Objective
This study explored, and compared, the attitudes of student groups trained at the University of Portsmouth Dental Academy (UPDA) in 2010/2011 towards dental interprofessional education ...(IPE).
Methods
The study population consisted of fifth‐year student dentists (n = 80) from King's College London Dental Institute, second‐ and third‐year dental hygiene and therapy (n = 38) and first‐year dental nursing (n = 14) students from UPDA. A 19‐item, validated and dentally modified questionnaire, ‘Readiness for Inter‐Professional Learning Scale (RIPLS)’, was administered. RIPLS contains three subscales: teamwork and collaboration, professional identity and roles and responsibilities. Mean (x¯) and standard deviation (SD) of the scores were calculated, following reversal of negative items. All the analyses were carried out using SPSS version 20 and STATA version 11.
Results
An overall response rate of 71% (n = 94) was achieved. In reference to teamwork and collaboration, all groups strongly indicated that IPE can contribute to learning teamwork skills (x¯ = 24.98, SD = 3.5) and improving relationships with team members (x¯ = 12.93, SD = 1.63); however, the scores did not differ between the groups (P = 0.09 and P = 0.16, respectively). Concerning professional identity, student dentists had significantly higher preference for a discipline‐based approach (P = 0.002); were more likely to agree that ‘it is not necessary for undergraduate dental and dental care professional students to learn together’ (P = 0.01); and perceived that ‘clinical problem‐solving skills can only be learnt effectively with other students from their own discipline’ (P = 0.02) than dental hygiene and therapy students. In relation to roles and responsibilities, participants demonstrated a strong sense of their own professional role. Student dentists reported that they had ‘to gain more knowledge and skills’ than dental hygiene and therapy (P = 0.01) and dental nursing (P = 0.01) students. Dental hygiene and therapy students were less likely than student dentists to agree that ‘the role of dental nurses and hygienists was to mainly provide support for dentists’ (P = 0.001).
Conclusion
The findings suggest that IPE was perceived as beneficial in relation to teamwork; however, the study raises issues regarding professional identity and roles. Educators should consider differing perceptions of professional roles and identities when planning and delivering interprofessional programmes.
The dynamic response of clamped circular monolithic and sandwich plates of equal areal mass and thickness has been measured by loading the plates at mid-span with metal foam projectiles. The sandwich ...plates comprise AISI 304 stainless steel face sheets and either AL-6XN stainless steel pyramidal core or AISI 304 stainless steel square-honeycomb lattice core. The resistance to shock loading is quantified by the permanent transverse deflection at mid-span of the plates as a function of projectile momentum. It is found that the sandwich plates have a higher shock resistance than monolithic plates of equal mass, and the square-honeycomb sandwich plates outperform the pyramidal core plates. Three-dimensional finite element simulations of the experiments are in good agreement with the experimental measurements. The finite element calculations indicate that the ratio of loading time to structural response time is approximately 0.5. Consequently, the tests do not lie in the impulsive regime, and projectile momentum alone is insufficient to quantify the level of loading.
The Small Animal Veterinary Surveillance Network (SAVSNET) has developed mathematical models to analyse the veterinary practice and diagnostic laboratory data to detect genuine outbreaks of canine ...disease in the United Kingdom. There are, however, no validated methods available to establish the clinical relevance of these genuine statistical outbreaks before their formal investigation is conducted. This study aimed to gain an actionable understanding of a veterinary practitioner's preferences regarding which outbreak scenarios have a substantial impact on veterinary practice for six priority canine diseases in the United Kingdom.
An intensity sampling approach was followed to recruit veterinary practitioners according to their years of experience and the size of their practice. In-depth semi-structured and structured interviews were conducted to describe an outbreak notification and outbreak response thresholds for six canine endemic diseases, exotic diseases, and syndromes. These thresholds reflected participants' preferred balance between the levels of excess case incidence and predictive certainty of the detection system. Interviews were transcribed, and a thematic analysis was performed using NVivo 12.
Seven interviews were completed. The findings indicate higher preferred levels of predictive certainty for endemic diseases than for exotic diseases, ranging from 95 to 99% and 80 to 90%, respectively. The levels of excess case incidence were considered clinically relevant at values representing an increase of two to four times in the normal case incidence expectancy for endemic agents, such as parvovirus, and where they indicated a single case in the practice's catchment area for exotic diseases such as leishmaniosis and babesiosis.
This study's innovative methodology uses veterinary practitioners' opinions to inform the selection of a notification threshold value in real-world applications of stochastic canine outbreak detection models. The clinically relevant thresholds derived from participants' needs will be used by SAVSNET to inform its outbreak detection system and to improve its response to canine disease outbreaks in the United Kingdom.
Heat-related illness (HRI) can affect all companion animals and is likely to become more common as global temperatures rise. The misconception that HRI is primarily a result of dogs being trapped in ...hot cars, highlights a lack of awareness of HRI risk factors within the UK companion animal population.
This project aimed to review all species of small animal presentations of HRI to UK veterinary practices participating in the Small Animal Veterinary Surveillance Network (SAVSNET), describe the inciting triggers and seasonality of HRI events, and review the clinical grade of canine patients presenting with HRI.
Electronic consultation records were submitted by volunteer veterinary practices across the UK to SAVSNET. Cases were defined as animals presented for consultation with strong evidence of current, or recent heat induced illness during the study period (2013-2018).
The HRI cases included 146 dogs, 16 cats, 8 guinea pigs, 3 rabbits and 1 ferret. Of the 118 HRI cases with a recorded trigger, exercise was the primary trigger for dogs presenting (73.5%); 7 (6.9%) canine HRI events followed vehicular confinement. Environmental HRI was recorded as a trigger for the remaining dogs (19.6%), and for all cats, guinea pigs, rabbits and the ferret. Brachycephalic breeds comprised 21.2% of canine HRI cases, and all rabbits were brachycephalic breeds. Dogs with HRI were presented between April and October, with 42.5% during July, typically the UK's hottest month of the year. Cats with HRI were presented between May and September, with 75.0% during June and July. The smaller companion species-ferrets, rabbits and guinea pigs-were presented during the UK's summer months June to August.
This study highlights the risk of HRI to all pet animals during the UK's warmer summer months (June to August). The findings support previous claims that exercise is the most common trigger of HRI in dogs, while environmental HRI (a hot ambient temperature) accounted for all HRI events in cats, rabbits, guinea pigs, and ferrets. Both brachycephalic dogs and rabbits were overrepresented, adding further evidence that owners of these animals should be particularly vigilant for HRI during hot weather.
Pharmaceutical agents (PAs) are commonly prescribed in companion animal practice in the United Kingdom. However, little is known about PA prescription on a population-level, particularly with respect ...to PAs authorised for human use alone prescribed via the veterinary cascade; this raises important questions regarding the efficacy and safety of PAs prescribed to companion animals. This study explored new approaches for describing PA prescription, diversity and co-prescription in dogs, cats and rabbits utilising electronic health records (EHRs) from a sentinel network of 457 companion animal-treating veterinary sites throughout the UK over a 2-year period (2014–2016).
A novel text mining-based identification and classification methodology was utilised to semi-automatically map practitioner-defined product descriptions recorded in 918,333 EHRs from 413,870 dogs encompassing 1,242,270 prescriptions; 352,730 EHRs from 200,541 cats encompassing 491,554 prescriptions, and 22,526 EHRS from 13,398 rabbits encompassing 18,490 prescriptions respectively. PA prescription as a percentage of booked consultations was 65.4% (95% confidence interval, CI, 64.6–66.3) in dogs; in cats it was 69.1% (95% CI, 67.9–70.2) and in rabbits, 56.3% (95% CI, 54.7–57.8). Vaccines were the most commonly prescribed PAs in all three species, with antibiotics, antimycotics, and parasiticides also commonly prescribed. PA prescription utilising products authorised for human use only (hence, ‘human-authorised’) comprised 5.1% (95% CI, 4.7–5.5) of total canine prescription events; in cats it was 2.8% (95% CI, 2.6–3.0), and in rabbits, 7.8% (95% CI, 6.5–9.0). The most commonly prescribed human-authorised PA in dogs was metronidazole (antibiotic); in cats and rabbits it was ranitidine (H2 histamine receptor antagonist). Using a new approach utilising the Simpson’s Diversity Index (an ecological measure of relative animal, plant etc. species abundance), we identified differences in prescription based on presenting complaint and species, with rabbits generally exposed to a less diverse range of PAs than dogs or cats, potentially reflecting the paucity of authorised PAs for use in rabbits. Finally, through a novel application of network analysis, we demonstrated the existence of three major co-prescription groups (preventive health; treatment of disease, and euthanasia); a trend commonly observed in practice.
This study represents the first time PA prescription has been described across all pharmaceutical families in a large population of companion animals, encompassing PAs authorised for both veterinary and human-only use. These data form a baseline against which future studies could be compared, and provides some useful tools for understanding PA comparative efficacy and risks when prescribed in the varied setting of clinical practice.
Feline coronavirus infection is ubiquitous in domestic cats, and is particularly common where conditions are crowded. While most FCoV-infected cats are healthy or display only a mild enteritis, some ...go on to develop feline infectious peritonitis, a disease that is especially common in young cats and multi-cat environments. Up to 12% of FCoV-infected cats may succumb to FIP, with stress predisposing to the development of disease.
The ‘wet’ or effusive form, characterised by polyserositis (abdominal and/or thoracic effusion) and vasculitis, and the ‘dry’ or non-effusive form (pyogranulomatous lesions in organs) reflect clinical extremes of a continuum. The clinical picture of FIP is highly variable, depending on the distribution of the vasculitis and pyogranulomatous lesions. Fever refractory to antibiotics, lethargy, anorexia and weight loss are common non-specific signs. Ascites is the most obvious manifestation of the effusive form.
The aetiological diagnosis of FIP ante-mortem may be difficult, if not impossible. The background of the cat, its history, the clinical signs, laboratory changes, antibody titres and effusion analysis should all be used to help in decision-making about further diagnostic procedures. At the time of writing, there is no non-invasive confirmatory test available for cats without effusion.
In most cases FIP is fatal. Supportive treatment is aimed at suppressing the inflammatory and detrimental immune response. However, there are no controlled studies to prove any beneficial effect of corticosteroids.
At present, only one (intranasal) FIP vaccine is available, which is considered as being non-core. Kittens may profit from vaccination when they have not been exposed to FCoV (eg, in an early-weaning programme), particularly if they enter a FCoV-endemic environment.
A multi-step Coulomb excitation measurement with the GRETINA and CHICO2 detector arrays was carried out with a 430-MeV beam of the neutron-rich $^{110}$Ru (t$_{1/2}$= 12 s) isotope produced at the ...CARIBU facility. This represents the first successful measurement following the post-acceleration of an unstable isotope of a refractory element. The reduced transition probabilities obtained for levels near the ground state provide strong evidence for a triaxial shape; a conclusion confirmed by comparisons with the results of beyond-mean-field and triaxial rotor model calculations.
Squirrelpox virus (SQPV) shows little evidence for morbidity or mortality in North American grey squirrels (Sciurus carolinensis), in which the virus is endemic. However, more recently the virus has ...emerged to cause epidemics with high mortality in Eurasian red squirrels (S. vulgaris) in Great Britain, which are now threatened. Here we report the genome sequence of SQPV. Comparison with other Poxviridae revealed a core set of poxvirus genes, the phylogeny of which showed SQPV to be in a new Chordopoxvirus subfamily between the Molluscipoxviruses and Parapoxviruses. A number of SQPV genes were related to virulence, including three major histocomaptibility class I homologs, and one CD47 homolog. In addition, a novel potential virulence factor showing homology to mammalian oligoadenylate synthetase (OAS) was identified. This family of proteins normally causes activation of an endoribonuclease (RNaseL) within infected cells. The putative function of this novel SQPV protein was predicted in silico.
