The objective of this study is to frame mammogram breast detection model using the optimized hybrid classifier. Image pre-processing, tumor segmentation, feature extraction, and detection are the ...functional phases of the proposed breast cancer detection. A median filter eliminates the noise of the input mammogram. Further, the optimized region growing segmentation is carried out for segmenting the tumor from the image and the optimized region growing depends on a hybrid meta-heuristic algorithm termed as firefly updated chicken based CSO (FC-CSO). To the next of tumor segmentation, feature extraction is done, which intends to extract the features like grey level co-occurrence matrix (GLCM), and gray level run-length matrix (GRLM). The two deep learning architectures termed as convolutional neural network (CNN), and recurrent neural network (RNN). Moreover, both GLCM and GLRM are considered as input to RNN, and the tumor segmented binary image is considered as input to CNN. The result of this study shows that the AND operation of two classifier output will tend to yield the overall diagnostic accuracy, which outperforms the conventional models.

Biosensors provide an automated technology by presenting superfast, nondestructive and cost-effective detection in food analysis. SPR biosensor is an optical biosensor known for its versatility and has wider applications in food testing and analysis. It has an optical system for excitation and interrogation of surface plasmons, and a biomolecular recognition element to detect and seize the target analyte present in a sample. The optical signal detects the binding analyte, on the recognition element, which results in a change in refractive index at the surface and modifies the surface plasmons' propagation constant. SPR aids in label-free detection of various components such as adulterants, antibiotics, biomolecules, genetically modified foods, pesticides, insecticides, herbicides, microorganisms and microbial toxins in food and assures safety. The distinct advancements of SPR in food analysis have been found and discussed. The review also provides knowledge on the advantages and the key challenges encountered by SPR.
The objective of this study is to develop an automated clinically relevant diagnostic framework for a group of cardiovascular diseases, namely Valvular Heart Diseases (VHDs). Though for clinical ...screening, manual auscultation using a stethoscope is the current standard, it is losing its significance due to the demand of years of skill and experience of the clinician. While the physiological location for capturing heart sound signal, called phonocardiogram (PCG), is standardized, the measurement and classification of PCG into different VHDs need automation to redeem its importance. Recent research of the applicability of machine learning (ML) and deep learning (DL) in PCG based classification of VHDs produced high accuracy, but often loses the clinical explanation behind them, and hence often not endorsed by the healthcare fraternity. The proposed study attempts to bridge this gap with two types of clinically explainable automated frameworks: (a) one utilizes clinically relevant PCG features in statistical rule-based models (RBM) as well as ML-based Decision-Tree (DT) frameworks; (b) the other uses time-frequency representation (TFR) with 2D CNN and signal of PCG cycle with 1D CNN, reveals similar clinically explainable intelligence. As a case study, Mitral Valve Prolapse (MVP) - a specific VHD categorized into three severity subclasses of early systolic click (ESC), ESC with murmur (ESCM), and midsystolic click with murmur (MSCM). The signature events (S1, S2, SC, and murmur) in PCG are segmented using a novel Teager-Kaiser energy operator and continuous wavelet transform (TKEO-CWT) algorithm. Clinically relevant intra- and inter-event features like duration, intensity and pitch (dominant frequency) in RBM and DT produced 100% and 98.6% accuracies respectively. Interestingly, gradient class activation (Grad-CAM) of 1D and 2D convolution neural network (CNN) with PCG signal and scalogram as input also converges to the same clinical explanations. Thus the proposed clinically explainable frameworks demonstrate excellent potential in measurements and classifications in future clinical diagnostic products.

Background
Data are insufficient to compare transabdominal preperitoneal repair (TAPP) and total extraperitoneal (TEP) techniques of laparoscopic inguinal hernia repair. There is very scant data ...comparing the two techniques in terms of long-term outcomes, which include chronic groin pain, quality of life, and time to return to normal activity. This prospective, randomized, controlled trial compared TEP versus TAPP techniques of laparoscopic inguinal hernia repair in terms of these long-term outcomes.
Methods
This study was conducted from May 1, 2007 to March 30, 2012. Patients with uncomplicated groin hernia were randomized to transabdominal preperitoneal (TAPP) or totally extraperitoneal (TEP) techniques. A total of 160 patients were randomized to group I (TEP) and 154 patients to group II (TAPP). Pain was assessed with Visual Analogue Scale (VAS) preoperatively and postoperatively at 24 h, 1 week, 6 weeks, 3, 6, and 12 months, and yearly thereafter. Quality of life was assessed with Short Form–36 version 2 (SF 36v2) preoperatively and postoperatively at 3 months follow-up.
Results
Demographic, clinical profiles, intraoperative, and early postoperative parameters were well matched. There was significantly higher acute pain following TAPP repair; however, the chronic groin pain was comparable in both TEP and TAPP. Preoperative pain and immediate postoperative pain had significant correlation with chronic groin pain. Significant improvement from preoperative to postoperative quality of life was seen in both TEP and TAPP repairs, but there was no difference between TEP and TAPP in postoperative period. Time to return to normal activity also was similar between the two groups.
Conclusions
The TEP and TAPP techniques of laparoscopic repair of inguinal hernia have comparable long-term outcomes in terms of incidence of chronic groin pain, quality of life, and resumption of normal activities. Chronic groin pain had a significant correlation with preoperative pain and early postoperative pain. However, TAPP was associated with significantly higher incidence of early postoperative pain, longer operative time, and cord edema, whereas TEP was associated with a significant higher incidence of seroma formation. The cost was comparable between the two.
