Differentiation between pancreatic malignant and inflammatory tumors presents an important diagnostic problem. The ability to recognize pancreatic malignant tumors using Doppler evaluation of tissue ...perfusion has been recently demonstrated. The aim of the study was to assess the diagnostic value of Dynamic Tissue Perfusion Measurement (DTPM) in the differentiation between malignant and inflammatory pancreatic tumors. The study included 60 patients (35M, 25F, age 60.9 ± 2.3 years) with a malignant (Group 1, n = 30) or inflammatory (Group 2, n = 30) pancreatic tumor undergoing endoscopic ultrasound with the evaluation of tissue perfusion by Color Doppler and a simultaneous biopsy of lesions for cytological evaluation. In 20 patients the diagnosis was verified in the postoperative histopathological examination. Flow velocity (FV) and percentiles of the distribution of perfusion intensity (PR) evaluated by DTPM were analyzed with regard to receiver-operator-characteristics. FV as well as PR were significantly higher in Group 2 compared to Group 1. A threshold of 2.0 cm/sec for FV identified patients with malignancies with a sensitivity of 83% and specificity of 86%. In multivariable regression analysis, the best PR parameter for differentiating between malignant and inflammatory tumors was 97.5% percentile, whose value of 0.922 allowed for the recognition of pancreatic malignant tumors with a sensitivity of 62% and specificity of 83% (p < 0.001). In conclusion, Color Doppler ultrasound tissue perfusion parameters are a sensitive and specific tool in the differentiation between malignant and inflammatory pancreatic tumors.
Dynamic tissue perfusion measurement (DTPM) and single vessel flow measurement (SVFM) were assessed in differentiating inflammatory and malignant lesions of the pancreas. Sixty-nine patients (age ...62.0 ± 14.7; 33 Female and 36 Men; 40 with malignant and 29 with inflammatory lesions) in whom during the endoscopic ultrasound (EUS) of focal pancreatic lesions it was possible to adequately evaluate the flow in the color Doppler, and then perform a biopsy, were qualified for the study. The assessed DTPM parameters flow velocity (TFV), perfusion intensity (TPI), and resistive index (TRI) as well as the following SVFM parameters: flow velocity (FV), volume flow (VolF), and resistive index (RI) differed significantly between the malignant and inflammatory lesions (
< 0.005). TFV and TPI have slightly better discriminatory properties than the corresponding FV and VolF parameters (
< 0.10). Considering the Doppler parameters usually evaluated in a given method, the TPI = 0.009 cm/s (sensitivity 79%, specificity 92%, AUC 0.899,
< 0.001) was significantly better (
= 0.014) in differentiating between inflammatory and malignant pancreatic lesions in comparison to FV = 2.526 cm/s (sensitivity 79%, specificity 70%, AUC 0.731,
< 0.001). Tissue perfusion has better discriminatory properties in the differentiation of solid pancreatic lesions than the Doppler blood flow examination in the single vessel within the tumor.
Parenteral nutrition provides all essential nutrients directly into the blood stream without involvement of the gastrointestinal tract if conventional oral or enteral nutrition is not possible. ...The awareness of how important it is to provide adequate nutrition to the ill has accompanied people for a long time, as evidenced by the first attempts to conduct nutrition by bypassing the gastrointestinal tract taken as early as in the seventeenth century. The contemporary methods of nutrition have been established in the twentieth century and are still being improved. Nutrition is carried out using specially prepared formulations containing all necessary nutrients in the most absorbable form. These mixes are adapted to individual requirements, resulting from the underlying disease. They are available in the form of ready-made preparations or separate bottles. A method of nutrition is selected on the basis of its expected duration. Disease states are associated with increased catabolism with greater demand for energy and nutrients. This often leads to the development or exacerbation of malnutrition which affects convalescence after surgery and the general condition of the patient as well as increases the risk of hospital complications. As any medical procedure, parenteral nutrition is associated with a risk of side effects. However, the principles of care of the injection site and preparation of appropriate mixes tailored to patient’s needs help minimise the risk of adverse reactions. It is necessary to detect malnutrition early, estimate risk, create nutrition plans and include nutrition to therapy, which will improve the condition of hospitalised patients, accelerate their recovery, shorten hospital stay and reduce treatment costs.

The study was a retrospective evaluation of the occurrence of submucosal lesions (SML) and thickened wall (TW) of the upper gastrointestinal tract (UGIT) depending on age and sex.
Out of 20012 gastroscopies during the 4-year follow-up study, we enrolled 199 patients with pathological lesions in the wall of the UGIT. All patients underwent computed tomography and endoscopic ultrasound (EUS).
We analysed a total of 122 (78 males, 44 females, age: 64.0 ±12.9 years) out of 187 patients. 23.91% of SML in the oesophagus, 56.52% in the stomach, and 19.57% in the duodenum. A higher number of SMLs was found in men than in women (57.14% vs. 40.45%,
= 0.023), and the difference was greater over 50 years of age (85.71% vs. 40.00%,
= 0.031). We found less malignant SMLs compared to benign (35.87% vs. 64.13%,
= 0.026), especially in women (22.86% vs. 47.46%,
= 0.006). 26.67% of TW were in the oesophagus, 66.67% in the stomach, and 6.67% in the duodenum. There was a tendency towards increased incidence of TW over 50 years of age (8.58% vs. 18.30%,
= 0.074), which concerned men in particular (24.10% vs. 11.43%,
= 0.043). Until 65 years of age, these differences were significant for the oesophagus (27.27% vs. 0.00%,
= 0.044) and the stomach (25.93% vs. 4.00%,
= 0.029). As many as 70% of TW pathologies were malignant.
Submucosal lesions and TW of the upper gastrointestinal tract account for 0.61% of performed gastroscopies. They occur in men and usually over 50 years of age.
The meaning of the term high-quality endoscopy is gradually widening and consists of multiple elements. The highest possible quality of colonoscopies constitutes the basis of anticancer protective ...action by the identification and treatment of early precancerous lesions. Data from randomised trials demonstrate that only top quality endoscopy has a protective value against colon cancer morbidity. On the other hand, data coming from medical centres which do not meet high standards confirm the lack of protective anticancer value of low-quality colonoscopies and an increased rate of interval cancers. The fundamental indicators of high standards are: caecal intubation rate, adenoma detection rate, colonoscopy withdrawal time and bowel preparation. These parameters are gradually widened to include other factors such as: proximal colon polyp detection rate, retrieval rate of removed polyps, sedation practice and many others. One of the biggest challenges we have to face is the effort to perform endoscopic visual assessment of polyps in vivo. The principal aim of this action is to reduce the number of redundant histopathological tests and in general to reduce histopathology workload for very low-risk lesions, especially polyps ≤5 mm in diameter, this being the so called “resect and discard” strategy. One of useful tools is the assessment of the pattern of colonic crypt outlets, known as the pit pattern. The combination of the pit pattern and the Paris polyp classification determine high-risk lesions well in terms of advanced histology and technical problems with safe removal. Lesions originally found as unresectable should be reassessed and resected endoscopically in experienced expert centres.
Assessment of the entire surface of the mucosa of the diagnosed area is one of the key factors for high quality endoscopy. This is not obtainable without special preparation for endoscopy. Nationwide ...educational actions dedicated to informing patients how to prepare the bowel for colonoscopy have resulted in increased rate of detected pathological structures, clinically silent precancerous lesions in particular. Special pharmacological products for adequate colon preparation are available. Endoscopists are required to assess how well the colon was prepared for the procedure and include this information in their endoscopy report. The results of such efforts are already demonstrated by the decrease in the rate of colon cancer incidence and, most importantly, mortality. However, a similar effort is lacking as regards adequate preparation for gastroscopy despite it being the most common endoscopic examination. Generally, the required preparation for upper digestive tract endoscopy is limited to fasting for four to six hours before the procedure. Assessment of mucosa visibility is not required as an integral part of the gastroscopy report. This may be the reason for the low rate of early detection of gastric cancer and the high incidence of interval gastric cancer. Detection of advanced cancer results in low 5-year survival rates, with treatment mostly relying on extensive surgery. There are many publications available on premedication with simple mucolytic and anti-foaming agents for the detection of early precancerous lesions in the stomach, the bulk of them from the Far East, where gastric preparation for upper endoscopy is a routine procedure.
Wrzodziejące zapalenie jelita grubego i choroba Leśniowskiego-Crohna należą do grupy nieswoistych chorób zapalnych jelit o przewlekłym przebiegu i nieznanej etiologii. Z uwagi na niejednorodną ...patogenezę mają zróżnicowany obraz i przebieg kliniczny, co powoduje trudności diagnostyczne pomimo dostępności wielu wielospecjalistycznych badań. Wiadomo, że warunkiem koniecznym ustalenia ostatecznego rozpoznania jest potwierdzenie nieswoistej choroby zapalnej jelit w badaniu endoskopowym i histopatologicznym, niemniej jednak badania laboratoryjne są pomocne we wstępnej diagnostyce i odzwierciedlają zasięg oraz nasilenie procesu zapalnego. Białko C-reaktywne stanowi wykładnik procesu zapalnego, a występująca we wrzodziejącym zapaleniu jelita grubego niedokrwistość jest wynikiem zarówno przewlekłości choroby, jak i niedoboru żelaza wtórnego do utraty krwi. Diagnostyka laboratoryjna ma również zastosowanie w ocenie powikłań pozajelitowych dotyczących wątroby i dróg żółciowych, a także osteoporozy. Nieprawidłowe wyniki prób wątrobowych są częste w chorobach zapalnych jelit i mogą być spowodowane procesem zapalnym typowym dla tych chorób, ale też stosowanym leczeniem farmakologicznym. Chociaż powstają nowe metody wykorzystywane w diagnostyce przewodu pokarmowego, takie jak tomografia komputerowa, rezonans magnetyczny, pozytronowa tomografia emisyjna, to jednak badanie endoskopowe z oceną histopatologiczną pobranych wycinków jest (i jeszcze długo będzie) podstawową metodą diagnostyczną w rozpoznawaniu, monitorowaniu leczenia oraz nadzorze onkologicznym.
Zespół jelita drażliwego to przewlekłe i nawracające zaburzenia czynności jelit, charakteryzujące się bólami zwią‑ zanymi z wypróżnieniami oraz zmianą konsystencji lub częstości oddawania stolca. ...Stanowi poważny problem społeczny, ponieważ jest jedną z najczęstszych chorób przewodu pokarmowego. Etiopatologia schorzenia nie została dokładnie poznana i najpewniej jest wieloczynnikowa. Do rozważanych czynników patologicznych zali‑ cza się skład flory bakteryjnej jelita, nadwrażliwość trzewną, traumatyczne przeżycia, zaburzenia w zakresie funkcji serotoniny. W celu rozpoznania zespołu jelita drażliwego konieczne jest zastosowanie odpowiednich kryteriów dia‑ gnostycznych – aktualnie obowiązują kryteria rzymskie III. Szczególną uwagę trzeba zwrócić na możliwe występo‑ wanie objawów choroby organicznej lub obciążenie rodzinne rakiem jelita grubego. Pomimo wpływu na komfort życia zespół jelita drażliwego nie wpływa na czas przeżycia i nie wiąże się z poważnymi powikłaniami. Leczenie schorzenia powinno być zindywidualizowane, z uwzględnieniem sytuacji społecznej pacjenta. Należy także ocenić wpływ choroby na jakość życia. Leczenie farmakologiczne warto połączyć z edukacją chorego, a w niektórych przy‑ padkach z psychoterapią. Zalecane leczenie farmakologiczne zespołu jelita drażliwego obejmuje stosowanie 2–3 leków i powinno być nastawione na zniesienie lub złagodzenie najbardziej dokuczliwych objawów. Do stoso‑ wanych leków zaliczają się m.in. leki rozkurczowe, przeciwbiegunkowe, antagoniści receptora 5-HT3, agoniści receptora 5-HT4, antybiotyki, probiotyki, leki przeciwdepresyjne, leki psychotropowe. Obecnie trwają badania kli‑ niczne dotyczące zastosowania nowych substancji w leczeniu zespołu jelita drażliwego.

Ból w klatce piersiowej jest zawsze objawem wymagającym wnikliwej diagnostyki, zebrania dokładnych wywia‑ dów z pacjentem oraz badania lekarskiego. Należy do najczęstszych symptomów, jego pojawienie ...się wywołu‑ je niepokój u pacjentów i skłania do zgłoszenia się do lekarza bądź szpitala. Dolegliwości bólowe w tej okolicy wiązane są głównie z etiologią kardiologiczną. Jednakże podłoże sercowo-naczyniowe powyższych dolegliwo‑ ści stanowi tylko jedną z wielu przyczyn. Bóle w klatce piersiowej, pomimo różnej etiologii, mogą mieć podob‑ ny charakter i lokalizację, co wynika z unerwienia oraz anatomii i umiejscowienia różnych struktur wewnątrz klatki piersiowej. Źródłem bólu mogą być wszystkie struktury w obrębie klatki piersiowej, poza miąższem płuc‑ nym, który nie ma zakończeń czuciowych. Podstawowym elementem w diagnostyce różnicowej bólu w tej oko‑ licy jest wykluczenie stanów zagrożenia życia – ostrego zespołu wieńcowego, tętniaka rozwarstwiającego aorty, zatoru tętnicy płucnej i odmy opłucnowej. Pacjent z bólem w klatce piersiowej może trafić zarówno do kardio‑ loga, gastroenterologa, jak i do lekarza rodzinnego, pediatry, pulmonologa, neurologa, chirurga, ortopedy, reu‑ matologa, dermatologa oraz psychiatry. Ta interdyscyplinarność problemu skłoniła autorów do omówienia powyższego tematu. W pracy skupiono się na opisaniu pozasercowych bólów w klatce piersiowej i wykorzystaniu współczesnych metod diagnostyki, głównie w aspekcie ustalenia przyczyn gastroenterologicznych dolegliwości. W pracy nie analizowano etiologii sercowo-naczyniowej powyższych dolegliwości.
Trzustka ektopowa jest anomalią rozwojową polegającą na obecności tkanki trzustki w różnych lokalizacjach poza trzustką, bez łączności anatomicznej i naczyniowej z prawidłowo położonym narządem. ...Dzięki rozwo‑ jowi technik obrazowych stwierdza się ją coraz częściej. Jako czynniki etiopatogenetyczne w powstawaniu ektopii trzustkowej bierze się pod uwagę zaburzenia rozwoju wewnątrzpłodowego trzustki, metaplazję komó‑ rek multipotencjalnych, a na podstawie modeli zwierzęcych także mutacje określonych genów. Mnogość moż‑ liwych lokalizacji i odmienne typy histologiczne sprawiają, że obecność anomalii może mieć zróżnicowany przebieg kliniczny, od niemego do znacznie nasilonych objawów podmiotowych i przedmiotowych. W różni‑ cowaniu należy brać pod uwagę przede wszystkim zmiany złośliwe. Pewne rozpoznanie można ustalić po stwierdzeniu w badaniu histopatologicznym obecności tkanki trzustki, ale w szczególnych sytuacjach możliwe jest postawienie diagnozy z dużym prawdopodobieństwem tylko na podstawie badań endoskopowych i obra‑ zowych. W trakcie diagnostyki pojawia się problem, w jakim zakresie diagnozować pacjentów w kierunku zwy‑ kle nieistotnej klinicznie anomalii rozwojowej – jakie działania należy podjąć, aby nie narażać ich na postępo‑ wanie inwazyjne, a zarazem nie przeoczyć zmian złośliwych, błędnie rozpoznanych jako trzustka ektopowa. Leczenie podejmuje się jedynie w przypadku występowania objawów klinicznych lub stwierdzenia nowotworu w ektopii, a wybór metody leczenia zależy od wielkości i lokalizacji zmiany. Rokowanie jest pomyślne, nawet w przypadku wystąpienia następstw odległych. W pracy przedstawiono opis przypadku chorej z trzustką ektopową – na jej przykładzie zwrócono uwagę na znaczenie kliniczne oraz trudności w rozpoznawaniu i ewentualnym leczeniu tej wady rozwojowej.
