Background
Periostin is a matricellular protein, and its synthesis in airway epithelial cells and lung fibroblasts is induced by interleukin (IL)‐4 and IL‐13. The significance of periostin as a ...biomarker of TH2‐induced airway inflammation, and (importantly) as a measure of the response to TH2‐targeted therapy, has recently been emphasized. We explored the relationship between periostin and airway hyperresponsiveness (AHR) in asthmatic children.
Methods
The study included 83 children aged 6–15 years in an asthmatic group (n = 54) and healthy controls (n = 29). We measured the periostin levels in serum and performed methacholine and mannitol provocation challenges. The responses to mannitol were expressed as the provocative dose causing a 15% fall in the FEV1 (the PD15 dose).
Results
Of the 54 subjects with asthma, all had positive methacholine bronchial provocation test (BPT) results and 38 had positive mannitol BPT results. Children with asthma had significantly higher periostin levels than controls 76.0 (65.0–91.8) vs 71.0 (57.5–80.0) ng/mL; P = 0.017. Periostin levels were significantly correlated with both the methacholine PC20 and mannitol PD15 values.
Conclusion
Serum levels of periostin, a new biomarker induced by IL‐13, were higher in asthmatic children, and were associated with AHR to methacholine and mannitol.
Background
Pregnancy and the peripartum period is a hypercoagulable state increasing the risk of venous thromboembolism (VTE). There may be a role in utilising D‐dimer in the peripartum setting.
Aims
...The purpose of this review was to summarise the latest evidence regarding the diagnostic accuracy of D‐dimer in the peripartum setting with or without the addition of clinical decision rules.
Methods
We searched PubMed and CENTRAL databases to identify articles that included studies of women who had suspected VTE, underwent a D‐dimer index test to rule out VTE and where radiological imaging or clinical follow‐up, to a minimum of 30 days, was used as the reference standard.
Results
We included 11 studies in the systematic review and meta‐analysis. The log diagnostic odds ratio (DOR) for identifying VTE using D‐dimer was 1.56 (95% confidence interval (CI) 0.59–2.52). The pooled sensitivity was 87% (95% CI 76.8–93%), specificity was 63.2% (95% CI 47.1–76.7%), and the area under receiver operator characteristic (ROC) curves was 0.76. We included four studies evaluating D‐dimer combined with YEARS to detect VTE. The log DOR for identifying VTE using D‐dimer combined with YEARS was 1.13 (95% CI 0.005–2.25). The pooled sensitivity was 89.8% (95% CI 60.2–98.1%), specificity was 65.7% (95% CI 54.7–75.2%) and the area under ROC for studies included with the YEARS clinical decision rule was 0.49.
Conclusion
This review highlighted that D‐dimer use in the peripartum period for detection of VTE had a high sensitivity and high DOR but a poor area under ROC, which may limit its use in clinical practice.
Mutations of MKRN3, the gene encoding makorin RING-finger protein 3, lead to central precocious puberty (CPP). The aim of this study was to investigate mutations of the MKRN3 gene in Korean girls ...with CPP. Two hundred-sixty Korean girls with idiopathic CPP were included. Auxological and endocrine parameters were measured, and the entire MKRN3 gene was directly sequenced. MKRN3 gene analysis revealed one novel nonsense mutation (p.Gln281 *) and 6 missense variants (p.Ile100Phe, p.Gly196Val, p.Ile204Thr, p.Gln226Pro, p.Lys233Asn, and p.Ser396Arg). The novel nonsense mutation (p.Gln281 *) was a heterozygous C>T nucleotide change (c.841C>T) predicted to result in a truncated protein due to a premature stop codon in the MKRN3 gene. The nonsense mutation (p.Gln281 *) was only identified in one of the girls and her younger brother. Compared to previous reports on MKRN3 mutations in familial and sporadic cases of CPP, the present study reveals a relatively low number of MKRN 3 mutations in Korean girls with CPP. Larger samples of children with CPP and MKRN3 mutations are necessary in order to clarify whether the clinical course of puberty may differ as compared to idiopathic CPP.
The industry foundation class (IFC) standard building model schema is a necessary but not sufficient condition for achieving full interoperability between building information modeling (BIM) tools. ...Unless each information exchange within construction project workflows has its specific contents and level of detail defined, the breadth and flexibility of the IFC schema leaves room for errors. The national BIM standard approach for resolving the ambiguities of information exchange is based on “use cases,” which precisely define the data required in each information exchange between disciplines in engineering workflows. In this paper, we define specific procedures for developing information delivery manuals (IDM), which capture the use cases and the precise information to be exchanged. We also identify some of the data semantics we found problematic that require workflow specification. We further propose details of the information capture that allow the IDM to serve as a specification for later implementation and testing. The procedures are illustrated using examples from an IDM specification developed for the domain of architectural precast concrete.
Background
Sentinel node navigation surgery reduces the extent of gastric and lymph node dissection, and may improve quality of life. The benefit and harm of laparoscopic sentinel node navigation ...surgery (LSNNS) for early gastric cancer is unknown. The SENORITA (SEntinel Node ORIented Tailored Approach) trial investigated the pathological and surgical outcomes of LSNNS compared with laparoscopic standard gastrectomy (LSG) with lymph node dissection.
Methods
The SENORITA trial was an investigator‐initiated, open‐label, parallel‐assigned, non‐inferiority, multicentre RCT conducted in Korea. The primary endpoint was 3‐year disease‐free survival. The secondary endpoints, morbidity and mortality within 30 days of surgery, are reported in the present study.
Results
A total of 580 patients were randomized to LSG (292) or LSNNS (288). Surgery was undertaken in 527 patients (LSG 269, LSNNS 258). LSNNS could be performed according to the protocol in 245 of 258 patients, and a sentinel node basin was detected in 237 (96·7 per cent) Stomach‐preserving surgery was carried out in 210 of 258 patients (81·4 per cent). Postoperative complications occurred in 51 patients in the LSG group (19·0 per cent) and 40 (15·5 per cent) in the LSNNS group (P = 0·294). Complications with a Clavien–Dindo grade of III or higher occurred in 16 (5·9 per cent) and 13 (5·0 per cent) patients in the LSG and LSNNS groups respectively (P = 0·647).
Conclusion
The rate and severity of complications following LSNNS for early gastric cancer are comparable to those after LSG with lymph node dissection. Registration number: NCT01804998 (

La cirugía de navegación del ganglio centinela (sentinel node navigation surgery, SNNS) reduce la extensión de la resección gástrica y ganglionar, y puede mejorar la calidad de vida. Se desconoce el beneficio y el daño de la cirugía de navegación del ganglio centinela por vía laparoscópica (laparoscopic sentinel node navigation surgery, LSNNS) para el cáncer gástrico precoz. El ensayo clínico SENORITA investigó los resultados patológicos y quirúrgicos de LSNNS en comparación con la gastrectomía laparoscópica estándar (laparoscopic gastrectomy, LSG) con disección ganglionar (lymph node dissection, LND).
Métodos
El ensayo SENORITA fue un ensayo multicéntrico aleatorizado y controlado, iniciado por investigadores, abierto, con asignación a grupos paralelos y de no inferioridad llevado a cabo en Corea. El resultado primario fue la supervivencia libre de enfermedad a los 3 años. En el presente estudio, se describen los resultados secundarios correspondientes a morbilidad y mortalidad a los 30 días del postoperatorio.
Resultados
Un total de 580 pacientes fueron aleatorizados a LG (n = 292) o LSNNS (n = 288). La cirugía se realizó en 527 pacientes (LG 269, LSNNS 258). LSNNS pudo ser realizada de acuerdo con el protocolo en 245 de 258 pacientes y en 237 de 245 pacientes (96,7%) se detectó un ganglio centinela. La cirugía con preservación del estómago se realizó en 210 de 258 pacientes (81,4%). Las complicaciones postoperatorias se presentaron en 51 pacientes del grupo LSG (19,0%) y en 40 pacientes (15,5%) del grupo LSNNS (P = 0,294). Las complicaciones grado III o mayor de Clavien‐Dindo se detectaron en 16 (5,9%) y 13 pacientes (5,0%) de los grupos LSG y LSNNS, respectivamente (P = 0,647).
Conclusión
El porcentaje y la gravedad de las complicaciones tras LSNNS para cancer gástrico precoz son comparables a la LSG con LND.
The prospective, multicentre, randomized controlled phase III SENORITA trial evaluated the surgical and oncological outcomes of laparoscopic sentinel node navigation surgery (LSNNS) compared with laparoscopic standard gastrectomy (LSG) with lymph node dissection (LND) for early gastric cancer (EGC). LSNNS for EGC is a safe procedure in terms of postoperative morbidity and mortality compared with LSG and LND. We discuss the photoluminescence (PL) of semiconducting transition metal dichalcogenides on the basis of experiments and a microscopic theory. The latter connects ab initio calculations of the ...single-particle states and Coulomb matrix elements with a many-body description of optical emission spectra. For monolayer MoS2, we study the PL efficiency at the excitonic A and B transitions in terms of carrier populations in the band structure and provide a quantitative comparison to an (In)GaAs quantum well-structure. Suppression and enhancement of PL under biaxial strain is quantified in terms of changes in the local extrema of the conduction and valence bands. The large exciton binding energy in MoS2 enables two distinctly different excitation methods: above-band gap excitation and quasi-resonant excitation of excitonic resonances below the single-particle band gap. The latter case creates a nonequilibrium distribution of carriers predominantly in the K-valleys, which leads to strong emission from the A-exciton transition and a visible B-peak even if the band gap is indirect. For above-band gap excitation, we predict a strongly reduced emission intensity at comparable carrier densities and the absence of B-exciton emission. The results agree well with PL measurements performed on monolayer MoS2 at excitation wavelengths of 405 nm (above) and 532 nm (below the band gap).
ABSTRACT We report the results of our observing campaign on GRB 140903A, a nearby (z = 0.351) short-duration (T90 ∼ 0.3 s) gamma-ray burst discovered by Swift. We monitored the X-ray afterglow with ...Chandra up to 15 days after the burst and detected a steeper decay of the X-ray flux after tj 1 day. Continued monitoring at optical and radio wavelengths showed a similar decay in flux at nearly the same time, and we interpret it as evidence of a narrowly collimated jet. By using the standard fireball model to describe the afterglow evolution, we derive a jet opening angle θj 5° and a collimation-corrected total energy release E 2 erg. We further discuss the nature of the GRB progenitor system. Three main lines disfavor a massive star progenitor: the properties of the prompt gamma-ray emission, the age and low star formation rate of the host galaxy, and the lack of a bright supernova. We conclude that this event likely originated from a compact binary merger.
In this work we report the use of natural cellulose as the binder for composite electrodes for electrochemical double-layer capacitors (EDLCs). Composite electrodes containing natural cellulose as ...the binder can be prepared using either ionic liquid based slurries or aqueous suspensions containing a polymeric stabilizer and a surfactant. These composite electrodes display good mechanical stability, uniform morphology and a thermal stability comparable to those made with conventional binders. Moreover, when used as electrodes in EDLCs, they allow the realization of devices able to display high performance in term of specific capacitance and cycling stability in all type of electrolytes used in EDLCs. Considering the appealing properties of natural cellulose, the introduction of this material as a binder in EDLCs could be an important contribution for the development of cheap, safe and environmentally friendly EDLCs.
