Glioblastoma multiforme is the most aggressive type of primary brain tumor. Glioblastoma growth dynamics vary widely across patients, making it difficult to accurately gauge their response to ...treatment. We developed a model-based metric of therapy response called Days Gained that accounts for this heterogeneity. Here, we show in 63 newly diagnosed patients with glioblastoma that Days Gained scores from a simple glioblastoma growth model computed at the time of the first postradiotherapy MRI scan are prognostic for time to tumor recurrence and overall patient survival. After radiation treatment, Days Gained also distinguished patients with pseudoprogression from those with true progression. Because Days Gained scores can be easily computed with routinely available clinical imaging devices, this model offers immediate potential to be used in ongoing prospective studies.
The efficacy of simvastatin in critically ill patients with coronavirus disease 2019 (Covid-19) is unclear.
In an ongoing international, multifactorial, adaptive platform, randomized, controlled ...trial, we evaluated simvastatin (80 mg daily) as compared with no statin (control) in critically ill patients with Covid-19 who were not receiving statins at baseline. The primary outcome was respiratory and cardiovascular organ support-free days, assessed on an ordinal scale combining in-hospital death (assigned a value of -1) and days free of organ support through day 21 in survivors; the analyis used a Bayesian hierarchical ordinal model. The adaptive design included prespecified statistical stopping criteria for superiority (>99% posterior probability that the odds ratio was >1) and futility (>95% posterior probability that the odds ratio was <1.2).
Enrollment began on October 28, 2020. On January 8, 2023, enrollment was closed on the basis of a low anticipated likelihood that prespecified stopping criteria would be met as Covid-19 cases decreased. The final analysis included 2684 critically ill patients. The median number of organ support-free days was 11 (interquartile range, -1 to 17) in the simvastatin group and 7 (interquartile range, -1 to 16) in the control group; the posterior median adjusted odds ratio was 1.15 (95% credible interval, 0.98 to 1.34) for simvastatin as compared with control, yielding a 95.9% posterior probability of superiority. At 90 days, the hazard ratio for survival was 1.12 (95% credible interval, 0.95 to 1.32), yielding a 91.9% posterior probability of superiority of simvastatin. The results of secondary analyses were consistent with those of the primary analysis. Serious adverse events, such as elevated levels of liver enzymes and creatine kinase, were reported more frequently with simvastatin than with control.
Although recruitment was stopped because cases had decreased, among critically ill patients with Covid-19, simvastatin did not meet the prespecified criteria for superiority to control. (REMAP-CAP ClinicalTrials.gov number, NCT02735707.).
Head and neck squamous cell carcinoma (HNSCC) is an aggressive tumor with low response rates to frontline PD-1 blockade. Natural killer (NK) cells are a promising cellular therapy for T cell ...therapy-refractory cancers, but are frequently dysfunctional in patients with HNSCC. Strategies are needed to enhance NK cell responses against HNSCC. We hypothesized that memory-like (ML) NK cell differentiation, tumor targeting with cetuximab, and engineering with an anti-EphA2 (Erythropoietin-producing hepatocellular receptor A2) chimeric antigen receptor (CAR) enhance NK cell responses against HNSCC.
We generated ML NK and conventional (c)NK cells from healthy donors, then evaluated their ability to produce IFNγ, TNF, degranulate, and kill HNSCC cell lines and primary HNSCC cells, alone or in combination with cetuximab, in vitro and in vivo using xenograft models. ML and cNK cells were engineered to express anti-EphA2 CAR-CD8A-41BB-CD3z, and functional responses were assessed in vitro against HNSCC cell lines and primary HNSCC tumor cells.
Human ML NK cells displayed enhanced IFNγ and TNF production and both short- and long-term killing of HNSCC cell lines and primary targets, compared with cNK cells. These enhanced responses were further improved by cetuximab. Compared with controls, ML NK cells expressing anti-EphA2 CAR had increased IFNγ and cytotoxicity in response to EphA2+ cell lines and primary HNSCC targets.
These preclinical findings demonstrate that ML differentiation alone or coupled with either cetuximab-directed targeting or EphA2 CAR engineering were effective against HNSCCs and provide the rationale for investigating these combination approaches in early phase clinical trials for patients with HNSCC.
Mental health problems are a major source of morbidity and mortality for children and adolescents, affecting 15% to 20% of those under 18 years of age in the US.

Despite knowing much about mental health conditions affecting children, many speculate that the lack of standardized approaches to patient care contribute to poor outcomes, including substantial diagnostic variation, few remissions, risk for relapse or recidivism, and, ultimately, greater mortality due to an inability to accurately predict who will make a suicide attempt.
Studies support this over-reliance on the "art of medicine" (ie, subjective judgment without use of standardized measures), finding that only 17.9% of psychiatrists and 11.1% of psychologists in the US routinely administer symptom rating scales to their patients, despite studies suggesting that when using clinical judgment alone, mental health providers detect deterioration for only 21.4% of patients.

Gliomas are notoriously aggressive, malignant brain tumors that have variable response to treatment. These patients often have poor prognosis, informed primarily by histopathology. Mathematical ...neuro-oncology (MNO) is a young and burgeoning field that leverages mathematical models to predict and quantify response to therapies. These mathematical models can form the basis of modern "precision medicine" approaches to tailor therapy in a patient-specific manner. Patient-specific models (PSMs) can be used to overcome imaging limitations, improve prognostic predictions, stratify patients, and assess treatment response in silico. The information gleaned from such models can aid in the construction and efficacy of clinical trials and treatment protocols, accelerating the pace of clinical research in the war on cancer. This review focuses on the growing translation of PSM to clinical neuro-oncology. It will also provide a forward-looking view on a new era of patient-specific MNO.

4. We call for wider teaching of the tools and skills that facilitate evidence‐based conservation and also suggest that providing online teaching materials in multiple languages could be beneficial for improving global understanding of other subject areas.

1. Zum effektiven Handeln brauchen wir in der nächsten Generation von Naturschützern und Naturschutzmanagern kritische Denker mit einem grundlegenden Verständnis von evidenz‐basierten Entscheidungen und dem Wert von Evidenz‐Synthesen.
2. Wenn wir als Dozenten diese Aspekte nicht zu einem zentralen Bestandteil unserer Lehre machen, bereiten wir unsere Studierenden nur unzureichend darauf vor, wirksam zur zukünftigen Naturschutzpraxis beitragen zu können.
3. Um diese Aufgabe zentral anzugehen, haben wir Open‐Access‐Online‐Unterrichtsmaterialien in mehreren Sprachen erstellt, die in den ‚Applied Ecology Resources‘ gespeichert sind. Bisher haben mindestens 117 Dozenten aus 23 Ländern die Bedeutung dieses Themas erkannt und es entsprechend schon in ihre Lehre eingebaut beziehungsweise sind derzeit dabei, Ihren Studierenden die Möglichkeiten der Bewertung und Verwendung von direkten Belegen bei der Entscheidungsfindung im Naturschutz zu vermitteln. Diese Lehre umfasst 145 Kurse ‐ sowohl auf Bachelor und MSc/PhD‐Ebene wie auch Kurse zur beruflichen Weiterbildung.
4. Wir empfehlen für die Zukunft eine noch umfassendere Vermittlung der Instrumente und Fähigkeiten, die einem evidenzbasierten Naturschutz zugrunde liegen. Darüber hinaus stellen wir die Vorteile einer Bereitstellung von mehrsprachigen Online‐Unterrichtsmaterialien für eine bessere globale Wissensvermittlung auch für andere Fachbereiche heraus

1. 生物多様性をより効果的に保全していくために、次世代の実務者はエビデンスに基づいた意思決定の方法とエビデンスを統合することの価値を深く理解し、また批判的な思考を備える必要がある。
2. 生物多様性保全に関する教育課程においてこの課題を取り扱わなければ、現在の学生が今後の保全活動に対して効果的な貢献を行うようにはならないだろう。
3. この問題を解決するために、本稿ではエビデンスに基づいた保全についてのオープンアクセス・オンライン教材を多言語で作成し、それらをApplied Ecology Resourcesに収蔵した。これまでに、23カ国、145の学部・大学院教育、また専門的能力の開発コースに携わる117人がこの問題の重要性について同意し、保全に関する意思決定におけるエビデンスの評価や利用の技能についての講義を既に行っているか、これから行おうとしている。
4. エビデンスに基づいた保全を促進する手段や技能をより広く教育していくことは今後益々必要となるだろう。また、我々の取り組みのようにオンライン教材を多言語で提供することで、他の研究分野でも世界的な理解の向上を図ることができると考える。

1. Para ser eficaz, a próxima geração de conservacionistas e gestores de vida selvagem tem de ter a capacidade de desenvolver pensamento crítico e deve possuir um conhecimento profundo sobre como tomar decisões com base em evidência, entendendo também o valor de sintetizar a mesma.
2. Se, como educadores, não fazemos destas prioridades uma parte nuclear do que leccionamos, estaremos a falhar na preparação dos nossos estudantes, impossibilitando‐os de realizar uma contribuição efectiva na prática da conservação de natureza.
3. Para ajudar a ultrapassar este problema criámos materiais de ensino on‐line, com acesso aberto e em várias línguas, disponíveis em “Applied Ecology Resources”. Até à data, 117 educadores de 23 países reconheceram a importância deste processo e já leccionam, ou estão prestes a fazê‐lo, ensinando como avaliar ou usar evidências na tomada de decisões, aplicadas à conservação da natureza. Aqui se incluem 145 unidades curriculares do primeiro e segundo ciclos do ensino superior, bem como cursos de desenvolvimento profissional.
4. Apelamos à expansão do ensino de ferramentas e competências que facilitem a tomada de decisão na área da conservação da natureza com base em evidência e sugerimos que distribuir material de ensino online em múltiplas línguas, poderá ser também benéfico no sentido de melhorar a compreensão global noutras áreas de conhecimento.

2. Si, en tant qu'éducateurs, nous ne faisons pas de ces priorités un élément central de ce que nous enseignons, nous ne préparons pas nos élèves à apporter une contribution efficace à la conservation de la nature.
3. Pour aider à surmonter ce problème, nous avons créé du matériel pédagogique en ligne en libre accès dans plusieurs langues qui est stocké sur le site internet Applied Ecology Resources.
