Zika virus (ZIKV) is a mosquito-borne flavivirus that has newly emerged as a significant global threat, especially to pregnancy. Recent major outbreaks in the Pacific and in Central and South America ...have been associated with an increased incidence of microcephaly and other abnormalities of the central nervous system in neonates. The causal link between ZIKV infection during pregnancy and microcephaly is now strongly supported. Over 2 billion people live in regions conducive to ZIKV transmission, with ~4 million infections in the Americas predicted for 2016. Given the scale of the current pandemic and the serious and long-term consequences of infection during pregnancy, the impact of ZIKV on health services and affected communities could be enormous. This further highlights the need for a rapid global public health and research response to ZIKV to limit and prevent its impact through the development of therapeutics, vaccines, and improved diagnostics. Here we review the epidemiology of ZIKV; the threat to pregnancy; the clinical consequences and broader impact of ZIKV infections; and the virus biology underpinning new interventions, diagnostics, and insights into the mechanisms of disease.
Summary Background With epidemic strains of Clostridium difficile posing a substantial healthcare burden internationally, there is a need for longitudinal evaluation of Clostridium difficile ...infection (CDI) events in Australia. Aim To evaluate time trends and severity of illness for CDI events in Australian healthcare facilities. Methods All CDI events in patients admitted to Victorian public hospitals between 1st October 2010 and 31st December 2014 were reported to the Victorian Healthcare Associated Infection Surveillance System. CDI was defined as the isolation of a toxin-producing C. difficile organism in a diarrhoeal specimen, and classified as community-associated (CA-CDI) or healthcare-associated (HA-CDI). Severe disease was defined as admission to an intensive care unit, requirement for surgery and/or death due to infection. Time trends were examined using a mixed-effects Poisson regression model, and the Walter and Edward test of seasonality was applied to evaluate potential cyclical patterns. Findings In total, 6736 CDI events were reported across 89 healthcare facilities. Of these, 4826 (71.6%) were HA-CDI, corresponding to a rate of 2.49/10,000 occupied bed days (OBDs). The incidence of HA-CDI was highest in the fifth quarter of surveillance (3.6/10,000 OBDs), followed by a reduction. Severe disease was reported in 1.66% of events, with the proportion being significantly higher for CA-CDI compared with HA-CDI (2.21 vs 1.45%, P = 0.03). The highest and lowest incidence of HA-CDI occurred in March and October, respectively. Conclusions A low incidence of HA-CDI was reported in Victoria compared with US/European surveillance reports. Seasonality was evident, together with diminishing HA-CDI rates in 2012–2014. Severe infections were more common in CA-CDI, supporting future enhanced surveillance in community settings.
Abstract
We report Fermi Large Area Telescope (LAT) observations of the radio-loud active galactic nucleus SBS 0846+513 (z = 0.5835), optically classified as a narrow-line Seyfert 1 galaxy, together ...with new and archival radio-to-X-ray data. The source was not active at γ-ray energies during the first two years of Fermi operation. A significant increase in activity was observed during 2010 October-2011 August. In particular, a strong γ-ray flare was observed in 2011 June reaching an isotropic γ-ray luminosity (0.1-300 GeV) of 1.0 × 1048 erg s−1, comparable to that of the brightest flat spectrum radio quasars, and showing spectral evolution in γ rays. An apparent superluminal velocity of (8.2 ± 1.5)c in the jet was inferred from 2011 to 2012 Very Long Baseline Array (VLBA) images, suggesting the presence of a highly relativistic jet.
Both the power released by this object during the flaring activity and the apparent superluminal velocity are strong indications of the presence of a relativistic jet as powerful as those of blazars. In addition, variability and spectral properties in radio and γ-ray bands indicate blazar-like behaviour, suggesting that, except for some distinct optical characteristics, SBS 0846+513 could be considered as a young blazar at the low end of the blazar's black hole mass distribution.
For laying hens, the immediate aftermath and healing period of a keel fracture (KF) is characterized by reduced ability to perform species-specific behavior, access resources, and pain. However, the ...longer-term impacts, once the fracture is completely healed, are less clear. As well as acute pain and behavioral changes, a negative experience can shape future responses to putatively threatening stimuli, raising future fear, and anxiety levels during husbandry-related events. We aimed to determine whether hens that had previously sustained keel bone fractures, but were now outside of the peak age range for new fractures, showed higher fear and anxiety levels compared to intact hens. We also determined if healed keel bone fractures were associated with reduced production, changes in behavior and resource use. One hundred and fifty hens with a palpation score of 1 ( “KF”) and 150 hens with a palpation score of 0 (keel fracture free, “KFF”) were selected from a commercial farm at 63 wk of age and housed in 6 groups (3 × KF and 3 × KFF). We compared production (hen weight and feed consumption, egg quantity, quality and weight, floor eggs, shell thickness, and weight) and home pen behavior (behaviors and transitional movements) in both groups. Finally, we measured the responses of KF (n = 75) and KFF (n = 75) during tonic immobility, novel arena, and novel object tests. KF and KFF hens did not differ in their responses to the tonic immobility, novel arena, and novel object tests, nor were there differences between the 2 groups in home pen behavior and transitional movements. KFF birds were lighter and laid eggs with less eggshell membrane compared KF birds, but no differences were found between KF and KFF in any other production measures. We found no evidence that healed KFs were associated with detrimental welfare effects in laying hens, but further work is required to determine the mechanisms and implications of the lower body weight and egg shell membrane.
ABSTRACT We study the kinematics of ridge lines on the parsec-scale jet of the active galactic nucleus BL Lacertae. We show that the ridge lines display transverse patterns that move superluminally ...downstream, and that the moving patterns are analogous to waves on a whip. Their apparent speeds βapp (units of c) range from 3.9 to 13.5, corresponding to in the galaxy frame. We show that the magnetic field in the jet is well ordered with a strong transverse component, and assume that it is helical and that the transverse patterns are Alfvén waves propagating downstream on the longitudinal component of the magnetic field. The wave-induced transverse speed of the jet is non-relativistic ( ). In 2010 the wave activity subsided and the jet then displayed a mild wiggle that had a complex oscillatory behavior. The Alfvén waves appear to be excited by changes in the position angle of the recollimation shock, in analogy to exciting a wave on a whip by shaking the handle. A simple model of the system with plasma sound speed βs = 0.3 and apparent speed of a slow MHD wave βapp, S = 4 yields Lorentz factor of the beam Γbeam ∼ 4.5, pitch angle of the helix (in the beam frame) ∼ 67°, Alfvén speed βA ∼ 0.64, and magnetosonic Mach number Mms ∼ 4.7. This describes a plasma in which the magnetic field is dominant and in a rather tight helix, and Alfvén waves are responsible for the moving transverse patterns.
The narrow-line Seyfert 1 galaxy SBS 0846+513 was first detected by the Large Area Telescope on board Fermi in 2011 June-July when it underwent a period of flaring activity. Since then, as Fermi ...continues to accumulate data on this source, its flux has been monitored on a daily basis. Two further γ-ray flaring episodes from SBS 0846+513 were observed in 2012 May and August, reaching a daily peak flux integrated above 100 MeV of (50 ± 12) × 10−8 ph cm−2 s−1, and (73 ± 14) × 10−8 ph cm−2 s−1 on May 24 and August 7, respectively. Three outbursts were detected at 15 GHz by the Owens Valley Radio Observatory 40 m telescope in 2012 May, 2012 October and 2013 January, suggesting a complex connection with the γ-ray activity. The most likely scenario suggests that the 2012 May γ-ray flare may not be directly related to the radio activity observed over the same period, while the two γ-ray flaring episodes may be related to the radio activity observed at 15 GHz in 2012 October and 2013 January. The γ-ray flare in 2012 May triggered Swift
observations that confirmed that SBS 0846+513 was also exhibiting high activity in the optical, UV and X-ray bands, thus providing a firm identification between the γ-ray source and the lower energy counterpart. We compared the spectral energy distribution (SED) of the flaring state in 2012 May with that of a quiescent state. The two SEDs, modelled as an external Compton component of seed photons from a dust torus, could be fitted by changing the electron distribution parameters as well as the magnetic field. No significant evidence of thermal emission from the accretion disc has been observed. Interestingly, in the 5 GHz radio luminosity versus synchrotron peak frequency plot SBS 0846+513 seems to lie in the flat spectrum radio quasar part of the so-called 'blazar sequence'.
Abstract
The Galactic Center (GC) region hosts a variety of powerful astronomical sources and rare astrophysical processes that emit a large flux of nonthermal radiation. The inner 375 pc × 600 pc ...region, called the Central Molecular Zone, is home to the supermassive black hole Sagittarius A*, massive cloud complexes, and particle accelerators such as supernova remnants (SNRs). We present the results of our improved analysis of the very-high-energy gamma-ray emission above 2 TeV from the GC using 125 hr of data taken with the Very Energetic Radiation Imaging Telescope Array System imaging-atmospheric Cerenkov telescope between 2010 and 2018. The central source VER J1745–290, consistent with the position of Sagittarius A*, is detected at a significance of 38 standard deviations above the background level (38
σ
), and we report its spectrum and light curve. Its differential spectrum is consistent with a power law with exponential cutoff, with a spectral index of
, a flux normalization at 5.3 TeV of
TeV
−1
cm
−2
s
−1
, and cutoff energy of
TeV. We also present results on the diffuse emission near the GC, obtained by combining data from multiple regions along the GC ridge, which yield a cumulative significance of 9.5
σ
. The diffuse GC ridge spectrum is best fit by a power law with a hard index of 2.19 ± 0.20, showing no evidence of a cutoff up to 40 TeV. This strengthens the evidence for a potential accelerator of PeV cosmic rays being present in the GC. We also provide spectra of the other sources in our field of view with significant detections, composite SNR G0.9+0.1, and HESS J1746–285.
We have conducted a detailed investigation of the broadband spectral properties of the gamma-ray selected blazars of the Fermi LAT Bright AGN Sample (LBAS). By combining our accurately estimated ...Fermi gamma-ray spectra with Swift, radio, infra-red, optical, and other hard X-ray/gamma-ray data, collected within 3 months of the LBAS data taking period, we were able to assemble high-quality and quasi-simultaneous spectral energy distributions (SED) for 48 LBAS blazars. The SED of these gamma-ray sources is similar to that of blazars discovered at other wavelengths, clearly showing, in the usual log nu-log nu F-nu representation, the typical broadband spectral signatures normally attributed to a combination of low-energy synchrotron radiation followed by inverse Compton emission of one or more components. We have used these SED to characterize the peak intensity of both the low-and the high-energy components. The results have been used to derive empirical relationships that estimate the position of the two peaks from the broadband colors (i.e., the radio to optical, alpha(ro), and optical to X-ray, alpha(ox), spectral slopes) and from the gamma-ray spectral index. Our data show that the synchrotron peak frequency (nu(S)(peak)) is positioned between 10(12.5) and 10(14.5) Hz in broad-lined flat spectrum radio quasars (FSRQs) and between 10(13) and 10(17) Hz in featureless BL Lacertae objects. We find that the gamma-ray spectral slope is strongly correlated with the synchrotron peak energy and with the X-ray spectral index, as expected at first order in synchrotron-inverse Compton scenarios. However, simple homogeneous, one-zone, synchrotron self-Compton (SSC) models cannot explain most of our SED, especially in the case of FSRQs and low energy peaked (LBL) BL Lacs. More complex models involving external Compton radiation or multiple SSC components are required to reproduce the overall SED and the observed spectral variability. While more than 50% of known radio bright high energy peaked (HBL) BL Lacs are detected in the LBAS sample, only less than 13% of known bright FSRQs and LBL BL Lacs are included. This suggests that the latter sources, as a class, may be much fainter gamma-ray emitters than LBAS blazars, and could in fact radiate close to the expectations of simple SSC models. We categorized all our sources according to a new physical classification scheme based on the generally accepted paradigm for Active Galactic Nuclei and on the results of this SED study. Since the LAT detector is more sensitive to flat spectrum gamma-ray sources, the correlation between nu(S)(peak) and gamma-ray spectral index strongly favors the detection of high energy peaked blazars, thus explaining the Fermi overabundance of this type of sources compared to radio and EGRET samples. This selection effect is similar to that experienced in the soft X-ray band where HBL BL Lacs are the dominant type of blazars.
We report the discovery by the Large Area Telescope (LAT) onboard the Fermi Gamma-Ray Space Telescope of high-energy. gamma-ray emission from the peculiar quasar PMN J0948+0022 (z = 0.5846). The ...optical spectrum of this object exhibits rather narrow H beta (FWHM(H beta) similar to 1500 km s(-1)), weak forbidden lines, and is therefore classified as a narrow-line type I quasar. This class of objects is thought to have relatively small black hole mass and to accrete at a high Eddington ratio. The radio loudness and variability of the compact radio core indicate the presence of a relativistic jet. Quasi-simultaneous radio/optical/X-ray and gamma-ray observations are presented. Both radio and gamma-ray emissions (observed over five months) are strongly variable. The simultaneous optical and X-ray data from Swift show a blue continuum attributed to the accretion disk and a hard X-ray spectrum attributed to the jet. The resulting broadband spectral energy distribution (SED) and, in particular, the gamma-ray spectrum measured by Fermi are similar to those of more powerful Flat-Spectrum Radio Quasars (FSRQs). A comparison of the radio and gamma-ray characteristics of PMN J0948+0022 with the other blazars detected by LAT shows that this source has a relatively low radio and gamma-ray power with respect to other FSRQs. The physical parameters obtained from modeling the SED also fall at the low power end of the FSRQ parameter region discussed in Celotti & Ghisellini. We suggest that the similarity of the SED of PMN J0948+0022 to that of more massive and more powerful quasars can be understood in a scenario in which the SED properties depend on the Eddington ratio rather than on the absolute power.

Breast development in transwomen is an important issue, affecting general psychological functioning. Current hormonal therapies are imperfect, with 60% of patients requesting mammoplasty.
...Interventions:
Interventions included the following: 1) comparing the effects on transwomen's requests for mammoplasty of estrogen valerate, ethinylestradiol, and conjugated equine estrogen (CEE) hormone treatments; and 2) comparing the effects of GnRH analogs and androgen antagonists.
Objective:
The objective of the study was to identify which hormone regimen is associated with the greatest subsequent request for augmentation mammoplasty.
Design:
The study was a controlled, retrospective case audit.
Setting:
The study was conducted at a single-center National Health Service tertiary care unit.
Patients:
Patients were eligible for breast augmentation after 2 yr of estrogen treatment, were Tanner IV or higher breast development, and reported psychological distress due to small breasts. One hundred sixty-five subjects and 165 age-matched controls were identified.
Outcome Measure:
The outcome measure was a mammoplasty request.
Results:
There were significantly more self-medicating individuals than controls in the mammoplasty group (11.5 vs. 6%, P < 0.05). The type of estrogen use did not affect the outcome. Compared with other antiandrogens, spironolactone use was significantly higher in those requesting mammoplasty (4.8 vs. 1.8%, P = 0.002). Thromboembolism occurred in 1.2% of individuals, more frequently in those treated with CEE than in those treated with either estrogen valerate or ethinylestradiol (4.4 vs. 0.6 vs. 0.7%, P = 0.026). Depression was noted in approximately 30% of individuals.
Conclusions:
Self-medication with estrogen is significantly more likely to result in a later request for mammoplasty than is treatment prescribed by licensed practitioners. Previous spironolactone use is more common in those requesting mammoplasty. CEE treatment is associated with a higher incidence of thromboembolism than treatment with other estrogen types.
