The French College of Obstetrics and Gynecology (CNGOF) releases its first global recommendations for clinical practice in contraception, to provide physicians with an updated synthesis of available ...data as a basis for their practice. The French Health Authority (HAS) methodology was used. Twelve practical issues were selected by the organizing committee and the task force members. The available literature was screened until December 2017, and allowed the release of evidence-based, graded recommendations. This synthesis is issued from 12 developed texts, previously reviewed by experts and physicians from public and private practices, with an experience in the contraceptive field. Male and female sterilization, as well as the use of hormonal treatments without contraceptive label were excluded from the field of this analysis. Specific practical recommendations on the management of contraception prescription, patient information including efficacy, risks, and benefits of the different contraception methods, follow up, intrauterine contraception, emergency contraception, local and natural methods, contraception in teenagers and after 40, contraception in vascular high-risk situations, and in case of cancer risk are provided. The short/mid-term future of contraception mostly relies on improving the use of currently available methods. This includes reinforced information for users and increased access to contraception for women, whatever the social and clinical context. That is the goal of these recommendations.

The first results of major randomized trials of HT had alerted to an increase in cardiovascular events and breast cancer with the use of oral HT, generally, tipping the benefit-risk balance of the deleterious side. Complementary analyzes have shown the importance of the window of intervention (less than 10 years after the menopause) and the age of the woman to start the HT. On the contrary, they have shown a significant decrease of the coronary events. For woman suffering from hypertension and important climacteric symptoms, it is important to evaluate the whole cardiovascular risk in order to decide the possibility of prescribing a HT. Thus, the group of experts proposes a prescription assistance algorithm based on the stratification of cardiovascular risk, always favoring, when it is authorized, HT by transdermal route of administration.

Le traitement hormonal de ménopause (THM) peut-il être utilisé chez les femmes hypertendues ? Le groupe d’experts de la Société française d’hypertension a effectué une revue de la littérature récente ...afin de répondre à cette question sur les bases scientifiques les plus récentes. Si l’utilisation d’un THM par voie orale est associée à une discrète augmentation des chiffres de pression artérielle, la voie transdermique semble de meilleure sécurité d’emploi vis-à-vis de ce risque. Les premiers résultats des grands essais randomisés du THM en prévention primaire ont alerté sur une augmentation des évènements cardio-vasculaires et du cancer du sein en cas de prescription de THM par voie orale faisant basculer, en général, la balance bénéfice-risque du côté délétère. Les analyses complémentaires ont montré l’importance de la fenêtre d’intervention (moins de 10 ans après la ménopause) et de l’âge de la femme pour débuter le THM et ont montré au contraire une diminution significative des évènements coronariens. Chez une femme hypertendue, il est donc important d’évaluer l’ensemble du risque cardio-vasculaire afin d’apprécier la possibilité de prescription d’un THM en cas de symptômes climatériques invalidants. Ainsi, le groupe d’experts propose un algorithme d’aide à la prescription basé sur la stratification du risque cardio-vasculaire et en privilégiant toujours lorsqu’il est autorisé le THM par voie transdermique.

Objectif Évaluation des caractéristiques cliniques et biologiques des femmes lors d’un 1er évènement thromboembolique veineux (ETV), et du pourcentage d’ETV évitables. Patientes et méthodes Toutes ...les femmes ayant eu un 1er ETV et consultant en hémostase à l’Hôtel-Dieu entre 2000 et 2009, âgées de moins de 46 ans, ont été incluses dans la cohorte COREVE. Résultats Parmi les 3072 femmes, 30 % ( n = 923) étaient non-utilisatrices, 68 % ( n = 2079) utilisaient une contraception estro-progestative (CHC) et 2 % ( n = 70) une contraception progestative seule (CP). Les utilisatrices de CHC étaient plus jeunes (29,0 ± 7,2 ; moyenne ± SD) que les non-utilisatrices (31,6 ± 7,1) p < 0,0001 ou utilisatrices de CP (33,6 ± 7,6) p < 0,0001. L’indice de masse corporel (IMC) était plus élevé chez les utilisatrices de CP (25,5 ± 5,8) que chez les utilisatrices de CHC (23,0 ± 4,4) p < 0,0001. Une mutation du facteur V Leiden a été retrouvée plus fréquemment dans le groupe CHC que chez les non-utilisatrices (respectivement 17 % et 13 % ; OR = 1,36 1,09–1,70). Les utilisatrices de CHC contenant de l’acétate de cyprotérone étaient significativement plus jeunes ( p < 0,0001), plus minces ( p = 0,0002) et avaient plus d’ETV favorisés par un voyage ( p = 0,002) par rapport aux utilisatrices d’autres CHC. Au total, plus de 12 % des utilisatrices de CHC avaient une prescription inadaptée en raison de facteurs de risque connus tels que le post-partum, l’IMC ≥ 25, l’âge ≥ 40 ans et les antécédents familiaux au 1er degré d’ETV. Conclusion Les caractéristiques des utilisatrices de CHC ayant un 1er ETV diffèrent des non-utilisatrices. Nos résultats soulignent le nombre substantiel d’ETV évitables chez les femmes jeunes.
ObjectivesThe use of anatomic model is particularly suitable for teaching gynecological examination. Although simulation training was proved to be effective in improving medical students’ technical ...skills, its interest in reducing their apprehension about pelvic examination was poorly evaluated. The aim of the present study was to evaluate low-fidelity simulation sessions to reduce the discomfort and apprehension of medical students towards gynecological examination. The secondary objective was to appreciate student satisfaction with these sessions.MethodsDescriptive analysis of simulation-based teaching of breast and pelvic examination on Ilumens platform at the university Paris 5 among 3rd and 4th year medical students. It consisted in a before/after evaluation, using an anonymous questionnaire of self-evaluation and satisfaction at the beginning and at the end of the session.ResultsFour hundred and thirty-two students answered both questionnaires. Seventy-two percent apprehended the pelvic examination initially and 62% were uncomfortable with the idea of performing it. Among the students who had never performed pelvic examination, more than 80% felt less uncomfortable with the idea of performing pelvic examination and were less apprehensive about it at the end of the simulation session.ConclusionThe benefits of low-fidelity simulation in gynecology extend to the well-being of medical students who are less apprehensive about performing gynecological examination.
ObjectifsL’apprentissage de l’examen gynécologique se prête particulièrement bien à un apprentissage sur simulateur. Si la simulation s’est révélée efficace pour améliorer les compétences techniques des étudiants, son intérêt pour diminuer leur appréhension vis-à-vis de cet examen reste peu étudié. L’objectif de cette étude était d’évaluer l’intérêt des séances de simulation basse fidélité pour réduire la gêne et l’appréhension des étudiants en médecine quant à l’examen gynécologique. L’objectif secondaire était d’apprécier la satisfaction des étudiants quant à ces séances.MéthodesAnalyse descriptive de l’enseignement par simulation des gestes médicaux en gynécologie auprès d’étudiants en DFGSM3 et en DFASM2 en formation initiale sur la plateforme Ilumens à l’université Paris 5. Il s’agissait d’une étude avant/après avec un 1er questionnaire anonyme d’autoévaluation et de satisfaction distribué en début de séance, puis un 2e en fin de séance.RésultatQuatre-cent-trente-deux étudiants ont répondu aux questionnaires avant et après la séance de simulation. Soixante-douze pour cent des étudiants appréhendaient l’examen pelvien initialement et 62 % étaient gênés à l’idée de le réaliser. Parmi les étudiants n’ayant jamais examiné de patientes, plus de 80 % se sentaient moins gênés à l’idée de réaliser un examen pelvien et appréhendaient moins le geste à l’issu de la séance de simulation.ConclusionLes bénéfices de la simulation basse fidélité en gynécologie s’étendent au bien-être des étudiants en médecine qui appréhendent moins la réalisation de l’examen gynécologique.
