Les onychomycoses représentent environ 30 % des mycoses superficielles rencontrées en consultation de dermatologie, et près de 50 % des étiologies d’onychopathies. Les champignons en cause sont ...essentiellement les dermatophytes, habituellement rencontrés au niveau des ongles des pieds, suivis par les levures qui prédominent au niveau des mains. Beaucoup plus rarement, des moisissures peuvent être impliquées. Ces dernières ne représenteraient que 2 à 17 % des étiologies d’onychomycoses. Il convient, dans cette dernière catégorie, de distinguer les onychomycoses à pseudodermatophytes dues aux Neoscytalidium (ex-Scytalidium) et à Onychocola canadensis dont l’affinité pour la kératine unguéale est réelle, des onychomycoses causées par d’autres moisissures (Aspergillus, Fusarium, Scopulariopsis, Acremonium…) qui sont davantage des colonisatrices d’ongles pathologiques que de véritables agents pathogènes. Le rôle des laboratoires (mycologie et anatomopathologie) est essentiel, à la fois pour identifier précisément l’espèce en cause et pour mettre en évidence un parasitisme fongique au niveau de l’ongle concerné. Il est en effet primordial, afin d’attribuer un rôle pathogène aux différentes espèces de moisissures, de réaliser avec rigueur et précision des prélèvements à visée mycologique et/ou histologique. Le traitement des onychomycoses à moisissures est difficile, d’autant qu’il n’existe actuellement pas de consensus. Le choix d’une molécule antifongique dépend en effet de l’espèce en cause, du degré d’atteinte unguéale et du terrain du patient concerné. Le plus souvent, une avulsion mécanique ou chimique des zones atteintes suivie d’un antifongique local peut suffire à guérir l’onyxis, mais dans certains cas le recours à un traitement par voie générale doit être envisagé.
Onychomycoses represent about 30% of superficial mycosis that are encountered in Dermatology consults. Fungi such as dermatophytes, which are mainly found on the feet nails, cause nearly 50% of these onychopathies. Yeasts are predominantly present on hands, whereas non-dermatophytic moulds are very seldom involved in both foot and hand nails infections. According to literature, these moulds are responsible for 2 to 17% of onychomycoses. Nevertheless, we have to differentiate between onychomycoses due to pseudodermatophytes such as Neoscytalidium (ex-Scytalidium) and Onychocola canadensis, which present a high affinity for keratin, and onychomycoses due to filamentous fungi such as Aspergillus, Fusarium, Scopulariopsis, Acremonium… These saprophytic moulds are indeed most of the time considered as colonizers rather than real pathogens agents. Mycology and histopathology laboratories play an important role. They allow to identify the species that is involved in nail infection, but also to confirm parasitism by the fungus in the infected nails. Indeed, before attributing any pathogenic role to non-dermatophytic moulds, it is essential to precisely evaluate their pathogenicity through samples and accurate mycological and/or histological analysis. The treatment of onychomycoses due to non-dermatophytic moulds is difficult, as there is today no consensus. The choice of an antifungal agent will first depend on the species that is involved in the infection, but also on the severity of nail lesions and on the patient himself. In most cases, the onychomycosis will be cured with chemical or mechanical removing of the infected tissues, followed by a local antifungal treatment. In some cases, a systemic therapy will be discussed.

Les onychomycoses représentent environ 50 % des causes de pathologie unguéale. Les dermatophytes (notamment Trichophyton rubrum et Trichophyton interdigitale) sont les principaux agents isolés au ...niveau des pieds. Les levures du genre Candida (Candida albicans, Candida parapsilosis,…) prédominent aux mains, associées le plus souvent à un périonyxis. En dehors des classiques dermatophytes et des levures, d’autres champignons sont parfois isolés d’ongles pathologiques. Parmi eux, on trouve surtout des espèces appartenant aux genres Scopulariopsis, Aspergillus et Fusarium, dont le pouvoir pathogène doit être précisé. D’autres moisissures ayant une affinité particulière pour la kératine (pseudodermatophytes) sont considérées comme de vrais pathogènes. Il s’agit d’espèces issues de régions tropicales ou subtropicales comme Neoscytalidium dimidiatum (ex Scytalidium dimidiatum) ou de pays tempérés ou froids comme Onychocola canadensis qui est impliqué dans des onyxis des pieds chez le sujet âgé. Une approche pluridisciplinaire, clinico-biologique, est nécessaire pour confirmer la mycose et identifier l’espèce en cause. Cette revue comparative des différentes méthodes de diagnostic souligne l’intérêt de l’examen histologique de l’ongle, ainsi que celui des techniques de biologie moléculaire, qui s’avèrent très contributives au diagnostic d’onychomycose. L’interprétation des résultats s’efforcera de démontrer que le champignon isolé du prélèvement d’ongles pathologiques est bien à l’origine des lésions.

ans. Nous rapportons deux nouveaux cas de sinusites survenues chez une femme de 57
ans et un homme de 56
ans. Il s’agissait d’un cas de sinusite chronique et d’un cas de sinusite allergique où ce basidiomycète a été identifié après isolement en culture à la fois par son aspect microscopique, macroscopique et par la biologie moléculaire. Dans les deux cas, l’évacuation et le drainage par débridement chirurgical ont été suffisants pour obtenir la guérison. Schizophyllum commune is a basidiomycete fungus of the agaricomycetes class. It is usually found in forest habitats in the form of saprophytic and lignicolous fungus. Its implication in human pathology is rare, only 50 clinical cases among which 18 cases of sinusitis have been described in the last 60 years. We report two new cases of sinusitis. The first was a 57-year-old woman with chronic sinusitis and the second was a 56-year-old man suffering from an allergic sinusitis. This basidiomycete was identified after isolation in culture both by macroscopic and microscopic aspect and molecular biology. In both cases, evacuation and drainage by surgical debridment were sufficient to obtain a cure. The main bioclinical characteristics of

A 36-year-old woman returning from holiday in Portugal presented with bilateral ocular pain and acute visual loss. Eye examination revealed an inflammation in the anterior segment associated with ...corneal keratitis. During her trip, she wore contact lenses and rinsed the storage case using tap water. Corneal scrapings, contact lenses and cleaning solution from the case were inoculated onto agar plates supplemented with heat-inactivated bacteria. Bilateral Acanthamoeba keratitis was diagnosed, as microscopic examination of cultures from the last two samples revealed typical, spherical double-walled cystic structures (Fig. 1).
Since echinocandins are recommended as first line therapy for invasive candidiasis, detection of resistance, mainly due to alteration in FKS protein, is of main interest. EUCAST AFST recommends ...testing both MIC of anidulafungin and micafungin, and breakpoints (BPs) have been proposed to detect echinocandin-resistant isolates. We analyzed MIC distribution for all three available echinocandins of 2,787 clinical yeast isolates corresponding to 5 common and 16 rare yeast species, using the standardized EUCAST method for anidulafungin and modified for caspofungin and micafungin (AM3-MIC). In our database, 64 isolates of common pathogenic species were resistant to anidulafungin, according to the EUCAST BP, and/or to caspofungin, using our previously published threshold (AM3-MIC ≥ 0.5 mg/L). Among these 64 isolates, 50 exhibited 21 different FKS mutations. We analyzed the capacity of caspofungin AM3-MIC and anidulafungin MIC determination in detecting isolates with FKS mutation. They were always identified using caspofungin AM3-MIC and the local threshold while some isolates were misclassified using anidulafungin MIC and EUCAST threshold. However, both methods misclassified four wild-type C. glabrata as resistant. Based on a large data set from a single center, the use of AM3-MIC testing for caspofungin looks promising in identifying non-wild-type C. albicans, C. tropicalis and P. kudiravzevii isolates, but additional multicenter comparison is mandatory to conclude on the possible superiority of AM3-MIC testing compared to the EUCAST method.

Le diagnostic d’anaphylaxie au blé induite par l’effort est difficile, parfois mis en défaut par les investigations complémentaires, y compris lors de test de réintroduction (TPO) car les conditions ...de réalisation ne reproduisent pas toujours la réalité ou les doses de charge sont insuffisantes. Nous rapportons le cas d’un homme de 51 ans aux antécédents d’anaphylaxie au crabe en 2009 (urticaire et douleurs abdominales) sensibilisé aux crustacés, carmin de cochenille et acariens et pour lequel une allergie au gluten avait été suspectée du fait d’urticaire à l’effort.
Le patient présente une sensibilisation douteuse au gluten (Tableau 1). Il est asymptomatique depuis 2014 sous régime sans gluten (quelques écarts mais jamais suivis d’effort). Une épreuve d’effort sur tapis roulant sans gluten est réalisée à J1, puis un TPO au pain (100g farine de blé bio + 60g gluten + levure) à J15, puis un TPO (idem J15) suivi d’un effort (idem J1).
Cette observation montre que l’absence de sensibilisation au blé par des méthodes classiques ne permet pas d’écarter formellement le diagnostic d’anaphylaxie au blé. Le diagnostic sera écarté dans des conditions « extrèmes » avec une forte charge en gluten 1 combinée à un effort maximal avec passage en aérobiose et contrôle Vmax. Compte tenu des conditions de déclenchement (forte dose et effort maximal, absence de signe de gravité), nous avons autorisé la consommation de gluten en dehors de cofacteurs habituels (alcool, effort, AINS). L’hypothèse d’un syndrome pancake (allergie par ingestion d’acariens dans la farine bio, du fait de l’allergie aux crustacés) semble exclue par un examen à la loupe binoculaire ne retrouvant pas d’arthropodes dans la farine et le gluten ingérés.
Scytalidium
dimidiatum, synanamorphe de
Nattrassia
mangiferae, est un champignon filamenteux phytopathogène rencontré à l’état naturel exclusivement dans les régions chaudes du globe. Chez l’homme, ...ce phaeohyphomycète est responsable d’onyxis et d’atteintes cutanées pseudodermatophytiques, mais des atteintes sous-cutanées profondes, ainsi que des formes invasives ont également été décrites chez des patients présentant une immunodépression importante. Nous rapportons ici le cas d’une patiente transplantée rénale, hospitalisée pour altération de l’état général dans un contexte d’immunodépression sévère. Outre de nombreuses autres complications infectieuses opportunistes, la patiente présentait une lésion d’aspect pseudotumoral localisée au niveau de l’avant-bras. La biopsie-exérèse de cette lésion a permis la mise en évidence de filaments mycéliens à l’examen anatomopathologique et l’isolement d’un
Scytalidium
dimidiatum en culture. L’originalité de ce cas de scytalidiose cutanée réside dans le fait que la patiente, originaire de Bulgarie, n’avait séjourné récemment dans aucun pays tropical ou sub-tropical.
Scytalidium
dimidiatum, the synanamorph of
Nattrassia
mangiferae, is a mold plant pathogen that is exclusively found in tropical and subtropical areas. This dematiaceous fungus can cause superficial skin and nail infections that mimic those caused by dermatophytes, but deep subcutaneous infections and disseminated diseases have also been described in immunosuppressed patients. Here, we report the case of a kidney transplant female recipient who was hospitalized for profound physical deterioration in a context of severe immunodepression. In addition to many other opportunistic infections, she presented a pseudotumoral lesion localized to the right forearm. The histological examination of a biopsy specimen revealed septate hyphae and culture allowed the isolation of
Scytalidium
dimidiatum. This case of cutaneous scytalidiosis is particular, as the patient, who lived in Bulgaria, did not recently stay in a tropical or subtropical country.
