents chez les patients avec antécédents d’IU (* : 20,8% ; 26,2% et 2 %, vs sans antécédent : * 7,4 % ; 8,4 % et 6,6 %) ou d’IG (* : 14,3% ; 50,0% et 23,1 % vs sans antécédent : * 0,6 % ; 4,0 % et 3,3 %). La plupart des patients ont présenté un seul épisode IU ou IG. Très peu de patients ont interrompu l’étude à cause de ces événements la plupart étant d’intensité légère. * : respectivement pour les groupes PBO, EMPA10 et EMPA 25 Conclusion Dans cette analyse groupée des données de 2 477 patients, et comparativement au placebo, EMPA n’a pas été associée à une augmentation de la fréquence des Infections Urinaires mais a été associé à celle des Infections Génitales.
Introduction Nous avons analysé les données groupées de quatre études de phase III randomisées contrôlées par placebo concernant 2 477 patients DT2 (55,6 ans ± 10,2 ; HbA1c 7,99 % ± 0,8 ;IMC 28,7 ...kg/m2 ± 5,5) recevant empagliflozine (EMPA) 10 mg ou 25 mg ou un placebo pendant 24 semaines en monothérapie, en addition à metformine (MET) ± sulfamide hypoglycémiant (SU) ou en addition à pioglitazone ± MET. Patients et méthodes L’analyse intégrale portait sur l’HbA1c, la glycémie à jeun (GAJ), le poids, la pression artérielle systolique et diastolique (PAS et PAD) (N + : 825,831,821). Les lipides et l’acide urique ont été évalués chez tous les patients traités (N + : 825,830,822). Les effets sur la PAS et la PAD ont également été évalués chez les patients présentant une PA non contrôlée (PAS ≥ 130 mm Hg ou PAD ≥ 80 mm Hg) à l’état initial (N + : 501,517,506). Résultats EMPA (10 et 25 respectivement) a réduit significativement versus placebo à la semaine 24 : HbA1c (%) : – 0,62*** et – 0,68*** ; GAJ (mg/dl) : – 27,9***et – 30,6*** ; poids (kg) : – 1,81 et – 2,01 ; versus baseline (mmHg) PAS : – 3,4*** et – 3,8*** ; PAD : – 1,2*** et – 1,5*** ; acide urique (umol/l) : – 28,95*** et – 29,55***. On note des réductions PAS/PAD plus prononcées quand la PA initiale est non contrôlée. De petites augmentations (mmol/l) du cholestérol HDL : 0,07*** et 0,07*** et LDL : 0,08 et 0,10** sont notées ainsi qu’une faible diminution des triglycérides avec EMPA versus PBO. Conclusion L’analyse groupée des données de quatre études (phase III) montre que 24 semaines sous EMPA 10 mg ou 25 mg améliore de façon cliniquement cohérente : les paramètres glycémiques, le poids, la PA avec des effets positifs sur l’acide urique et faibles sur les lipides. But But de cette étude de 12 semaines en double-aveugle, contrôlée par placebo était d’évaluer efficacité et innocuité de 5 doses de BI 10773, puissant inhibiteur de la SGLT-2 chez des patients ...diabétiques de type 2 (DT2) insuffisamment contrôlés par la metformine. Patients et méthodes 495 patients (HbA1c moyenne initiale 7,9 %, âge 58 ans, hommes 51 %, IMC 31,4 kg/m2 ) ont été randomisés pour recevoir BI 10773 (1 mg, 5 mg, 10 mg, 25 mg ou 50 mg/jour), un placebo (PBO), la sitagliptine 100 mg (SITA) en ouvert, en association avec de la metformine pendant 12 semaines. Le critère principal d’évaluation était la modification du taux HbA1c à 12 semaines versus la valeur initiale. Résultats Réductions significatives et dose-dépendantes de l’HbA1c versus PBO montrées avec BI 10773 avec une diminution maximale de ∼0,7 % (effet placebo soustrait) pour 10 et 25 mg, alors que SITA a fait diminuer l’HbA1c de ∼0,6 % vs PBO. Réduction significative de la glycémie à jeun et du poids corporel observés versus PBO dans tous les groupes traités par BI 10773 sauf le groupe 1 mg. Réduction maximale (PBO corrigée) de la PAS de ∼ 6 mmHg observée avec BI 10773 25 mg. Fréquence des événements indésirables (EI) comparable dans tous les groupes (BI 10773 : 38,5 %, PBO : 36,6 %, SITA : 35,2 %). L’EI le plus fréquent était l’infection de l’appareil urinaire (3,1 % pour BI 10773 versus 2,8 % pour PBO), la pollakiurie (2,5 % versus 1,4 %). Infections génitales : 2,5 % des cas avec BI 10773 et 1,4 % des cas avec SITA., mais pas pour PBO. Taux d’hypoglycémie similaires entre les groupes. Conclusion BI 10773 a donc entraîné des réductions dose-dépendantes de l’HbA1c, de la glycémie à jeun, du poids corporel et a bien été toléré avec une fréquence légèrement accrue versus PBO des infections génitales mais pas des infections de l’appareil urinaire.
