Doxycycline-induced esophageal ulcers (DIEU) are rarely observed in endoscopy units. On the other hand doxycycline itself has been claimed as an offending drug in 1/4 out of about 1000 cases of drug ...induced esophageal ulcers reported in the literature so far. In this article we present two patients with typical history and endoscopic features of esophageal ulcers as an consequence of doxycycline therapy. The therapy consisted of doxycycline withdrawal along with treatment with proton pump inhibitors and sucralfate which had led to disappearance of chest discomfort within one week period. Fourteen days of the tratment beginning complete recovery of the mucosal defects has occurred in both patients. In conclusion, in case of chest pain and painful swallowing occurring in a person who takes doxycycline, DIEU has to be considered followed by the esophagoscopy which would confirm clinical suspicious with high specificity leading to correct diagnosis and treatment of this condition.
Background Combined oral contraceptive (COC) use is the most commonly used reversible method of birth control. The incorrect use of COCs is frequent and one of the most common causes of unintended ...pregnancies. Community pharmacists (CPs) are in a strategic position to improve COC use because they are the last health professional to interact with patients before drug use. Objective To evaluate the COC dispensing practices of CPs in a developing country. Method A cross-sectional study was conducted in community pharmacies of Assis and Ourinhos microregions, Brazil, between June 1, 2012, and October 30, 2012. Four simulated patients (SPs) (with counseled audio recording) visited community pharmacies with a prescription for Ciclo 21 registered (a COC containing ethinyl estradiol 30 mcg + levonorgestrel 15 mcg). The audio recording of every SP visit was listened to independently by 3 researchers to evaluate the COC dispensing practice. The percentage of CPs who performed a screening for safe use of COCs (i.e., taking of patients' medical and family history, and measuring of blood pressure) and provided counseling, as well as the quality of the screening and counseling, were evaluated. Results Of the 185 CPs contacted, 41 (22.2%) agreed to participate in the study and finished the study protocol. Only 3 CPs asked the SP a question (1 question asked by each professional), and all of the questions were closed-ended, viz., "do you smoke?" (n = 2) and "what is your age?" (n = 1). None of the CPs measured the patient's blood pressure. Six CPs provided counseling when dispensing COCs (drug dosing, 5 CPs; possible adverse effects, 2 CPs), and one CP provided counseling regarding both aspects. Conclusion The CPs evaluated did not dispense COC appropriately and could influence in the occurrence of negatives therapeutic outcomes such as adverse effects and treatment failure.

A case of mesalazine‐induced acute interstitial nephritis (AIN) in a 41‐year‐old patient with ulcerative colitis (UC) is reported here. Clinical symptoms such as fever and arthralgia, and laboratory ...findings such as eosinophilia and renal failure suggested AIN, which was confirmed by biopsy. With withdrawal of mesalazine and intravenous methylprednisolone the patient's renal function was recovered. It is observed that early discontinuation of mesalazine is associated with amelioration of interstitial nephritis in most patients, so the recommendation is that patients receiving mesalazine should undergo routine monitoring of renal function. Delayed diagnosis may lead to permanent renal function impairment.

This paper describes the concentrations of airborne bacteria and fungi in a cage housed laying hens facility, during a production year. Levels of airborne bacteria established in the air of the ...poultry house ranged from 1.02x10.sup.4 CFU/m.sup.3 measured in April, to 7.72x10.sup.4 CFU/m.sup.3 measured in December. Mean values of the total number of fungi ranged from 0.075x10.sup.4 CFU/m.sup.3 measured in September, to 8.56x10.sup.4 CFU/m.sup.3 measured in June. Established values of air temperature, relative humidity and air velocity were, generally, in accordance with the technology-predicted ranges. The determined number of bacteria and fungi in the air, as well as the statistically significant impact of microclimate conditions on their number in the air of the cage housed laying hens facility, are in accordance with considerations for standards set for air quality in livestock buildings and the development of reliable systems for monitoring the above factors. The aim is to create production that, besides the economic aspects, must include the protection of animals and people, as well as safety of food and environment. Broj bakterija izmjerenih u zraku objekta kretao se od 1,02x10.sup.4 CFU/m.sup.3, mjereno u travnju, do 7,72x10.sup.4 CFU/m.sup.3, mjereno u prosincu. Srednje vrijednosti ukupnog broja gljivica iznosile su od 0,075x10.sup.4 CFU/m.sup.3, izmjerene u rujnu, do 8,56x10.sup.4 CFU/m.sup.3, izmjereno u lipnju. Izmjerene vrijednosti temperature zraka, relativne vlaznosti i brzine strujanja zraka bile su vecinom u tehnologijom predvidenim rasponima. Utvrdeni broj bakterija i gljivica u zraku, kao i statisticki znacajan utjecaj mikroklimatskih uvjeta na njihov broj u zraku objekta kavezno drzanih nesilica, ukazuju na potrebu za postavljanjem standarda o kakvoci zraka u nastambama za zivotinje te razvoj pouzdanog sustava za pracenje navedenih cimbenika. Cilj je stvoriti proizvodnju koja, osim zadovoljenja ekonomskih cimbenika, mora sadrzavati i zastitu zivotinja i ljudi, kao i sigurnost hrane i okolisa.
