Neuroimaging measures and chemical biomarkers may be important indices of clinical progression in normal aging and mild cognitive impairment (MCI) and need to be evaluated longitudinally.
To ...characterize cross-sectionally and longitudinally clinical measures in normal controls, subjects with MCI, and subjects with mild Alzheimer disease (AD) to enable the assessment of the utility of neuroimaging and chemical biomarker measures.
A total of 819 subjects (229 cognitively normal, 398 with MCI, and 192 with AD) were enrolled at baseline and followed for 12 months using standard cognitive and functional measures typical of clinical trials.
The subjects with MCI were more memory impaired than the cognitively normal subjects but not as impaired as the subjects with AD. Nonmemory cognitive measures were only minimally impaired in the subjects with MCI. The subjects with MCI progressed to dementia in 12 months at a rate of 16.5% per year. Approximately 50% of the subjects with MCI were on antidementia therapies. There was minimal movement on the Alzheimer's Disease Assessment Scale-Cognitive Subscale for the normal control subjects, slight movement for the subjects with MCI of 1.1, and a modest change for the subjects with AD of 4.3. Baseline CSF measures of Abeta-42 separated the 3 groups as expected and successfully predicted the 12-month change in cognitive measures.
The Alzheimer's Disease Neuroimaging Initiative has successfully recruited cohorts of cognitively normal subjects, subjects with mild cognitive impairment (MCI), and subjects with Alzheimer disease with anticipated baseline characteristics. The 12-month progression rate of MCI was as predicted, and the CSF measures heralded progression of clinical measures over 12 months.

A systematic literature review was conducted to identify published studies on the prevalence of diabetes, impaired glucose tolerance and hyperglycaemia in pregnancy in the period from 1990 to 2016. The highest quality studies on diabetes prevalence were selected for each country. A logistic regression model was used to generate age-specific prevalence estimates or each country. Estimates for countries without data were extrapolated from similar countries.
It was estimated that in 2017 there are 451 million (age 18–99 years) people with diabetes worldwide. These figures were expected to increase to 693 million) by 2045. It was estimated that almost half of all people (49.7%) living with diabetes are undiagnosed. Moreover, there was an estimated 374 million people with impaired glucose tolerance (IGT) and it was projected that almost 21.3 million live births to women were affected by some form of hyperglycaemia in pregnancy. In 2017, approximately 5 million deaths worldwide were attributable to diabetes in the 20–99 years age range. The global healthcare expenditure on people with diabetes was estimated to be USD 850 billion in 2017.
The new estimates of diabetes prevalence, deaths attributable to diabetes and healthcare expenditure due to diabetes present a large social, financial and health system burden across the world.
Abstract
X-ray spectra of quiescent low-mass X-ray binaries containing neutron stars can be fit with atmosphere models to constrain the mass and the radius. Mass-radius constraints can be used to ...place limits on the equation of state of dense matter. We perform fits to the X-ray spectrum of a quiescent neutron star in the globular cluster M13, utilizing data from ROSAT, Chandra, and XMM–Newton, and constrain the mass–radius relation. Assuming an atmosphere composed of hydrogen and a 1.4 M⊙ neutron star, we find the radius to be $R_{\rm NS}=12.2^{+1.5}_{-1.1}$ km, a significant improvement in precision over previous measurements. Incorporating an uncertainty on the distance to M13 relaxes the radius constraints slightly and we find $R_{\rm NS}=12.3^{+1.9}_{-1.7}$ km (for a 1.4M⊙ neutron star with a hydrogen atmosphere), which is still an improvement in precision over previous measurements, some of which do not consider distance uncertainty. We also discuss how the composition of the atmosphere affects the derived radius, finding that a helium atmosphere implies a significantly larger radius.
Most cancer-related deaths are a result of metastasis, and thus the importance of this process as a target of therapy cannot be understated. By asking 'how can we effectively treat cancer?', we do ...not capture the complexity of a disease encompassing >200 different cancer types - many consisting of multiple subtypes - with considerable intratumoural heterogeneity, which can result in variable responses to a specific therapy. Moreover, we have much less information on the pathophysiological characteristics of metastases than is available for the primary tumour. Most disseminated tumour cells that arrive in distant tissues, surrounded by unfamiliar cells and a foreign microenvironment, are likely to die; however, those that survive can generate metastatic tumours with a markedly different biology from that of the primary tumour. To treat metastasis effectively, we must inhibit fundamental metastatic processes and develop specific preclinical and clinical strategies that do not rely on primary tumour responses. To address this crucial issue, Cancer Research UK and Cancer Therapeutics CRC Australia formed a Metastasis Working Group with representatives from not-for-profit, academic, government, industry and regulatory bodies in order to develop recommendations on how to tackle the challenges associated with treating (micro)metastatic disease. Herein, we describe the challenges identified as well as the proposed approaches for discovering and developing anticancer agents designed specifically to prevent or delay the metastatic outgrowth of cancer.
ABSTRACT
MAXI J1820+070 (optical counterpart ASASSN-18ey) is a black hole candidate discovered through its recent very bright outburst. The low extinction column and long duration at high flux allow ...detailed measurements of the accretion process to be made. In this work, we compare the evolution of X-ray spectral and timing properties through the initial hard state of the outburst. We show that the inner accretion disc, as measured by relativistic reflection, remains steady throughout this period of the outburst. Nevertheless, subtle spectral variability is observed, which is well explained by a change in coronal geometry. However, characteristic features of the temporal variability – low-frequency roll-over and quasi-periodic oscillation frequency – increase drastically in frequency, as the outburst proceeds. This suggests that the variability time-scales are governed by coronal conditions rather than solely by the inner disc radius. We also find a strong correlation between X-ray luminosity and coronal temperature. This can be explained by electron pair production with a changing effective radius and a non-thermal electron fraction of $\sim 20$ per cent.
BACKGROUND An ectopic pregnancy is a pregnancy which occurs outside of the uterine cavity, and over 98% implant in the Fallopian tube. Tubal ectopic pregnancy remains the most common cause of ...maternal mortality in the first trimester of pregnancy. The epidemiological risk factors for tubal ectopic pregnancy are well established and include: tubal damage as a result of surgery or infection (particularly Chlamydia trachomatis), smoking and in vitro fertilization. This review appraises the data to date researching the aetiology of tubal ectopic pregnancy. METHODS Scientific literature was searched for studies investigating the underlying aetiology of tubal ectopic pregnancy. RESULTS Existing data addressing the underlying cause of tubal ectopic pregnancy are mostly descriptive. There are currently few good animal models of tubal ectopic pregnancy. There are limited data explaining the link between risk factors and tubal implantation. CONCLUSIONS Current evidence supports the hypothesis that tubal ectopic pregnancy is caused by a combination of retention of the embryo within the Fallopian tube due to impaired embryo-tubal transport and alterations in the tubal environment allowing early implantation to occur. Future studies are needed that address the functional consequences of infection and smoking on Fallopian tube physiology. A greater understanding of the aetiology of tubal ectopic pregnancy is critical for the development of improved preventative measures, the advancement of diagnostic screening methods and the development of novel treatments.
The scientific community, forest managers, environmental organizations, carbon-offset trading systems and policy-makers require tools to account for forest carbon stocks and carbon stock changes. In ...this paper we describe updates to the Carbon Budget Model of the Canadian Forest Sector (CBM-CFS3) implemented over the past years. This model of carbon-dynamics implements a Tier 3 approach of the Intergovernmental Panel on Climate Change (IPCC) Good Practice Guidance for reporting on carbon stocks and carbon stock changes resulting from Land Use, Land-use Change and Forestry (LULUCF). The CBM-CFS3 is a generic modelling framework that can be applied at the stand, landscape and national levels. The model provides a spatially referenced, hierarchical system for integrating datasets originating from different forest inventory and monitoring programs and includes a structure that allows for tracking of land areas by different land-use and land-use change classes. Ecosystem pools in CBM-CFS3 can be easily mapped to IPCC-defined pools and validated against field measurements. The model uses sophisticated algorithms for converting volume to biomass and explicitly simulates individual annual disturbance events (natural and anthropogenic). Several important scientific updates have been made to improve the representation of ecosystem structure and processes from previous versions of CBM-CFS. These include: (1) an expanded representation of dead organic matter and soil carbon, particularly standing dead trees, and a new algorithm for initializing these pools prior to simulation, (2) a change in the input data requirement for simulating growth from biomass to readily available merchantable volume curves, and new algorithms for converting volume to biomass, (3) improved prediction of belowground biomass, and (4) improved parameters for soil organic matter decay, fire, insect disturbances, and forest management. In addition, an operational-scale version of CBM-CFS3 is freely available and includes tools to import data in standard formats, including the output of several timber supply models that are commonly used in Canada. Although developed for Canadian forests, the flexible nature of the model has enabled it to be adapted for use in several other countries.
We present results from the spectral fitting of the candidate black hole X-ray binary Swift J1753.5-0127 in an accretion state previously unseen in this source. We fit the 0.7–78 keV spectrum with a ...number of models, however the preferred model is one of a multitemperature disc with an inner disc temperature kT
in = 0.252 ± 0.003 keV scattered into a steep power-law with photon index
$\Gamma =6.39^{+0.08}_{-0.02}$
and an additional hard power-law tail (Γ = 1.79 ± 0.02). We report on the emergence of a strong disc-dominated component in the X-ray spectrum and we conclude that the source has entered the soft state for the first time in its ∼10 yr prolonged outburst. Using reasonable estimates for the distance to the source (3 kpc) and black hole mass (5 M⊙), we find the unabsorbed luminosity (0.1–100 keV) to be ≈0.60 per cent of the Eddington luminosity, making this one of the lowest luminosity soft states recorded in X-ray binaries. We also find that the accretion disc extended towards the compact object during its transition from hard to soft, with the inner radius estimated to be
