Multi Detector Computed Tomography (MDCT) is widely used in the preoperative staging of gastric cancer. MRI has an emerging role in the evaluation of intestinal diseases, although its role in the ...staging of gastric cancers is still to be defined. The aim of our study was to compare the diagnostic accuracy of MDCT and Magnetic Resonance Imaging (MRI) in the diagnosis and preoperative staging of gastric cancer, in comparison with histopathology.
Twenty-five patients with an endoscopic diagnosis of gastric cancer underwent preoperative contrast-enhanced MDCT and MRI, blind to the results of endoscopy. MDCT (64 slices) was performed after oral administration of 800-1000 mL of tap water and scopolamine injection five minutes before the examination. The scan was performed in the axial plane before and after intravenous injection of iodinate contrast medium. Multiplanar reconstruction images were obtained on coronal and sagittal planes. MRI was performed with a 1.5 T Magnet, using the same patient's preparation, by acquiring T2-weighted HASTE sequences, with or without fat saturation (FS), True FISP (True fast imaging with steady state precession) and T1-weighted VIBE (Volumetric interpolated breath-hold examination) sequences, with and without FS, before and after contrast agent (gadolinium) i.v. injection. Gold standards (GS) were surgical and histopathological findings. Two groups of radiologists, blind each other, analyzed MRI images and MDCT findings, and related to GS results.
Detection rate of gastric lesions and T staging for gastric cancer were similar for MRI and MDCT (92%); MRI imaging was superior than MDCT in staging the T parameter (60% versus 48%); the accuracy of MRI imaging and 64-MDCT did not differ significantly in the evaluation of N staging (68% versus 72%).
Both MRI and MDCT were comparable in staging gastric cancer. MRI was more accurate in evaluation of T stage than MDCT, although both imaging modalities showed low accuracy in detection of early gastric cancer and in differentiation of T2 from T3 stage.
Le dispositif présenté ici a débuté en janvier 1991 dans le département de la Vienne. Cette longévité nous permet de porter un regard critique à la fois sur les conditions d’installation puis de ...viabilité d’un tel dispositif. Sa particularité provient surtout d’un couplage entre deux types d’interventions : les permanences d’évaluation clinique d’infirmiers ou d’éducateurs spécialisés en collège et lycée (PEC) à destination des élèves, d’une part et d’autre part, les équipes adultes ressources (EAR) à destination des professeurs et des autres membres de la vie scolaire. Ce partenariat a permis une meilleure régulation des adressages vers les services de pédopsychiatrie en les limitant considérablement, ce qui a un fort impact sur la liste d’attente des CMP. Il a également sensibilisé les infirmiers et éducateurs spécialisés qui y interviennent au poids de la scolarité dans le bien-être psychique des adolescents et leur a permis de mieux prendre conscience de l’organisation mais aussi des contraintes de l’éducation nationale. De leur côté, la fréquentation régulière des soignants, que ce soit dans le cadre des PEC ou des EAR, a constitué une réelle formation sur site à la psychopathologie de l’adolescence pour les professionnels de l’éducation nationale. Enfin, nous avons pu analyser statistiquement 11 années de fonctionnement. Les chiffres concernant la santé psychique des adolescents et les troubles dont ils souffrent sont exactement conformes à ceux de la littérature.
The organization presented here has existed since January 1991 in the department of Vienne. Its longevity permits us to have a critical opinion on the conditions necessary to install and thereafter to make function this dispositive. The originality of this is the association between two systems: permanence in secondary schools by nurses working in mental health services for the adolescents themselves (PEC) and another system for helping teachers and other members of the school community (EAR). This partner system allowed a better regulation for directing an adolescent with psychic discomfort to psychiatry services which has a real effect on the waiting list in medical and psychological centers. It has also sensitized mental health nurses to the impact of scolarization on presence of mind and allowed them to be conscious of difficulties which teachers have and the constraints of a school's organization. On the other hand, frequent meetings and discussions with mental health nurses and school personnel have resulted in a true formation within the school. We have also analyzed statistics after 11years of functioning. The results of our study are exactly identical to those of the literature.

Il est recommandé de les prendre en ...charge dans le cadre de parcours de soins de type filière fracture. L’objectif du travail est d’évaluer les facteurs de risque de mortalité et de nouvelle fracture à court terme des patients hospitalisés pour fracture sévère et pris en charge dans des filières fracturesPatients et méthodesLa cohorte CROSS (Conséquences et Risques d’une OStéoporose Sévère) est une étude française prospective nationale, multicentrique conduite dans 12 centres ayant une filière fracture. Les sujets inclus étaient âgés de plus de 60 ans et étaient hospitalisés pour une fracture récente (moins de 3 mois) et sévère (hanche, bassin, vertèbre, humérus) de faible traumatisme. À l’inclusion et à 2 ans, des variables démographiques, antécédents de fracture, fracture incidente, facteurs de risques osseux et de chutes, tests neuromusculaires, FRAX, histoire des traitements, comorbidités, score de Charlson et mesure de la densité minérale osseuse (DMO) par ostéodensitométrie ont été collectées. Pour évaluer les facteurs de risque de mortalité et de nouvelle fracture, une analyse de régression de Cox multivariée a été réalisée.RésultatsAu total, 895 patients (79 % femmes, âge médian de 81 ans 71–85) ont été inclus dans la cohorte avec une fracture sévère : vertébrale clinique (43,3 %), hanche (37,5 %), bassin (10,3 %) et humérus (11,1 %). 40 % des patients avaient un antécédent personnel de fracture de faible traumatisme, seuls 17,7% avaient reçu un traitement antiostéoporotique dans les 5 années précédant la fracture index alors que 21,4 % recevaient une supplémentation en calcium et 46,3 % en vitamine D. À l’inclusion, 48 % des patients avaient une ostéoporose densitométrique. Au cours du suivi des 2 ans (données complétées pour 95 % de la population), 116 fractures sévères (chez 110 patients, 12,9 %) et 80 décès (8,9 %) sont survenus. Un traitement anti-ostéoporotique a été prescrit chez 49,1 % des patients après la fracture (75 % bisphosphonates) et initié dans les 3 mois chez 63 % des patients.L’analyse multivariée a montré que la DMO basse au rachis (OR = 24,6, IC 95 % 2,84–247, p = 0,027), les chutes multiples (OR = 2,80, IC 95 % 1,11–6,65, p = 0,023), l’initiation d’un traitement antiostéoporotique (OR = 2,17, IC 95 % 1,19–4,08, p = 0,013) et l’âge (OR = 1,04, IC 95 % 1,01–1,08, p = 0,027) étaient significativement associés au risque de nouvelle fracture à 2 ans. L’usage d’une aide à la marche (RR = 2,71, IC 95 % 1,15–6,39, p = 0,02), la présence d’un diabète (RR = 3,70, IC 95 % 1,04–13,2, p = 0,044) ou d’un cancer métatastatique (RR = 15,5, IC 95 % 1,08–221, p = 0,043) étaient associés avec le risque de décès alors que l’initiation d’un traitement antiostéoporotique était associée à une diminution du risque de décès (RR = 0,19, IC 95 % 0,07–0,49), p < 0,001).ConclusionChez les patients hospitalisés pour une fracture sévère ostéoporotique et suivis dans une filière fracture, l’initiation d’un traitement antiostéoporotique est associée à une diminution du risque de mortalité. Dans cette population, une DMO basse est un déterminant important du risque de refracture à court terme.
Lattice vibrational property has been determined in ZrB(2) system using the temperature-dependent extended X-ray-absorption fine-structure (EXAFS) technique from room temperature to 28K. The smooth ...behavior of Debye-Waller factor curve with temperature is slightly abnormal for the first pair Zr-B. In order to reproduce this curve, an improved Einstein mode with two Einstein frequencies has been used. The quantitative analysis of temperature-dependent Debye-Waller factor of Zr-B pair shows one Einstein frequency is very high and the other is small. These frequencies correspond to the vibration of boron layer atoms and transition-metal layer atoms, respectively. Based on the Einstein mode with one frequency, the vibrational frequency for Zr-Zr pair has been also obtained. Zirconium diboride has two types of Zr-Zr interaction. One is in-plane and the other is out-of-plane along the high symmetry axis. Our analysis shows there is a little difference between in-plane Zr-Zr vibration and out-of-plane one. And the smaller Einstein vibrational frequency for the Zr-B shell is just between the two ones of the Zr-Zr shells. Our results show that the lattice vibrational behavior in ZrB(2) presents obvious particularity and anisotropy.
Radical prostatectomy has evolved considerably during the last 20 years, with one hand, a thorough knowledge of the anatomy of the prostate, and secondly, the introduction of new conservation ...techniques of neurovascular bundles. Despite this progress, the rate of post-prostatectomy erectile dysfunction varies between 30 and 90% depending on the series and this element impacts the quality of life after radical prostatectomy. The objective of this work was to conduct a review of the literature in order to evaluate the various protocols of post-radical prostatectomy erectile rehabilitation.
A systematic review of the literature from the basic PubMed search was performed using the following keywords. Therapy erectile dysfunction, radical prostatectomy, penile rehabilitation, phosphodiesterase 5 inhibitor, intracavernous injection.
The critical analysis of selected studies highlights the lack of standard protocol that established erectile rehabilitation early after radical prostatectomy. These studies have shown that early treatment, injections or intracavernous PDE5 inhibitors improved recovery of erections as recommended by the French Association of Urology.
Management of disorders of post-radical prostatectomy sexuality is essential, it must be organized in a well-established protocol combining pharmacological and therapeutic education assistances delivered by practitioners or nurses. It would need to develop in a clinical trial a standard protocol.
