Se realizó un estudio descriptivo transversal con el objetivo de evaluar las necesidades de aprendizaje sobre prevención preconcepcional de Cardiopatías Congénitas de las mujeres en edad fértil en ...tres consultorios del médico de la familia del policlínico universitario "Carlos Verdugo" y adolescentes de la Escuela Secundaria Básica Urbana "Generación del Centenario" de la ciudad de Matanzas. Se consultó a un panel de especialistas para determinar los aspectos a explorar en los cuestionarios y a cada ítems se le asignó un valor, llevándose a una escala ajustada para tres categorías de conocimiento: bien, regular y mal; y tres categorías de necesidades educativas sentidas: alta, media y baja. Se demuestra que existe una insuficiente prevención preconcepcional de Cardiopatías Congénitas en el área de salud correspondiente al Policlínico Universitario Carlos Verdugo, pues el nivel de conocimiento sobre el tema es deficiente en la comunidad. Se recomienda el diseño de un sistema de actividades educativas.

Se recomienda el diseño de un sistema de actividades educativas.We carried out a transversal descriptive study with the objective of evaluating learning necessities on pre-conceptional prevention of congenital heart diseases in fertile-age women in three family doctor´s offices of the Teaching Polyclinic "Carlos Verdugo" and in teenagers of the Urban High School "Generación del Centenario" of Matanzas. A panel of specialists was interviewed to determine the questions to be included in the poll and each item received a value reflected in a scale adjusted to three knowledge categories: excellent, regular and bad; and three categories of educative necessities: high, media and low. It showed that there is an insufficient pre-conceptional prevention of congenital heart diseases in the health area of the Teaching Polyclinic Carlos Verdugo, because the level of knowledge on the theme in the community is deficient. We recommend designing a system of educative interventions.
Clinical, electrophysiological, genetic and biochemical deficiencies variability were evaluated in 52 patients diagnosed of mitochondrial respiratory chain diseases (MRCD).
26 men and 26 women, aged ...19 to 79 years, were tested by clinical examination, electrophysiological techniques, muscle biopsy and genetic and biochemical studies.
The patients were classified into seven phenotypes: myopathy, chronic progressive external ophthalmoplegia, progressive ophthalmoplegia plus ataxia, Kearns-Sayre syndrome, mitochondrial encephalomyopathy with lactic acidosis and stroke episodes (MELAS), myoclonic encephalopathy with ragged-red fibers (MERRF), and encephalopathies. Each phenotype may begin by different ways. The electromiography showed myopathy in 39 cases and various types of neuropathy in 10. Ragged-red COX negative fibers or widespread electron microscopic abnormalities were found in 47 cases. Simple deletions, multiple deletions and three different point mutations were observed. Deficiency of complexes I, II, III and IV were found alone or in different associations.
MRCD shows wide variations in clinical, genetic and biochemical studies. Some patients with nonspecific manifestations, mainly of central nervous system, need careful attention and to be on account of diagnostic suspicion.

To determine the frequency and the epidemiological characteristics of biological-exposure incidents occurring among healthcare personnel.

Absolute and relative frequencies were calculated for several variables (position held, area of care, type of injuring object, activity, etc) and for the different categories of each variable.
There were 20,235 registered incidents. Annual incidence rates were as follows: 1994, 51 per 1,000; 1995, 58 per 1,000, 1996, 54 per 1,000; and 1997, 59 per 1,000. Mean age of accident victims was as follows: 1994, 35.68 (standard deviation SD, 16.26); 1995, 33.6 (SD, 11.9); 1996,38.2 (SD, 17.27); and 1997, 36.7 (SD, 16.33) years. Of the 20,235 incidents, 15,860 (80.7%) occurred to women; 50% (9,833) accidents were among nursing staff. The type of incident most frequently reported was percutaneous injury (81.1%). The highest frequency of accidents was seen in medical and surgical areas (28% and 25.6%, respectively). Blood and blood products were the most commonly involved material (87.6%). Administration of intramuscular or intravenous medication was the activity associated with the highest accident rate (20.3%). The most frequent immediate action in response was rinsing and disinfecting (65.6%).
The incident registry was highly stable in terms of incidence rates over the observation period and served to highlight the large number of incidents recorded each year. The potential implications of the results are the need to explore reasons for increased exposures in certain areas, with the aim of focusing prevention efforts, and, similarly, to establish the factors associated with diminished incidence rates to model successful measures.
A 9-year-old boy diagnosed with gingival hypertrophy and with a history of West syndrome and associated low platelet levels underwent gingival reduction surgery. Because difficult intubation was ...foreseen, the fiberoptic tube was inserted through the nose with the patient breathing spontaneously under remifentanil for sedation and analgesia. The procedure was carried out under balanced general anesthesia and with standard monitoring. At the end of gingivectomy, a tracheostomy was performed and the patient was transferred to the pediatric intensive care unit for postoperative observation.
