Introduction Exosomes are nanovesicles found in large quantities in biological fluids and tumors of patients with nasopharyngeal carcinoma (NPC). These tumor exosomes play an important role in tumor ...progression due to their immunosuppressive properties. In addition, it has been reported that the frequency and suppressor functions of CD4 + CD25highFOXP3 + CD127low regulatory T cells (Treg) are also higher in NPC patients than healthy donors. Interactions between NPC-derived exosomes and Treg remain unknown. Here we investigated their ability to induce, expand, activate and recruit human Treg. Material and methods Treg recruitment by exosomes derived from NPC cell lines (C15/C17-Exo), exosomes isolated from NPC patients’ plasma (Patient-Exo), and CCL20 was tested in vitro using Boyden chamber assays and in vivo using a xenograft SCID mouse model ( N = 5), both in the presence and absence of anti-CCL20 monoclonal antibodies (mAb). Impact of these NPC exosomes (NPC-Exo) on Treg phenotype and function was determined using adapted assays (FACS, Q-PCR, ELISA and MLR). Experiments were performed in comparison with exosomes derived from plasma of healthy donors (HD-Exo). Results CCL20 allowed the intra-tumoral recruitment of human Treg. NPC-Exo also facilitated Treg recruitment (3.30 ± 0.34-fold increase, P < .001), which was statistically significantly inhibited ( P < .001) by an anti-CCL20 blocking mAb. NPC-Exo also recruited conventional CD4 + CD25- T cells and mediated their conversion into inhibitory CD4 + CD25high cells. Moreover, NPC-Exo statistically significantly enhanced ( P = 0.0048) the expansion of human Treg, inducing the generation of Tim3Low Treg with increased expression of CD25 and FOXP3. Finally, NPC-Exo induced an overexpression of cell markers associated with Treg phenotype, properties and recruitment capacity. For example, GZMB mean fold change was 21.45 ± 1.75. These results were consistent with a stronger suppression of responder cells’ proliferation ( P < .001), and the secretion of immunosuppressive cytokines (IL10, TGFB1). Conclusion Interactions between NPC-Exo and Treg represent a newly defined mechanism that may be involved in regulating peripheral tolerance by tumors and in supporting immune evasion in human NPC.
Epstein-Barr virus (EBV) is associated with several malignant diseases that can be distinguished by their patterns of viral latent gene expression. We developed here an original peptidic approach to ...favor the induction of a specific CD4+ T-cell response against EBV latency II malignancies (Hodgkin's lymphoma, nasopharyngeal carcinoma, T/NK lymphoma). Previously, we selected 6 peptides derived from EBV nuclear antigen-1, latency membrane proteins (LMP)-1, and LMP-2 highly promiscuous for major histocompatibility complex class II molecules and showed their ability to induce interferon-γ-secreting CD4+ T cells. We confirmed here that all peptides used in cocktail are recognized by human CD4+ memory T cells from healthy donors, inducing a broad T-helper (Th)1 cytokine secretion interferon-γ, interleukin-2. Furthermore, we have generated EBV-specific CD4+ T-cell lines and proved their cytotoxic potential, not only on original models expressing latency II antigens (EBV-transformed T cell or monocyte), but also on lymphoblastoid cell lines expressing latency III antigens (lymphoblastoid cell lines). In addition, granzyme B enzyme-linked immunospot assays suggested that a part of this specific cytotoxic activity could be linked to the granule lytic pathway. Very importantly, we have showed that neither phenotypical changes nor functional activities of CD4+CD25+CD127(low)-regulatory T cells were observed in response to EBV+ peptides, avoiding any risk of aggravation of the preexisting immunosuppressive environment reported in EBV-associated malignancies. In conclusion, our promiscuous peptide cocktail could be used safely in immunotherapeutic approaches against EBV latency II malignancies, mainly to prevent relapse in high-risk patients further to classic treatments.
Acute appendicitis is the most common abdominal emergency. Its etiopathogenesis appears to be multifactorial. Several studies suggested a relationship between the development of acute appendicitis ...and some environmental factors. Air pollution predisposes some people to develop perforated appendicitis. However, data are relatively scarce and the results still controversial.
Determine the seasonal variation of acute appendicitis and study the association between perforated appendicitis and short-term exposure to climatic factors and to air pollutants.
A cross-sectional study was conducted including patients hospitalized in the general surgery department of Farhat Hached University Hospital in Sousse for acute appendicitis between January 1st and December 31st, 2014. Climatic conditions were collected from the National Institute of Meteorology. Data on air pollution were given by the National Agency for the Protection of the Environment and obtained by the modeling of the atmospheric pollution. For statistical analysis, we used mean concentrations of each environmental factor corresponding to the day of hospital admission and lagged by the 7 previous days. These factors were compared between the group of patients with perforated appendicitis and patients with nonperforated appendicitis.
We collected 246 cases of acute appendicitis. Perforated appendicitis was reported in 15.2% of the cases. The incidence of acute appendicitis was higher during summer. Compared to nonperforated appendicitis, perforated appendicitis was significantly associated with the mean relative humidity of the 5 day lag (p = 0.046), rainfall of the 7 day lag (p = 0.043), and consultation delay (p <10-3). Furthermore, perforated appendicitis was significantly associated with the daily mean concentration of carbon dioxide (p = 0.042), the 2- day lag mean concentration of particulate matter less than 10 μ (PM10 ) (p = 0.016), and the 2-day lag mean concentration of ozone (p = 0.048). After multivariate statistical analysis, predictive factors for perforated appendicitis were the consultation delay (OR: 1.621, 95% CI 1.288 - 2.039; p<10-3) and the 2 day lag mean concentration of PM10 (OR: 1.066, 95% CI 1.007- 1.130; p = 0.029).
Short-term exposure to particulate matter was associated with perforated appendicitis. Further large-scale studies are needed to support this conclusion.

La paralysie cérébrale (PC) est le handicap moteur le plus fréquent chez l’enfant. Le problème d’accès aux soins dans les pays à revenu faible ou intermédiaire appelle à des actions pour mettre en ...évidence les défaillances.
Notre objectif était de faire l’état des lieux et d’identifier les freins à la prise en charge de la PC spastique chez les enfants paralysés cérébraux admis dans notre service.
Nous avons mené une étude descriptive transversale incluant des enfants âgés de 24 mois et plus atteints de PC spastique ou dyskinétique hospitalisés dans notre service de médecine physique et de réadaptation, sur la période comprise entre juin 2020 et juin 2022. Les données ont été recueillies à partir du registre des injections de toxine botulique (TB), des dossiers médicaux des patients et d’un entretien téléphonique avec l’aidant principal du patient.
Nous avons inclus 42 enfants. L’âge moyen à la première consultation en MPR était de 34,12 mois. Le délai moyen entre deux séances d’injection de TB était de 8,64 mois. Onze patients ne faisaient pas de séances de kinésithérapie et un seul patient faisait de l’ergothérapie. L’orthophonie n’était pas accessible pour 14 patients. Les patients n’utilisaient pas d’appareillage orthopédique dans 42,9 % des cas. Neuf patients n’ont pas eu accès à une chirurgie jugée nécessaire.
Notre audit a mis l’accent sur plusieurs défaillances par rapport aux recommandations internationales, liées principalement au problème d’accès aux soins. En effet, au Royaume-Uni, Le « National Institute for Health and Care Excellence » (NICE) recommande d’adresser les enfants atteints de PC à des structures spécialisés qui peuvent assurer les soins mais aussi la scolarisation de cette population.
La prise en charge de la PC est un défi pour les médecins tunisiens. Vu les difficultés d’accès pour nos patients, la qualité des soins doit être évaluée en permanence.
Introduction : Plusieurs données cliniques et épidémiologiques pointent en faveur d’un lien possible entre l’exposition tabagique et la dermatite de contact (DC). Objectifs : Préciser les différences ...cliniques et épidémiologiques des DC chez les sujets tabagiques et non-tabagiques, et déterminer l’influence du tabagisme sur le profil allergologique de la DC. Méthodes : Étude rétrospective descriptive portant sur tous les patients qui ont consulté au service de Médecine du Travail et de Pathologie Professionnelle du CHU Farhat Hached de Sousse (Tunisie) durant une période de 8 ans pour exploration d’une DC et qui ont été testés à la Batterie Standard Européenne (BSE). Résultats : Au total, 767 patients ont été colligés au cours de la période d’étude dont 40% étaient tabagiques. Le groupe des tabagiques était caractérisé par une prédominance masculine (p=10-3) et une ancienneté professionnelle plus importante par rapport aux non-tabagiques (p=0.01). Les antécédents personnels d’atopie étaient prédominants chez les non-tabagiques (p=0,02). Parmi les allergènes de la BSE, il y avait une association significative entre le tabagisme et la DC due aux métaux (chrome, cobalt) et aux conservateurs. Après régression logistique binaire, les variables associées à l’exposition tabagique étaient le genre masculin (OR=12,12 ; IC 95%=6,07 - 24,21 ; p=10-3), l’allergie au Kathon CG (OR=3,69 ; IC 95%=1,24 - 10,81 ; p=0,018), et l’atteinte de la main droite (OR= 2,83 ; IC 95%=1,29 - 6,17 ; p=0,005). Conclusion : Notre étude a révélé l’existence d’un effet du tabagisme sur les caractéristiques cliniques et allergologiques de la DC.
Le travail de bureau est devenu très répandu surtout depuis l’instauration du télétravail. Il expose les salariés à des risques professionnels de diverses natures dont la dermatite de contact ...allergique (DCA) qui représente une part non négligeable de l’ensemble de ces risques.
Identifier les principaux allergènes en cause de DCA chez les employés de bureau dans la région du centre tunisien.
Il s’agit d’une étude épidémiologique descriptive rétrospective qui a porté sur tous les patients ayant consulté durant la période entre 1991 et 2020, à l’unité de dermato-allergologie au service de médecine du travail du CHU Farhat Hached de Sousse) dans le cadre du bilan étiologique de leur DCA et ayant bénéficié d’un patch test à la batterie standard européenne (BSE).
Au total, 173 patients employés de bureau ont été recensés durant la période d’étude. L’âge moyen était de 39,5±10,5 ans, avec une prédominance masculine (57,9 % des cas). Vingt-deux patients avaient un antécédent personnel allergique. L’ancienneté professionnelle moyenne au poste était de 12±9,5 ans. La localisation initiale des lésions était au niveau des mains dans 40,8 % des cas. Les lésions étaient érythémato-vésiculeuses dans 52,2 % des cas. Un aspect suintant des lésions était associé dans 19,9 % des cas. Les allergènes les plus incriminés à la BSE étaient le nickel dans 23,1 % des cas, le bichromate de potassium dans 15,6 % des cas et le cobalt dans 14,4 % des cas. Seulement 15,3 % des cas ont bénéficié d’une déclaration de leur DCA en maladie professionnelle.
Nos résultats montrent que les métaux constituent la principale cause de DCA chez les employés de bureau. Il est ainsi judicieux d’instaurer des mesures de prévention adéquates et rigoureuses afin de réduire ce risque immergeant.
