BACKGROUND Pancreatic cancer is one of the most common cancers in the world and a major cause of cancer mortality. Therefore, it is extremely important to distinguish between malignant and benign ...changes quickly and accurately. This single-center study aimed to assess the discriminatory properties of the color Doppler vascularity index (CDVI) in the diagnosis of focal chronic pancreatitis and malignant pancreatic tumors. MATERIAL AND METHODS Seventy-nine patients (42 men, 37 women; age 62.0±13.5 years; 46 adenocarcinomas; 33 pancreatitis) qualified for this study. During endosonographic examination, pancreatic tumors were assessed in the color Doppler option. The dynamic tissue perfusion measurement was used to calculate tissue flow velocity (TFV), tissue perfusion intensity (TPI), and vascularization as the CDVI. RESULTS TFV, TPI, and CDVI were significantly lower in the group with malignant tumors than in the group with pancreatitis (P<0.001). In the receiver operating characteristic analysis, results of TFV=2.181 cm/s, TPI=0.009 cm/s, and CDVI=0.268 allowed for significant prediction of malignant tumors (P<0.001), with sensitivity of 75.8%, 69.7%, and 72.7% and specificity of 91.3%, 93.5%, and 80.4%, respectively, without significant differences between perfusion parameters and CDVI (P=0.07). CONCLUSIONS The findings from this study showed that color Doppler imaging and the use of the CDVI could provide an adjunctive diagnostic approach to distinguish between pancreatic adenocarcinoma and focal chronic pancreatitis. Owing to the possibility of calculating vascularization by non-Doppler methods, the method may be an easier and more accessible diagnostic option for malignant pancreatic tumors than perfusion assessed in external software.
Differentiation between pancreatic malignant and inflammatory tumors presents an important diagnostic problem. The ability to recognize pancreatic malignant tumors using Doppler evaluation of tissue ...perfusion has been recently demonstrated. The aim of the study was to assess the diagnostic value of Dynamic Tissue Perfusion Measurement (DTPM) in the differentiation between malignant and inflammatory pancreatic tumors. The study included 60 patients (35M, 25F, age 60.9 ± 2.3 years) with a malignant (Group 1, n = 30) or inflammatory (Group 2, n = 30) pancreatic tumor undergoing endoscopic ultrasound with the evaluation of tissue perfusion by Color Doppler and a simultaneous biopsy of lesions for cytological evaluation. In 20 patients the diagnosis was verified in the postoperative histopathological examination. Flow velocity (FV) and percentiles of the distribution of perfusion intensity (PR) evaluated by DTPM were analyzed with regard to receiver-operator-characteristics. FV as well as PR were significantly higher in Group 2 compared to Group 1. A threshold of 2.0 cm/sec for FV identified patients with malignancies with a sensitivity of 83% and specificity of 86%. In multivariable regression analysis, the best PR parameter for differentiating between malignant and inflammatory tumors was 97.5% percentile, whose value of 0.922 allowed for the recognition of pancreatic malignant tumors with a sensitivity of 62% and specificity of 83% (p < 0.001). In conclusion, Color Doppler ultrasound tissue perfusion parameters are a sensitive and specific tool in the differentiation between malignant and inflammatory pancreatic tumors.
Dynamic tissue perfusion measurement (DTPM) and single vessel flow measurement (SVFM) were assessed in differentiating inflammatory and malignant lesions of the pancreas. Sixty-nine patients (age ...62.0 ± 14.7; 33 Female and 36 Men; 40 with malignant and 29 with inflammatory lesions) in whom during the endoscopic ultrasound (EUS) of focal pancreatic lesions it was possible to adequately evaluate the flow in the color Doppler, and then perform a biopsy, were qualified for the study. The assessed DTPM parameters flow velocity (TFV), perfusion intensity (TPI), and resistive index (TRI) as well as the following SVFM parameters: flow velocity (FV), volume flow (VolF), and resistive index (RI) differed significantly between the malignant and inflammatory lesions (
< 0.005). TFV and TPI have slightly better discriminatory properties than the corresponding FV and VolF parameters (
< 0.10). Considering the Doppler parameters usually evaluated in a given method, the TPI = 0.009 cm/s (sensitivity 79%, specificity 92%, AUC 0.899,
< 0.001) was significantly better (
= 0.014) in differentiating between inflammatory and malignant pancreatic lesions in comparison to FV = 2.526 cm/s (sensitivity 79%, specificity 70%, AUC 0.731,
< 0.001). Tissue perfusion has better discriminatory properties in the differentiation of solid pancreatic lesions than the Doppler blood flow examination in the single vessel within the tumor.

Wiadomo, że warunkiem koniecznym ustalenia ostatecznego rozpoznania jest potwierdzenie nieswoistej choroby zapalnej jelit w badaniu endoskopowym i histopatologicznym, niemniej jednak badania laboratoryjne są pomocne we wstępnej diagnostyce i odzwierciedlają zasięg oraz nasilenie procesu zapalnego. Białko C-reaktywne stanowi wykładnik procesu zapalnego, a występująca we wrzodziejącym zapaleniu jelita grubego niedokrwistość jest wynikiem zarówno przewlekłości choroby, jak i niedoboru żelaza wtórnego do utraty krwi. Diagnostyka laboratoryjna ma również zastosowanie w ocenie powikłań pozajelitowych dotyczących wątroby i dróg żółciowych, a także osteoporozy. Nieprawidłowe wyniki prób wątrobowych są częste w chorobach zapalnych jelit i mogą być spowodowane procesem zapalnym typowym dla tych chorób, ale też stosowanym leczeniem farmakologicznym. Chociaż powstają nowe metody wykorzystywane w diagnostyce przewodu pokarmowego, takie jak tomografia komputerowa, rezonans magnetyczny, pozytronowa tomografia emisyjna, to jednak badanie endoskopowe z oceną histopatologiczną pobranych wycinków jest (i jeszcze długo będzie) podstawową metodą diagnostyczną w rozpoznawaniu, monitorowaniu leczenia oraz nadzorze onkologicznym.

The aim of the study is to evaluate the diagnostic properties of ultrasound parameters of organ perfusion in the detection of malignant tumors of the pancreas.
Thirty-six patients with a focal lesion of the pancreas underwent endosonography with color flow imaging and biopsy for histological evaluation. Five patients were excluded because of the absence of the Doppler signal in pancreatic lesion. In the dynamic tissue perfusion measurement (DTPM) means of flow velocity (FV), resistive index, pulsatility index, and perfusion relief intensity (PR) were estimated.
In the group with malignant tumors FV was significantly lower compared with the group with inflammatory changes. In receiver operating characteristic (ROC) analysis FV below the optimal cut-off point of 2.382 cm/s identified patients with malignant lesions with a sensitivity of 92% and specificity of 90%. In the group with malignant tumors significantly lower values of PR in all considered percentiles were observed. Based on the ROC analysis in the group with solid tumors, it was found that PR25 ≤ 0.057 allowed to recognize malignancies with a sensitivity of 100% and specificity of 80%, and in the groups with solid and cystic tumors with a sensitivity of 100% and specificity of 79%.
FV and PR intensity derived from DTPM are reliable markers in recognition of pancreatic malignant masses.
Purpose
Renal cortical perfusion measured in noninvasive, dynamic ultrasonic method is connected with the hemodynamic cardiac properties and renal function. Antihypertensive drugs affect the ...functioning of the heart and kidneys. The aim of the study was to evaluate the effect of a chronic use of antihypertensive drugs on ultrasound parameters of renal cortical perfusion.
Methods
The study included 56 consecutive patients (49 M + 7 F, age 54.0 ± 13.3) with stable chronic kidney disease and hypertension. Color Doppler dynamic tissue perfusion measurement was used to assess renal cortical perfusion.
Results
Patients were treated with a mean of 2.7 ± 1.4 antihypertensive drugs, of which diuretics accounted for 25%, angiotensin-converting enzyme inhibitors (ACE-I) together with angiotensin receptor blockers (ARB) 24%, beta-blockers (BB) 23%, calcium channel blockers 16%, alpha-1 blockers (α1B) 9% and centrally acting drugs 3%. All investigated groups of drugs correlated significantly with parameters of renal perfusion. In multivariable regression analyses adjusted to age, diuretics were connected with the decrease (

Conclusions
The type of antihypertensive therapy had a significant influence on the ultrasound parameters of renal cortical perfusion. Noninvasive, ultrasonic dynamic tissue perfusion measurement method appears to be an adequate tool to assess the impact of drugs on renal cortical perfusion.

Etiopatologia schorzenia nie została dokładnie poznana i najpewniej jest wieloczynnikowa. Do rozważanych czynników patologicznych zali‑ cza się skład flory bakteryjnej jelita, nadwrażliwość trzewną, traumatyczne przeżycia, zaburzenia w zakresie funkcji serotoniny. W celu rozpoznania zespołu jelita drażliwego konieczne jest zastosowanie odpowiednich kryteriów dia‑ gnostycznych – aktualnie obowiązują kryteria rzymskie III. Szczególną uwagę trzeba zwrócić na możliwe występo‑ wanie objawów choroby organicznej lub obciążenie rodzinne rakiem jelita grubego. Pomimo wpływu na komfort życia zespół jelita drażliwego nie wpływa na czas przeżycia i nie wiąże się z poważnymi powikłaniami. Leczenie schorzenia powinno być zindywidualizowane, z uwzględnieniem sytuacji społecznej pacjenta. Należy także ocenić wpływ choroby na jakość życia. Leczenie farmakologiczne warto połączyć z edukacją chorego, a w niektórych przy‑ padkach z psychoterapią. Zalecane leczenie farmakologiczne zespołu jelita drażliwego obejmuje stosowanie 2–3 leków i powinno być nastawione na zniesienie lub złagodzenie najbardziej dokuczliwych objawów. Do stoso‑ wanych leków zaliczają się m.in. leki rozkurczowe, przeciwbiegunkowe, antagoniści receptora 5-HT3, agoniści receptora 5-HT4, antybiotyki, probiotyki, leki przeciwdepresyjne, leki psychotropowe. Obecnie trwają badania kli‑ niczne dotyczące zastosowania nowych substancji w leczeniu zespołu jelita drażliwego.

Ponieważ liczba osób w wieku podeszłym zwiększa się, należy spodziewać się wzrostu liczby pacjentów z tą patologią. W ostatnim czasie zwiększa się również liczba osób w młodym wieku dotkniętych tą cho‑ robą. Etiopatogeneza uchyłkowatości jelita grubego nie jest do końca jasna. Przypuszcza się, że podstawowym czynnikiem odpowiedzialnym za powstanie uchyłku jest dieta uboga we włókna roślinne. Prawdopodobnie jest to związane z zachodnim stylem życia. Chociaż uchyłki często nie dają objawów, ich obecność wiąże się z ryzykiem powikłań, takich jak zapalenie, krwawienie z uchyłka czy zapalenie okrężnicy związane z obecnością uchyłków. Objawowa choroba uchyłkowa może mieć różny obraz kliniczny: może przebiegać łagodnie, skąpoobjawowo, podobnie jak zespół jelita nadwrażliwego, lub mogą towarzyszyć jej ciężkie ataki zapalenia uchyłków powikłanych ropniem lub perforacją. Celem pracy jest omówienie epidemiologii, etiopatogenezy, przebiegu klinicznego choroby uchyłkowej oraz zaleceń dotyczących rozpoznania i leczenia w oparciu o bieżącą literaturę medyczną.
Parenteral nutrition provides all essential nutrients directly into the blood stream without involvement of the gastrointestinal tract if conventional oral or enteral nutrition is not possible. ...The awareness of how important it is to provide adequate nutrition to the ill has accompanied people for a long time, as evidenced by the first attempts to conduct nutrition by bypassing the gastrointestinal tract taken as early as in the seventeenth century. The contemporary methods of nutrition have been established in the twentieth century and are still being improved. Nutrition is carried out using specially prepared formulations containing all necessary nutrients in the most absorbable form. These mixes are adapted to individual requirements, resulting from the underlying disease. They are available in the form of ready-made preparations or separate bottles. A method of nutrition is selected on the basis of its expected duration. Disease states are associated with increased catabolism with greater demand for energy and nutrients. This often leads to the development or exacerbation of malnutrition which affects convalescence after surgery and the general condition of the patient as well as increases the risk of hospital complications. As any medical procedure, parenteral nutrition is associated with a risk of side effects. However, the principles of care of the injection site and preparation of appropriate mixes tailored to patient’s needs help minimise the risk of adverse reactions. It is necessary to detect malnutrition early, estimate risk, create nutrition plans and include nutrition to therapy, which will improve the condition of hospitalised patients, accelerate their recovery, shorten hospital stay and reduce treatment costs.
Renal perfusion, which depends on cardiac function, is a factor conditioning the work of kidneys. The objective of the study was to assess the influence of cardiac function, including left ...ventricular contractility and relaxation, on renal cortical perfusion in patients with hypertension and chronic kidney disease treated pharmacologically. There were 63 patients (7 F and 56 M; aged 56 ± 14) with hypertension and stable chronic kidney disease enrolled into the study. Serum cystatin C, with estimated glomerular filtration rate (eGFR), ambulatory blood pressure monitoring, carotid intima-media thickness (cIMT), echocardiography with speckle tracking imaging and the calculation of global longitudinal strain (GLS), diameter of vena cava inferior (VCI), and an ultrasound dynamic tissue perfusion measurement of the renal cortex were performed. We found that the renal cortical perfusion correlated significantly with age, renal function, cIMT, GLS, left ventricular ejection fraction (LVEF), left ventricular mass index (LVMI), diastolic peak values of early (E) and late (A) mitral inflow velocities ratio (E/A) and E to early diastolic mitral annular tissue velocity (E/E'), but not with VCI, or the right ventricle echocardiographic parameters. In multivariable regression analysis adjusted to age, only eGFR, E/E', and GLS were independently related to renal cortical perfusion (r
= 0.44; p < 0.001). In conclusion, the intensity of left ventricular strain and relaxation independently influence renal cortical perfusion in hypertensive patients with chronic kidney disease. A reduction in left ventricular global longitudinal strain is superior to left ventricular ejection fraction in the prediction of a decline in renal cortical perfusion.
