SUMMARY
The southern Pacific Islands region is highly seismically active and includes earthquakes from four major subduction systems, seafloor fracture zones and transform faults, and other sources ...of crustal seismicity. Since 1900, the area has experienced >350 earthquakes of M > 7.0, including 11 of M ≥ 8.0. Given the elevated threat of earthquakes, several probabilistic seismic hazard analyses have been published for this region or encompassed subregions; however, those that are publicly accessible do not provide complete coverage of the region using homogeneous methodologies. Here, we present a probabilistic seismic hazard model for the southern Pacific Islands that comprehensively covers the Solomon Islands in the northwest to the Tonga islands in the southeast. The seismic source model accounts for active shallow crustal seismicity with seafloor faults and gridded smoothed seismicity, subduction interfaces using faults with geometries defined based on geophysical data sets and models, and intraslab seismicity modelled by a set of ruptures that occupy the slab volume. Each source type is assigned occurrence rates based on subcatalogues classified to each respective tectonic context. Subduction interface and crustal fault occurrence rates also incorporate a tectonic component based on their respective characteristic earthquakes. We demonstrate the use of non-standard magnitude–frequency distributions to reproduce the observed occurrence rates. For subduction interface sources, we use various versions of the source model to account for epistemic uncertainty in factors impacting the maximum magnitude earthquake permissible by each source, varying the interface lower depth and segmentation as well as the magnitude scaling relationship used to compute the maximum magnitude earthquake and subsequently its occurrence rate. The ground motion characterization uses a logic tree that weights three ground motion prediction equations for each tectonic region. We compute hazard maps for 10 and 2 per cent probability of exceedance in 50 yr on rock sites, discussing the regional distribution of peak ground acceleration and spectral acceleration with a period of 1.0 s, honing in on the hazard curves and uniform hazard spectra of several capital or populous cities and drawing comparisons to other recent hazard models. The results reveal that the most hazardous landmasses are the island chains closest to subduction trenches, as well as localized areas with high rates of seismicity occurring in active shallow crust. We use seismic hazard disaggregation to demonstrate that at selected cities located above subduction zones, the PGA with 10 per cent probability of exceedance in 50 yr is controlled by Mw > 7.0 subduction interface and intraslab earthquakes, while at cities far from subduction zones, Mw < 6.5 crustal earthquakes contribute most. The model is used for southern Pacific Islands coverage in the Global Earthquake Model Global Hazard Mosaic.

Cryptococcal meningitis is a lethal disease with few therapeutic options. Induction therapy with fluconazole has been consistently demonstrated to be associated with suboptimal microbiological and ...clinical outcomes. Exposure to fluconazole causes dynamic changes in antifungal susceptibility, which are associated with the development of aneuploidy. The implications of this phenomenon for pharmacodynamics of fluconazole for cryptococcal meningitis are poorly understood. The pharmacodynamics of fluconazole were studied using a hollow-fiber infection model (HFIM) and a well-characterized murine model of cryptococcal meningoencephalitis. The relationship between drug exposure and both antifungal killing and the emergence of resistance was quantified. The same relationships were further evaluated in a recently described group of patients with cryptococcal meningitis undergoing induction therapy with fluconazole at 800 to 1,200 mg/day. The pattern of emergence of fluconazole resistance followed an "inverted U." Resistance amplification was maximal and suppressed at ratios of the area under the concentration-time curve for the free, unbound fraction of the drug to the MIC (
AUC:MIC) of 34.5 to 138 and 305.6, respectively. Emergence of resistance was observed
with an
AUC:MIC of 231.4. Aneuploidy with duplication of chromosome 1 was demonstrated to be the underlying mechanism in both experimental models. The pharmacokinetic (PK)-pharmacodynamic model accurately described the PK, antifungal killing, and emergence of resistance. Monte Carlo simulations from the clinical pharmacokinetic-pharmacodynamic model showed that only 12.8% of simulated patients receiving fluconazole at 1,200 mg/day achieved sterilization of the cerebrospinal fluid (CSF) after 2 weeks and that 83.4% had a persistent subpopulation that was resistant to fluconazole. Fluconazole is primarily ineffective due to the emergence of resistance. Treatment with 1,200 mg/day leads to the killing of a susceptible subpopulation but is compromised by the emergence of resistance.
Cryptococcal meningitis is a lethal disease with few treatment options. The incidence remains high and intricately linked with the HIV/AIDS epidemic. In many parts of the world, fluconazole is the only agent that is available for the initial treatment of cryptococcal meningitis despite considerable evidence that it is associated with suboptimal microbiological and clinical outcomes. Fluconazole has a fungistatic mode of action: it predominantly inhibits growth rather than causing fungal killing. Our work shows that the pattern of fluconazole activity is caused by the emergence of resistance in
not detected by standard susceptibility tests, with chromosomal duplication/aneuploidy as the main mechanism. Resistance emergence is related to drug exposure and occurs with the use of clinically relevant regimens. Hence, fluconazole (and potentially other agents that target 14-alpha-demethylase) is compromised by an intrinsic property that limits its effectiveness. However, this resistance may be potentially overcome by dosage escalation or the use of combination therapy.
Immunization via the respiratory route is predicted to increase the effectiveness of a SARS-CoV-2 vaccine. Here, we evaluate the immunogenicity and protective efficacy of one or two doses of a ...live-attenuated murine pneumonia virus vector expressing SARS-CoV-2 prefusion-stabilized spike protein (MPV/S-2P), delivered intranasally/intratracheally to male rhesus macaques. A single dose of MPV/S-2P is highly immunogenic, and a second dose increases the magnitude and breadth of the mucosal and systemic anti-S antibody responses and increases levels of dimeric anti-S IgA in the airways. MPV/S-2P also induces S-specific CD4
and CD8
T-cells in the airways that differentiate into large populations of tissue-resident memory cells within a month after the boost. One dose induces substantial protection against SARS-CoV-2 challenge, and two doses of MPV/S-2P are fully protective against SARS-CoV-2 challenge virus replication in the airways. A prime/boost immunization with a mucosally-administered live-attenuated MPV vector could thus be highly effective in preventing SARS-CoV-2 infection and replication.
Reports on the development and preliminary validation of the Child PTSD Symptom Scale (CPSS) for children and adolescents. The CPSS is a new instrument that was developed to assess the severity of ...Diagnostic and Statistical Manual of Mental Disorders (4th ed.; American Psychiatric Association, 1994) posttraumatic stress disorder symptoms in children exposed to trauma. The CPSS was administered to 75 school-age children approximately 2 years after the 1994 Northridge, California, earthquake. The psychometric properties of the CPSS show high internal consistency and test-retest reliability for both the total score and the three subscales. Convergent validity with the Child Post-Traumatic Stress Disorder Reaction Index (CPTSD-RI) was established. As expected, the correlations of the CPSS with depression and anxiety measures were lower than those with the CPTSD-RI, providing some support for discriminant validity of the CPSS. These results suggest that the CPSS is a useful tool for the assessment of posttraumatic stress disorder (PTSD) severity and for the screening of PTSD diagnosis among traumatized children.
Properly quantifying environmental heat stress (HS) is still a major challenge in livestock breeding programs, especially as adverse climatic events become more common. The definition of critical ...periods and climatic variables to be used as the environmental gradient is a key step for genetically evaluating heat tolerance (HTol). Therefore, the main objectives of this study were to define the best critical periods and environmental variables (ENV) to evaluate HT and estimate variance components for HT in Large White pigs. The traits included in this study were ultrasound backfat thickness (BFT), ultrasound muscle depth (MDP), piglet weaning weight (WW), off-test weight (OTW), interval between farrowing (IBF), total number of piglets born (TNB), number of piglets born alive (NBA), number of piglets born dead (NBD), number of piglets weaned (WN), and weaning to estrus interval (IWE). Seven climatic variables based on public weather station data were compared based on three criteria, including the following: (1) strongest G×E estimate as measured by the slope term, (2) ENV yielding the highest theoretical accuracy of the genomic estimated breeding values (GEBV), and (3) variable yielding the highest distribution of GEBV per ENV. Relative humidity (for BFT, MDP, NBD, WN, and WW) and maximum temperature (for OTW, TNB, NBA, IBF, and IWE) are the recommended ENV based on the analyzed criteria. The acute HS (average of 30 days before the measurement date) is the critical period recommended for OTW, BFT, and MDP in the studied population. For WN, WW, IBF, and IWE, a period ranging from 34 days prior to farrowing up to weaning is recommended. For TNB, NBA, and NBD, the critical period from 20 days prior to breeding up to 30 days into gestation is recommended. The genetic correlation values indicate that the traits were largely (WN, WW, IBF, and IWE), moderately (OTW, TNB, and NBA), or weakly (MDP, BFT, and NBD) affected by G×E interactions. This study provides relevant recommendations of critical periods and climatic gradients for several traits in order to evaluate HS in Large White pigs. These observations demonstrate that HT in Large White pigs is heritable, and genetic progress can be achieved through genetic and genomic selection.
This study examines the characteristics of a large number of extreme rain events over the eastern two-thirds of the United States. Over a 5-yr period, 184 events are identified where the 24-h ...precipitation total at one or more stations exceeds the 50-yr recurrence amount for that location. Over the entire region of study, these events are most common in July. In the northern United States, extreme rain events are confined almost exclusively to the warm season; in the southern part of the country, these events are distributed more evenly throughout the year. National composite radar reflectivity data are used to classify each event as a mesoscale convective system (MCS), a synoptic system, or a tropical system, and then to classify the MCS and synoptic events into subclassifications based on their organizational structures. This analysis shows that 66% of all the events and 74% of the warm-season events are associated with MCSs; nearly all of the cool-season events are caused by storms with strong synoptic forcing. Similarly, nearly all of the extreme rain events in the northern part of the country are caused by MCSs; synoptic and tropical systems play a larger role in the South and East. MCS-related events are found to most commonly begin at around 1800 local standard time (LST), produce their peak rainfall between 2100 and 2300 LST, and dissipate or move out of the affected area by 0300 LST.
Abstract Bacterial wilt, caused by Ralstonia pseudosolanacearum (Rpsol) and R . solanacearum (Rsol), poses a significant challenge to solanaceous plant cultivation worldwide, particularly in tropical ...and subtropical regions. Even though Brazil is recognized as one of the centres of origin and diversity of Rsol, in certain regions of this large country there is an emerging prevalence of Rpsol in production fields. Therefore, this study aimed to comprehensively investigate the adaptive traits of Rpsol and Rsol using a polyphasic approach. A diverse collection of isolates from both species was assessed for their physiological, biochemical, ecological and pathogenic traits. Rsol isolates demonstrated greater adaptability to a broader range of temperature, salinity and pH. They also exhibited enhanced abilities in biofilm formation and bacteriocin production. Conversely, Rpsol isolates exhibited a broader utilization of carbon sources and displayed a wider spectrum of resistance to inhibitory substances. Moreover, they demonstrated higher infectivity towards different solanaceous hosts, showing a faster invasion and colonization process in the roots and stems of tomato plants compared to Rsol isolates. Based on our findings, we concluded that Rsol exhibited greater physiological and ecological adaptability, while Rpsol showed greater pathogenic and biochemical adaptability. These results suggest that the coexistence of both species is maintained through a balance of distinct traits within each species.
Adenomatous polyps are the most common neoplastic findings uncovered in people who undergo colorectal screening or have a diagnostic workup for symptoms. It was common practice in the 1970s for these ...patients to have annual follow‐up surveillance examinations to detect additional new adenomas as well as missed synchronous adenomas. As a result of the National Polyp Study report in 1993, which demonstrated clearly in a randomized design that the first postpolypectomy examination could be deferred for 3 years, guidelines published by a gastrointestinal consortium in 1997 recommended that the first follow‐up surveillance be 3 years after polypectomy for most patients. In 2003, these guidelines were updated, colonoscopy was recommended as the only follow‐up examination, and stratification at baseline into lower and higher risk for subsequent adenomas was suggested. The 1997 and 2003 guidelines dealt with both screening and surveillance. However, it has become increasingly clear that postpolypectomy surveillance is now a large part of endoscopic practice, draining resources from screening and diagnosis. In addition, surveys have demonstrated that a large proportion of endoscopists are conducting surveillance examinations at shorter intervals than recommended in the guidelines. In the present paper, a careful analytic approach was designed addressing all evidence available in the literature to delineate predictors of advanced pathology, both cancer and advanced adenomas, so that patients can be more definitely stratified at their baseline colonoscopy into those at lower or increased risk for a subsequent advanced neoplasia. People at increased risk have either three or more adenomas, or high‐grade dysplasia, or villous features, or an adenoma ≥1 cm in size. It is recommended that they have a 3‐year follow‐up colonoscopy. People at lower risk who have one or two small (<1 cm) tubular adenomas with no high‐grade dysplasia can have a follow up in 5 to 10 years, whereas people with hyperplastic polyps only should have a 10‐year follow up as average‐risk people. Recent papers have reported a significant number of missed cancers by colonoscopy. However, high‐quality baseline colonoscopy with excellent patient preparation and adequate withdrawal time should minimize this and reduce clinicians' concerns. These guidelines were developed jointly by the US Multi‐Society Task Force on Colorectal Cancer and the American Cancer Society to provide a broader consensus and thereby increase utilization of the recommendations by endoscopists. Adoption of these guidelines nationally can have a dramatic impact on shifting available resources from intensive surveillance to screening. It has been shown that the first screening colonoscopy and polypectomy produces the greatest effects on reducing the incidence of colorectal cancer in patients with adenomatous polyps.
