At the ATLAS experiment, the Detector Control System (DCS) is used to oversee detector conditions and supervise the running of equipment. It is essential that information from the DCS about the ...status of individual sub-detectors be extracted and taken into account when determining the quality of data taken and its suitability for different analyses. DCS information is written to the ATLAS conditions database and then summarised to provide a status flag for each sub-detector and displayed on the web. We discuss how this DCS information should be used, and the technicalities of making this summary.

Trotz aller klinisch-wissenschaftlichen Bemühungen in den letzten Jahrzehnten ist die Prognose von Patienten mit malignen Melanomen im Stadium der Fernmetastasierung nach wie vor bis ...auf wenige Ausnahmen infaust. Eine effiziente, adjuvante Therapie ist daher bei Patienten mit einem bedeutsamen Risiko einer Metastasierung von größtem Interesse. Eine systemische adjuvante Chemotherapie wies in prospektiv-randomisierten Studien im Gegensatz zu ersten Untersuchungen mit historischen Kontrollkollektiven übereinstimmend keinen Vorteil für die Behandelten auf. Die Therapie mit den Interferonen-α-2a und -2b ist die erste und bisher einzige adjuvante Therapie, die in kontrollierten Studien zu signifikanten Vorteilen für die Behandelten führte und in der Indikation der adjuvanten Therapie des malignen Melanoms in Deutschland zugelassen wurde. Der aktuelle Fokus liegt zum einen auf vergleichenden Untersuchungen zu Standard-Interferon-α und pegyliertem Interferon sowie andererseits in der Erprobung innovativer Therapieansätze durch neue Substanzen wie z. B. Vakzinierung mit MAGE-A3 und dem CTLA4-Antikörper Ipilimumab.
The biomechanical properties of the proximal tibia with respect to the application of compressible intramedullary nails were examined in a biomechanical model using human cadaver bones. A reference ...level was examined histomorphometrically in 24 cryopreserved human tibias. Three series of single or double interlocking fixations were tested at various distances from the tibial plateau. Yield strength, rigidity of the bone stock, maximum load, and compression distances were determined during axial strain. The positioning of the interlocking screws had a considerable influence on maximum load and deformation behavior of the bone. The double interlocking tended to display the best stability in the bone stock, and the distally placed single interlocking method was next best. Failure of the bone stock was observed with significant variations in compression distance and load, with great variations between individual specimens. The stability of the bone stock could not be accurately predicted using such clinical parameters as the patient's age, bone dimensions, radiographic parameters, and bone density. When using a tibial compression nail, the proximal interlocking screw should be placed relatively distal to achieve adequate support. It is possible to apply compression with one proximal interlocking screw under low preload or, preferably, with two interlocking screws.
To evaluate the efficacy and safety of vindesine in patients with metastatic melanoma after complete metastasectomy. One hundred and forty-two patients with metastatic spread to regional sites, lymph ...nodes, and distant sites after complete metastasectomy were randomized to receive either treatment with vindesine for 2 years or observation alone. Vindesine 3 mg/m intravenously was administered biweekly for the first 26 weeks following 3-week intervals for an additional 26 weeks and thereafter every 4 weeks for 52 weeks. One hundred and thirty-nine patients were eligible for intent-to-treat analysis. Median follow-up time was 46 months. Median recurrence free survival was 7.9 months in the vindesine group and 7.6 months in the observational group (P=0.40). Three-year overall survival rate was 54.9% (37 patients) for patients receiving vindesine in comparison to 43.6% (31 patients) in the observation arm (P=0.07). No grade IV toxicity was observed. The two major side effects in the vindesine group were alopecia and peripheral neuropathy. Ten patients went off treatment because of grade III toxicity. Adjuvant treatment with vindesine did not significantly prolong disease free or overall survival in high-risk melanoma patients. Thus, this randomized trial did not confirm earlier reports of beneficial effects of adjuvant vindesine and can therefore not be recommended.
Zusammenfassung
Erstmalig seit 3 Jahrzenten stehen neue Medikamente für das metastasierte Melanom vor der baldigen Zulassung, und die Behandlung wird sich durch Early-Access-Programme bereits ab 2011 ...deutlich ändern. Die Therapie des malignen Melanoms im Stadium der Fernmetastasierung bleibt für die Mehrzahl der Patienten dennoch palliativ. Möglichst lang andauernde Remissionen mit Verlängerung der Überlebenszeit werden angestrebt, wobei bei einem kleineren Teil der Patienten auch Heilungen mit den neuen Medikamenten möglich erscheinen. Soweit die Metastasierung ein Organsystem nicht überschreitet und eine operative Entfernung des Tumors möglich ist, ist die operative Metastasenresektion die Therapie der Wahl. Bei ausgedehnter Metastasierung ist eine Systemtherapie indiziert. Neue Therapiestrategien wie die Blockade von „cytotoxic T lymphocyte-associated antigen 4“ (CTLA-4) durch Antikörper induzierten bei einigen Patienten teils lang anhaltende Tumorremissionen und führten zu einer statistisch evidenten Verlängerung des Gesamtüberlebens. Der therapeutische Einsatz von hoch selektiven Tyrosinkinaseinhibitoren zeigte in ersten Phase-II-Studien objektive Ansprechraten von ca. 80% bei vorigem Nachweis der BRAF-V600E-Mutation (bei 40–50% der Melanompatienten vorhanden) und verdeutlicht überzeugend die Notwendigkeit einer individualisierten Tumortherapie in der Zukunft.

Because of this controversy, however, the ...conclusions drawn by Huber-Wagner and colleagues cannot be derived from the presented data without caveats. ... we completely agree with T P Saltzherr and J C Goslings that a randomised controlled trial would provide the best evidence for the implementation of whole-body CT as a routine diagnostic strategy.
