To identify work process-related causes associated with nursing errors reported in newspapers.
This was a documentary and qualitative study based on the work process theory and hermeneutic analysis ...that examined 112 news articles published between 2012 and 2016 in 21 high-circulation Brazilian newspapers, organized and codified using Atlas.ti software.
The causes associated with the reported errors were associated with workforce (lack of professionals and training, turnover, work overload, lack of information, recklessness, negligence, and distraction); work instruments (similar labels or packages, storage, lack of product identification and information, and medical prescriptions); and the object of nursing work (overcrowding and specific characteristics of patient).
Analysis of the possible causes of reported errors identified the negative outcomes of nursing work, while also identifying elements of the work process that influenced these results. The findings emphasize the importance of understanding these errors so they can be avoided and of reviewing nursing work conditions to guarantee quality and safety of care.
RESUMO Objetivo Identificar as causas relacionadas ao processo de trabalho que estão associadas aos erros de enfermagem noticiados pelos jornais. Método Estudo documental, qualitativo, baseado na ...teoria do processo de trabalho e análise hermenêutica. Foram analisadas 112 notícias de 21 jornais de grande circulação no Brasil, no período de 2012 a 2016, organizadas e codificadas no software Atlas.ti. Resultados As causas associadas aos erros noticiados foram relacionadas à força de trabalho ( deficit de profissionais e capacitação, rotatividade, sobrecarga, falta de informação, imprudência, negligência e distração); aos instrumentos de trabalho (semelhança de rótulos/embalagens, armazenamento, falta de identificação e informação de produtos e prescrição médica) e ao objeto de trabalho (particularidades dos pacientes e superlotação). Conclusão A análise das possíveis causas atribuídas aos erros noticiados permitiu tornar público não somente o resultado negativo do trabalho de enfermagem, mas também os outros elementos do processo de trabalho que influenciam esse resultado. Por isso a importância de compreender esses erros para que sejam evitados, e as condições de trabalho da enfermagem, revistas, para a garantia da qualidade e da segurança.
RESUMEN Objetivo Identificar las causas relacionadas con el proceso laboral que están asociadas con los errores de enfermería noticiados por los periódicos. Método Estudio documental, cualitativo, basado en la teoría del proceso laboral y análisis hermenéutico. Fueron analizadas 112 noticias de 21 periódicos de gran circulación en Brasil, en el período de 2012 a 2016, organizadas y codificadas en el software Atlas.ti. Resultados Las causas asociadas con los errores noticiados estuvieron relacionadas con la fuerza de trabajo (déficit de profesionales y capacitación, rotatividad, sobrecarga, falta de información, imprudencia, negligencia y distracción); con los instrumentos de trabajo (semejanza de etiquetas/envases, almacenamiento, falta de identificación e información de productos y prescripción médica) y con el objeto de trabajo (particularidades de los pacientes y abarrotado). Conclusión El análisis de las posibles causas atribuidas a los errores noticiados permitió hacer público no solo el resultado negativo del trabajo de enfermería, sino también los demás elementos del proceso laboral que influencian dicho resultado. En virtud de ello, es importante comprender esos errores a fin de que se los evite y que se revisen las condiciones del trabajo enfermero, para la garantía de la calidad y la seguridad.

The analysis revealed the categories: The HOPE Project: impressions of its nurses and Nursing on the hospital ship SS HOPE: Brazilian impressions. These categories revealed a Nursing Service similar to that of a general hospital, ordered by scales and which functioned uninterruptedly in the three shifts.
The Nursing Service consisted exclusively of nurses; followed a hierarchical system; with accumulation of administrative and care functions; strict, precise and technical execution of the work. The language and cultural barriers do not seem to have compromised the Exchange and the work system in counterparts.

To analyze the nursing errors reported by the journalistic media and interpret the main implications of this communication for the visibility of this problem.
Documental research, qualitative, ...descriptive and exploratory, with data collected in news reports from Brazil and Portugal, analyzed through hermeneutics with resources of Atlas Software.
We analyzed 112 news items published between 2012 and 2016 that resulted in six categories: Year - highest occurrence in 2012; Age group of the patient - children; Professional category - nurses; Type of error - medication; Outcome - death; Possible attributed cause - occupational conditions.
Nursing mistakes are a challenge for the profession, and the way they are communicated by the media is not very explanatory, contributing to a negative visibility of the profession, and to making society insecure. Improving the way they are served in the media contributes to the visibility of the problem without affecting the professional image.
Estudo qualitativo, documental, interpretativo, com coleta de dados realizada em jornais de dois países, com o objetivo de interpretar o modo como as notícias sobre erros de enfermagem na mídia podem ...contribuir para a compreensão das questões relacionadas à segurança do paciente. Realizada análise hermenêutica com base no Processo de Trabalho e na Teoria do Agir Comunicativo, com auxílio do software Atlas.ti. Resultaram três categorias temáticas que permitiram compreender os problemas relacionados ao financiamento da saúde, às oportunidades de proporcionar a compreensão do problema à sociedade e às possibilidades de representar e fortalecer a identidade profissional. Conclui-se que os meios de comunicação podem ser importantes aliados da enfermagem na busca por melhores condições de trabalho e para dar visibilidade à problemática em si e ao subfinanciamento da saúde, pois as notícias permitem aproximações e distanciamentos, se inseridas no contexto histórico, político e social dos envolvidos.
ABSTRACT Objective: to interpret, from the perspective of the Theory of Communicative Action, how the outcome of nursing errors can become attractive to the media, highlighting the main implications ...for the image of the profession and the imaginary of society. Method: qualitative research, carried out in documentary sources using news published in the major newspapers available online in two countries, Brazil and Portugal, from 2012 to 2016. The analysis of the findings was carried out following the steps of hermeneutics, based on the Theory of Communicative Action. The data were organized and coded in the ATLAS.ti software. Results: the research included 112 published news. Four categories emerged from the analysis: The highlights in the headlines - The beginning of persuasion; Combining image and initial text - An explosive mix; The error that is not an error - The error that is a crime; and Applying the validity claims in the discourses. Conclusion: the media are continuous producers of ideologies and, therefore, possess social responsibility by inducing misinterpretations that can negatively interfere in the nurse-patient interaction. Giving greater emphasis to the outcome of the error, the media influences negatively the people perception of nursing labour which has a unique social importance.
RESUMEN Objetivo: interpretar, desde la perspectiva de la Teoría de la Acción Comunicativa, de qué manera el resultado de los errores de Enfermería puede volverse atractivo para los medios de comunicación, destacando las principales implicancias para la imagen de la profesión y para el imaginario de la sociedad. Método: investigación cualitativa realizada en una fuente documental caracterizada por noticias de periódicos de grande circulación disponibles en línea en dos países, Brasil y Portugal, en el período de 2012 a 2016. El análisis de los hallazgos se realizó siguiendo los pasos de la hermenéutica, con fundamentación en la Teoría de la Acción Comunicativa. Los datos se organizaron y codificaron en el software ATLAS.ti. Resultados: la investigación estuvo compuesta por 112 noticias; y surgieron cuatro categorías durante el análisis: Los puntos destacados en el no título de la noticia - El inicio de la persuasión; Combinación de imagen y texto inicial - Una mezcla explosiva; El error que no es error - El error que es un delito; y Aplicación de las pretensiones de validez en los discursos. Conclusión: los medios de comunicación son continuos productores de ideologías y, por ese motivo, tienen una responsabilidad social al inducir interpretaciones erróneas que pueden interferir negativamente en la interacción enfermero-paciente, al dar mayor énfasis al resultado del error, desacreditando así un trabajo de importancia social sin igual.

Método: pesquisa qualitativa realizada em fonte documental caracterizada por notícias de jornais de grande circulação disponíveis online em dois países, Brasil e Portugal, no período de 2012 a 2016. A análise dos achados foi realizada seguindo os passos da hermenêutica, com fundamentação na Teoria do Agir Comunicativo. Os dados foram organizados e codificados no software Atlas.ti. Resultados: a pesquisa foi composta por 112 notícias; e, da análise, resultaram quatro categorias: Os destaques no título da notícia - o início da persuasão; Combinação imagem e texto inicial - uma mistura explosiva; O erro que não é erro - o erro que é crime; e Aplicando as pretensões de validade nos discursos. Conclusão: os meios de comunicação são produtores contínuos de ideologias e, por isso, possuem responsabilidade social ao induzir interpretações equivocadas que podem interferir negativamente na interação enfermeiro-paciente, ao dar maior ênfase para o desfecho do erro, desacreditando, com isso, um trabalho de importância social ímpar.

ABSTRACT Objective: To know the social representations of comfort for patients’ family members in palliative care in intensive care. Method: Descriptive, qualitative study, theoretical framework ...adopted was Social Representations. 30 family members of patients admitted to an intensive care unit in palliative care participated. Data were collected through semi-structured individual interviews, organized and analyzed using the Collective Subject Discourse technique. Results: Pointed out as central ideas, positive and negative feelings of family members, communication and interaction with the team, ICU as excellence and palliative care as a measure of comfort for the patient and the family. Conclusion: The social representations about the comfort of family members of hospitalized patients in an intensive care unit in palliative care are identified by the family members’ feelings during the visit, communication, and the humanized care applied by nursing professionals in the patients in palliative care.
RESUMO Objetivo: Conhecer as representações sociais de conforto para familiares de pacientes em cuidados paliativos na terapia intensiva. Método: Estudo descritivo, qualitativo, referencial teórico adotado foi Representações Sociais. Participaram 30 familiares de pacientes internados em unidade de terapia intensiva em cuidados paliativos. Os dados foram coletados por meio de entrevistas individuais semiestruturadas, organizados e analisados por meio da técnica do Discurso do Sujeito Coletivo. Resultados: Apontaram como ideias centrais, sentimentos positivos e negativos dos familiares, a comunicação e a interação com a equipe, a UTI como excelência e o cuidado paliativo como medida de conforto para o paciente e a família. Conclusão: As representações sociais sobre o conforto dos familiares de pacientes internados em unidade de terapia intensiva em cuidados paliativos estão identificadas pelos sentimentos dos familiares durante a visita, comunicação, e o cuidado humanizado aplicado pelos profissionais de enfermagem nos pacientes em terapia paliativa.
RESUMEN Objetivo: Conocer las representaciones sociales de confort para familiares de pacientes en cuidados paliativos en cuidados intensivos. Método: Estudio descriptivo, cualitativo, se adoptó el marco teórico de Representaciones Sociales. Participaron 30 familiares de pacientes ingresados en una unidad de cuidados intensivos en cuidados paliativos. Los datos fueron recolectados a través de entrevistas individuales semiestructuradas, organizadas y analizadas utilizando la técnica del Discurso del sujeto colectivo. Resultados: Señalados como ideas centrales, sentimientos positivos y negativos de los miembros de la familia, comunicación e interacción con el equipo, UCI como excelencia y cuidados paliativos como medida de confort para el paciente y la familia. Conclusión: Las representaciones sociales sobre la comodidad de los miembros de la familia de los pacientes hospitalizados en una unidad de cuidados intensivos en cuidados paliativos se identifican por los sentimientos de los miembros de la familia durante la visita, la comunicación y la atención humanizada aplicada por los profesionales de enfermería en los pacientes en cuidados paliativos.
