Hypercalcemia demands special importance among the paraneoplastic endocrinopathology because of its frequency. The chronically developing hypercalcemia mostly disappears after successful treatment of ...tumor disease and it can be kept under control. But the acute hypercalcemic crisis requires a quick and effective therapy to normalize the serum-calcium-level because of threatening life. The procedure of treatment is presented by means of a case-report of a young patient with advanced ovarian carcinoma.

The age distribution reveals a frequency peak between the ages of 35 and 38 years. The striving for a 2-children family is evident. Operating complications occurred in 1.1 per cent of the cases only, 0.4 per cent were perforationes and 0.7 per cent were lacerations of the cervix. This is due to the general application of the vacuum aspiration and the preferred performance of the operations up to the 10th gestational week. The great significance of the application of effective contraceptives is emphasized, since 48 per cent of the women had renounced contraceptives without any reason. Over 40 per cent of the women had employed contraceptives previously, but later on they had abandoned such means, 20 per cent of these women without a special cause. The physician's responsibility for the education of the women is stressed.
Now as ever the carrying out of abortions demands from all clinics concerned measures reducing the expenditure of time for every patient with the medical treatment remaining consistently good. ...Besides this, certain problems - such as complications and social gynaecological questions - must be registered in a better way than it was till now in order to be able to answer scientific questions on this basis. Therefore, a new printed form for the clinical history--evolved particularly for women wishing an abortion - was introduced into the womens clinic of the Medical Academy. It simplifies documentation which the information being sufficient. By registering them in a computer there are favourable conditions for ascertain the findings and informing central institutions.
From 1976 to 1982 we practised 247 transvaginal punctures with peritoneal lavage with the aim of cytological follow-up of ovarian malignomas. Those who were submitted to this treatment were 73 women ...with ovarian carcinoma of stage I to III, in whom after operation, antineoplastic chemotherapy and possibly radiotherapy no tumor was no longer detectable clinically. In case of positive cytology puncture was repeated in intervals of three months, in case of negative findings of 6 to 8 months. 31.6 per cent of all puncture smears were cytologically positive and 53.8 per cent certainly negative. In 14.6 per cent of all cases no well defined statement could be allotted. We can recommend peritoneal lavage for monitoring of efficiency of therapy of ovarian carcinomas on account of its high evidence and trifling annoyance.
The growing importance of medical malignancy treatment leads to a quick increase of the number of cases which need treatment by cytotoxic drugs. This necessitates comprehensive follow-up care ...programs coping both with medical and individual requirements of patients. Long-term-chemotherapy requires intensive control and well-organized oncological, psychical and social care. These functions may be realized by oncological centres only by way of effective interdisciplinary co-operation incorporating regional health-service facilities. The model of care, elaborated in 1974 in the Gynecological Clinic of the Medical Academy Dresden, represents a close connection between in-patient intensive diagnostics and therapy and specialized predominantly out-patient long-term-therapy. The program of follow-up care introduced takes into account the efforts for greater therapeutical security, improvement of the quality of life and social rehabilitation.
