La bronchopneumopathie chronique obstructive (BPCO) est une maladie systémique aux multiples atteintes, émaillée d’exacerbations. L’inflammation, l’hypoxémie, les troubles de la coagulation et les ...perturbations des interactions coeur–poumon expliquent en partie les atteintes non pulmonaires observées notamment lors d’exacerbations aiguës. Les événements cardiovasculaires sont la première cause de mortalité des patients BPCO, et leur recrudescence est observée pendant plusieurs semaines après une exacerbation. Aux urgences, la prise en charge des patients en exacerbation de BPCO repose donc en plus du support ventilatoire sur une bonne évaluation des potentielles pathologies associées telles que l’insuffisance cardiaque, l’ischémie myocardique, l’insuffisance rénale ou encore la maladie thromboembolique. Cette évaluation globale permet d’adapter les thérapeutiques parfois délétères sur la fonction cardiaque ou l’équilibre acide−base et de prendre en compte le risque extrapulmonaire dans le choix du parcours de soins du patient. Dans cette mise au point, nous abordons quelques explications physiologiques des multiples perturbations observées au cours de l’exacerbation de BPCO, et proposons une vision globale de l’évaluation de ces patients admis pour une dyspnée ou un autre motif pouvant être en rapport avec la BPCO.
Chronic obstructive pulmonary disease (COPD) is a systemic disease with multiple comorbidities and organ disorders, punctuated by exacerbations. Inflammation, hypoxemia, coagulation disorders and disturbances of the heart–lung interactions explain in part the non-pulmonary disorders observed especially during acute exacerbations. Cardiovascular events are the leading cause of death in COPD patients, and their recrudescence is observed for several weeks after an exacerbation. The management of these patients in emergency departments consists therefore, in addition to the ventilatory support, in a good evaluation of concomitant heart failure, myocardial ischemic signs, renal failure, or thromboembolic disease. This global evaluation makes it possible to adapt the sometimes deleterious therapies on the cardiac function or the acid–base balance, and to take into account the extra-pulmonary risk in the choice of the patient’s care pathway. In this review, we discuss physiological explanations of the multiple disturbances observed during the exacerbation of COPD, and provide a global vision of what should be included in any evaluation of these patients admitted for dyspnea or any other reason that may be related to COPD.
L’oligoanalgésie chez le patient traumatisé aux urgences est une situation fréquente du fait de la complexité de la prise en charge de la douleur. L’une des solutions les mieux décrites à ce problème ...est l’utilisation d’analgésiques dès l’admission du patient. Ce type de protocole est à privilégier notamment en cas de filière de prise en charge rapide au sein des urgences. Le méthoxyflurane est un éther halogéné volatil utilisé en médecine. Son inhalation produit une analgésie supérieure au placebo. Du fait de sa rapidité d’action, de sa facilité d’emploi et de ses propriétés antalgiques, le méthoxyflurane doit faire partie de l’arsenal des thérapeutiques antalgiques aux urgences. Aux urgences du CHU de Grenoble-Alpes, nous privilégions l’utilisation du méthoxyflurane au sein d’un protocole d’analgésie multimodale du patient adulte traumatisé. Dans cette indication, le méthoxyflurane permet d’amorcer l’analgésie et de faire le pont jusqu’à ce que les autres thérapeutiques soient efficaces.
Oligoanalgesia in trauma patients is a common situation in emergency department because of the complexity of pain management. One of the best-described solutions to this problem is the use of nurse-initiated protocol upon admission. This type of protocol is to be preferred especially in the case of fast track in the emergency department. Methoxyflurane is volatile halogenated ether used in medicine. Inhalation of this gas produces analgesia superior to placebo without serious adverse effects. Due to its rapidity of action, its ease of use and its analgesic properties, methoxyflurane must be a part of the arsenal of antalgic therapies in the Emergency Department. At Grenoble Alpes Emergency Department, we favor the use of methoxyflurane in a multimodal analgesia protocol of trauma in adult patients. In this indication, methoxyflurane is used to initiate analgesia and bridge until other analgesic therapies are effective.
L’acidose métabolique est un trouble fréquemment rencontré en médecine d’urgence et en médecine intensive réanimation. La littérature s’étant enrichie de nouvelles données concernant la prise en ...charge de l’acidose métabolique, la Société de Réanimation de Langue Française (SRLF) et la Société Française de Médecine d’Urgence (SFMU) ont élaboré des recommandations formalisées d’experts selon la méthodologie GRADE. Les champs de la stratégie diagnostique, de l’orientation et de la prise en charge thérapeutique ont été traités et vingt-neuf recommandations ont été formulées : quatre recommandations fortes (Grade 1), dix recommandations faibles (Grade 2) et quinze avis d’experts. Toutes ont obtenu un accord fort. L’application des méthodes d’Henderson-Hasselbalch et de Stewart pour le diagnostic du mécanisme de l’acidose métabolique est discutée et un algorithme diagnostique est proposé. L’utilisation de la cétonémie et des lactatémies veineuse et capillaire est également traitée. L’intérêt du pH, de la lactatémie et de sa cinétique pour l’orientation des patients en pré-hospitalier et aux urgences est envisagé. Enfin, les modalités de l’insulinothérapie au cours de l’acidocétose diabétique, les indications de la perfusion de bicarbonate de sodium et de l’épuration extra-rénale ainsi que les modalités de la ventilation mécanique au cours des acidoses métaboliques sévères sont traitées dans la prise en charge thérapeutique.
Metabolic acidosis is a disorder frequently encountered in emergency medicine and intensive care medicine. As the literature has been enriched with new data on the management of metabolic acidosis, the French-speaking Intensive Care Society (SRLF) and the French Society of Emergency Medicine (SFMU) have developed formalized recommendations from experts using the GRADE methodology. The fields of diagnostic strategy, guidance and therapeutic management were addressed and 29 recommendations were made: four strong recommendations (Grade 1), ten weak recommendations (Grade 2) and fifteen expert opinions. All of them have reached a strong agreement. The application of Henderson–Hasselbalch and Stewart methods for the diagnosis of the metabolic acidosis mechanism is discussed and a diagnostic algorithm is proposed. The use of ketosis and venous and capillary lactatemia is also treated. The value of pH, lactatemia and its kinetics for the referral of patients in pre-hospital and emergency departments is considered. Finally, the modalities of insulin therapy during diabetic ketoacidosis, the indications for sodium bicarbonate infusion and extra-renal purification as well as the modalities of mechanical ventilation during severe metabolic acidosis are addressed in therapeutic management.
Les descriptions de l’incidence, de la gravité et des modalités de prise en charge des blocs auriculoventriculaires du troisième degré (BAV3) compliquant la phase aiguë des infarctus du myocarde, ...avec sus-décalage du segment ST (STEMI), sont rares et anciennes. Par ailleurs, les modalités de prise en charge des STEMI aigus ont beaucoup évolué. Le but de notre étude est d’évaluer l’incidence, de décrire la gravité et les modalités de prise en charge des BAV3 survenant à la phase aiguë des STEMI dans un contexte contemporain.
Matériel et méthodes
: Nous avons réalisé une étude observationnelle rétrospective de la prise en charge des STEMI aigus à partir du registre prospectif des STEMI du Réseau nord alpin des urgences (RENAU) sur les 19 hôpitaux des Alpes du Nord entre 2009 et 2012. Les patients présentant un BAV3 à la phase initiale de leur prise en charge ont été identifiés.
Résultats
: Deux mille sept cent neuf patients avec STEMI aigu ont été inclus sur la période d’étude. Cinquante-sept ont présenté un BAV3 (2 %). Cent cinquante-deux des 2 648 patients sans BAV3 (6 %) sont décédés à la phase hospitalière contre 7 des 57 patients (12 %) avec BAV3 (
p
= 0,047). Parmi les patients en BAV3, un traitement par atropine a été utilisé pour 26 patients et s’est révélé efficace pour (15 %) d’entre eux. L’isoprénaline a été utilisée pour huit patients et a induit une hypotension artérielle pour quatre d’entre eux. Une stimulation ventriculaire droite percutanée a été utilisée pour six patients et a toujours été efficace. Quatorze patients ont été thrombolysés (25 %). Une angioplastie de sauvetage a été nécessaire chez 10 des 14 patients en BAV3 traités par thrombolyse (71 %), comparés aux 325 des 840 patients sans BAV3 traités par thrombolyse (39 % ;
p
= 0,013).
Conclusion
: L’incidence des BAV3 à la phase aiguë des STEMI est faible. Le BAV3 représente un facteur de sévérité. Les stratégies thérapeutiques sont d’efficacité inégale, avec des effets indésirables induits par l’isoprenaline. La thrombolyse chez ces patients est moins efficace.
Introduction
: Third-degree atrioventricular block (TDAVB) can complicate acute myocardial infarction (AMI). However, optimal management is still controversial. The aim of this study was to assess the initial management of post-AMI TDAVB in a large French register.
Procedure
: We performed a retrospective study from a prospective register called “Réseau Nord Alpin des Urgences” (“North Alpine Network Register”), which integrates all patients with the diagnosis of acute coronary syndrome managed in the first 12 hours, between January 2009 and December 2012.
Results
: Among the 2709 patients of the study 57 (2%) had a TDAVB.
The presence of TDAVB was an independent predictor of in-hospital mortality (
P
= 0.047).
Atropine was used in 46% and was effective in 15% of cases. About 8 of 57 patients were treated with isoprenaline of which 50% developed a fall in blood pressure. In 11% of cases among the 57 patients, a transthoracic pacing was employed, with 100% efficacy.
Thrombolysis was used in 25 % of cases. Patients with TDAVB treated with thrombolysis were more likely to receive a rescue percutaneous intervention compared to the group of patients without TDAVB (71% vs 39 %;
P
= 0.013).
Conclusion
: Most effective prehospital treatment after testing atropine is transthoracic pacing. In the presence of TDAVB, reperfusion should be sought via primary percutaneous intervention due to the high prevalence of rescue interventions post thrombolysis in this group.
Influenza is a public health issue worldwide. Although antibiotics should not be used to treat viral infections, they are often prescribed to patients with influenza-like illness (ILI). Such misuse ...promotes antibiotic resistance. The role of rapid point-of-care tests (POCTs) in preventing antibiotic misuse in adults with ILI symptoms remains relatively unexplored.
To evaluate whether POCT implemented in 2018–2019 to detect influenza viruses led to a decrease in antibiotic prescriptions compared with laboratory-based influenza tests.
Adult patients with ILI in one emergency department (ED) were retrospectively enrolled over three epidemic seasons (from 2016–2017 to 2018–2019). The primary outcome was the rate of antibiotic prescriptions, which was compared between the three seasons in bivariate and multivariate analyses. Prescriptions for ancillary laboratory tests, chest X-rays and oseltamivir were also compared, along with hospitalizations and length of stay (LOS) at the ED.
Overall, 1849 patients were included. Median age was over 70 years throughout all three seasons. The number of antibiotic prescriptions was significantly different between the three periods in bivariate analysis (48.3% in 2016/2017, 44% in 2017/2018 and 31.1% in 2018/2019; P<0,0001) and in multivariate analysis (adjusted odds ratio (aOR) = 0.48, 95% confidence interval (CI) = 0.30–0.76 for 2018/2019 and aOR = 0.99, 95%CI = 0.67–1.46 for 2017/2018, compared with 2016/2017). There were significantly fewer prescriptions of ancillary laboratory tests, X-rays, hospitalizations and more oseltamivir prescriptions in 2018/2019, compared with the previous seasons. LOS was significantly lower in 2018/2019 only for influenza-positive patients.
ED influenza POCT decreased antibiotic use and led to less ancillary testing, X-rays and hospitalizations among patients with ILI. However, medico-economic studies are necessary before formulating definite recommendations.
Efficient ventilation is important during cardiopulmonary resuscitation (CPR). Nevertheless, there is insufficient knowledge on how the patient’s position affects ventilatory parameters during ...mechanically assisted CPR. We studied ventilatory parameters at different positive end-expiratory pressure (PEEP) levels and when using an inspiratory impedance valve (ITD) during horizontal and head-up CPR (HUP-CPR).
In this human cadaver experimental study, we measured tidal volume (VT) and pressure during CPR at different randomized PEEP levels (0, 5 or 10 cmH2O) or with an ITD. CPR was performed, in the following order: horizontal (FLAT), at 18° and then at 35° head-thorax elevation. During the inspiratory phase we measured the net tidal volume (VT) adjusted to predicted body weight (VTPBW), reversed airflow (RAF), and maximum and minimum airway pressure (Pmax and Pmin).
Using ten thawed fresh-frozen cadavers we analyzed the inspiratory phase of 1843 respiratory cycles, 229 without CPR and 1614 with CPR. In a mixed linear model, thoracic position and PEEP significantly impacted VTPBW (p < 0.001 for each), and the insufflation time, thoracic position and PEEP significantly affected the RAF (p < 0.001 for each) and Pmax (p < 0.001). For Pmin, only PEEP was significant (p < 0.001). In subgroup analysis, at 35° VTPBW and Pmax were significantly reduced compared with the flat or 18° position.
When using mechanical ventilation during CPR, it seems that the PEEP level and patient position are important determinants of respiratory parameters. Moreover, tidal volume seems to be lower when the thorax is positioned at 35°.
Mountainous areas pose a challenge for the out-of-hospital cardiac arrest (OHCA) chain of survival. Survival rates for OHCAs in mountainous areas may differ depending on the location. Increased ...survival has been observed compared to standard location when OHCA occurred on ski slopes. Limited data is available about OHCA in other mountainous areas. The objective was to compare the survival rates with a good neurological outcome of OHCAs occurring on ski slopes (On-S) and off the ski slopes (OffS) compared to other locations (OL).
Analysis of prospectively collected data from the cardiac arrest registry of the Northern French Alps Emergency Network (RENAU) from 2015 to 2021. The RENAU corresponding to an Emergency Medicine Network between all Emergency Medical Services and hospitals of 3 counties (Isère, Savoie, Haute-Savoie). The primary outcome was survival at 30 days with a Cerebral Performance Category scale (CPC) of 1 or 2 (1: Good Cerebral Performance, 2: Moderate Cerebral Disability).
A total of 9589 OHCAs were included: 213 in the On-S group, 141 in the Off-S group, and 9235 in the OL group. Cardiac etiology was more common in On-S conditions (On-S: 68.9% vs OffS: 51.1% vs OL: 66.7%, p < 0.001), while Off-S cardiac arrests were more often due to traumatic circumstances (OffS: 39.7% vs On-S: 21.7% vs OL: 7.7%, p < 0.001). Automated external defibrillator (AED) use before rescuers' arrival was lower in the Off-S group than in the other two groups (On-S: 15.2% vs OL: 4.5% vs OffS: 3.7%; p < 0.002). The first AED shock was longer in the Off-S group (median time in minutes: OffS: 22.0 (9.5–35.5) vs On-S: 10.0 (3.0–19.5) vs OL: 16.0 (11.0–27.0), p = 0.03). In multivariate analysis, on-slope OHCA remained a positive factor for 30-day survival with a CPC score of 1 or 2 with a 1.96 adjusted odds ratio (95% confidence interval (CI), 1.02–3.75, p = 0.04), whereas off-slope OHCA had an 0.88 adjusted odds ratio (95% CI, 0.28–2.72, p = 0.82).
OHCAs in ski-slopes conditions were associated with an improvement in neurological outcomes at 30 days, whereas off-slopes OHCAs were not. Ski-slopes rescue patrols are efficient in improving outcomes.
To provide an overview of cadaver models for cardiac arrest and to identify the most appropriate cadaver model to improve cardiopulmonary resuscitation through a systematic review.
The search ...strategy included PubMed, Embase, Current contents, Pascal, OpenSIGLE and reference tracking. The search concepts included “heart arrest”, “cardiopulmonary resuscitation” and “cadavers”. All studies, published until February 2019, in English or French, on research or simulation in the field of cardiac arrest and using cadaver models were eligible for inclusion.
Overall, 29 articles out of the 244 articles located were selected. The characteristics of the studies and the cadaver models were heterogenous. Indeed, 31% of the studies lacked a proper description of the model used and its specificities. Fresh cadavers were used in 55% of the studies and chest compressions were performed in 90%. This model was appreciated for its realism in terms of mechanical properties and tissue conservation. Thiel-embalmed cadavers also showed promising results concerning lung and chest compliance. The lack of circulation stood out as the strongest limitation of all types of human cadaver models.
Four types of cadaver models are used in cardiac arrest research. The great heterogeneity of these models coupled with unequal quality in reporting makes comparisons between studies difficult. There is a need for uniform reporting and standardisation of human cadaver models in cardiac arrest research.
Les hospitalisations en médecine interne via les urgences représentent une part importante du nombre total d’hospitalisations. Les urgentistes posent un diagnostic initial qui influence le ...déroulement du traitement après l’admission. Des discordances avec le diagnostic de sortie de médecine interne sont décrites dans la littérature, avec de potentielles conséquences médicales et économiques. L’objectif principal de notre étude est d’évaluer le taux de discordance diagnostique parmi les patients hospitalisés en médecine interne via les urgences, avec un diagnostic préliminaire ou définitif d’origine infectieuse. Les objectifs secondaires sont d’identifier les facteurs associés à ces écarts de diagnostics et d’en mesurer les conséquences.
Nous avons réalisé une étude rétrospective monocentrique sur les patients hospitalisés en médecine interne au Centre Hospitalier Grenoble Alpes via un service d’urgence du 22 mars 2021 au 22 juin 2021 inclus. Les patients ainsi que les facteurs susceptibles d’influencer la discordance diagnostique ont été inclus après leur sortie de l’hôpital. Nous avons déterminé s’il y avait un écart diagnostique entre le diagnostic de sortie des urgences et de sortie du service de médecine interne. Puis nous avons comparé les caractéristiques des patients et les conséquences médicales selon la présence ou l’absence d’un écart diagnostique.
Sur la période d’inclusion, 559 patients ont été hospitalisés en médecine interne. Parmi ces patients, 280 provenaient des urgences et avaient un diagnostic infectieux retenu aux urgences et/ou en médecine interne. Il s’agissait principalement d’hommes (59,3 %), âgés de 80 ans en moyenne, avec un score de Charlson supérieur à 6 et avec une prise de 7 médicaments par jour. Une infection COVID était le diagnostic en sortie d’hospitalisation chez moins d’un tiers des patients inclus. Un taux de discordance diagnostique a été retrouvé chez 65 des 280 patients inclus soit 23,2 %. Ils concernaient en particulier les infections respiratoires (16 patients) et urinaires (11 patients) ainsi que les diagnostics étiquetés d’origine non infectieuse (16 patients). Parmi les caractéristiques intrinsèques des patients pouvant prédire une discordance diagnostique, on note un âge plus élevé (p-value 0,0098), un score de Charlson plus important (p-value 0,03337). Parmi les caractéristiques du passage aux urgences, on retrouve un temps de passage plus long aux urgences (p-value 0,01599). La conclusion à une infection COVID en fin d’hospitalisation de médecine interne était associée de manière significative à l’absence de discordance diagnostique (p-value 0,0004176). Concernant l’étude de l’impact, on met en évidence une association statistiquement significative avec l’arrêt de l’antibiothérapie à l’arrivée dans le service (54 % vs 7,3 %, OR 0,07 IC95 %0,02 ; 0,22, p-value<0,0001).
La discordance diagnostique est un problème de santé publique qui affecte tous les services de l’hôpital mais aussi la médecine ambulatoire. L’intégration des connaissances actuelles dans notre pratique ainsi que la multiplication d’études robustes sur la prévalence et la recherche de facteurs causals sont nécessaires en vue de l’élaboration d’études interventionnelles dans l’avenir.
