INTRODUCTION: In Brazil, about 26,000 cases of colorectal cancer are diagnosed per year. Patients considered at the early stage of disease (without lymph node) evolve with tumor relapse or recurrence ...in up to a quarter of cases, probably due to understaging. Objective: Research on ex vivo sentinel lymph node in patients with colorectal adenocarcinoma. MATERIALS AND METHODS: We studied 37 patients who underwent curative surgical resection. The marker used to identify lymph nodes was patent blue dye injected into the peritumoral submucosa of the open surgical specimen immediately after its removal from the abdominal cavity. RESULTS: Ex vivo identification of sentinel lymph node with marker occurred in 13 (35.1%) patients. The sensitivity was 40% and 60% false negative. The detailed histological examination of sentinel lymph nodes with multilevel section and immunohistochemistry showed metastasis in one (4.3%) individual, considered ultra-staging. CONCLUSION: The ex vivo identification of sentinel lymph node had questionable benefits, and worse results when include patients with rectal cancer. Restaging of one patient was possible after multilevel section and immunohistochemistry of the sentinel lymph node, but more research is needed to evaluate the role of micrometastases in patients with colorectal cancer.
OBJETIVO: Analisar retrospectivamente uma série de casos que culminaram em amputação por carcinoma escamocelular avançado. MÉTODOS: Foram estudados oito pacientes com diagnóstico histológico de ...carcinoma escamocelular de pele que tiveram membros amputados por invasão tumoral, em nossa Instituição entre 2005 e 2008. Foram avaliados: fatores histológicos, institucionais e psicossociais que contribuíram para este desfecho desfavorável. RESULTADOS: A média de idade ao diagnóstico foi 63 anos; 37,5% dos pacientes (três pacientes) tinham exposição abusiva e contínua aos raios solares, e seis (75%) pacientes tinham outros fatores de risco para CEC de pele. Sete pacientes foram diagnosticados quando o tumor já era maior que 2cm, e foi necessário um período grande de tempo (6,7 anos em média) entre o aparecimento da lesão inicial e o diagnóstico histopatológico correto do tumor localmente avançado. CONCLUSÃO: O desfecho desfavorável (amputação) nos pacientes portadores de carcinoma espinocelular pode estar associado à agressividade da neoplasia e às comorbidades relacionadas, podendo ainda sofrer influência de fatores como acesso à saúde pública, qualidade da assistência médica e relação do paciente com a doença.
OBJECTIVE: To analyze retrospectively a series of cases culminating in amputation for advanced squamous cell carcinoma. METHODS: We studied eight patients with histologically confirmed squamous cell carcinoma of the skin that had limbs amputated by tumor invasion at our institution between 2005 and 2008. We evaluated the histological factors and the institutional and psychosocial factors that contributed to this unfavorable outcome. RESULTS: The mean age at diagnosis was 63 years, 37.5% of patients (three patients) had a history of abusive and continuous exposure to sunlight and six (75%) patients had other risk factors for SCC of the skin. Seven patients were diagnosed when the tumor was already larger than 2cm, and it required a large period of time (6.7 years on average) between the onset of the initial lesion and the correct histopathological diagnosis of locally advanced tumor. CONCLUSION: The unfavorable outcome (amputation) in patients with squamous cell carcinoma may be associated with aggressiveness of cancer and related comorbidities, and may also be influenced by factors such as access to public health, quality of care and patient's relationship with the disease.

Na apendicectomia laparotômica há preferência pela ...ligadura e sepultamento do coto enquanto, na laparoscópica, pela ligadura simples ou clampeamento. OBJETIVOS: Comparar duas técnicas de tratamento do coto apendicular na apendicectomia laparotômica (ligadura simples vs ligadura com confecção de bolsa e sepultamento) por meio de análise prospectiva e randomizada. MÉTODOS: Entre os anos de 2003 e 2005, 113 pacientes foram submetidos à apendicectomia laparotômica por apendicite aguda. O coto apendicular foi tratado por ligadura simples em 49 casos e por ligadura e sepultamento em 64 casos. Os dois grupos foram semelhantes em relação às médias de idade, gênero, sinais e sintomas pré-operatórios, tempo de evolução da doença e fase da apendicite aguda diagnosticada pelo exame anatomopatológico. RESULTADOS: Não houve diferença estatística significativa entre os dois grupos em relação ao tempo de duração da operação, presença de complicações (infecção de ferida operatória, deiscência de ferida, formação de abscessos, obstrução intestinal, seroma e formação de fístula) ou tempo de internação. CONCLUSÃO: As duas técnicas são igualmente seguras para tratamento do coto apendicular.

At laparotomy for appendectomy the stump is treated preferably by ligation and burial of the stump and, in laparoscopic surgery, by simple ligation or clamping. AIM: To compare two techniques for the treatment of appendicular stump in appendectomy (simple ligation vs ligation with purse making and burial) by a prospective and randomized study. METHODS: Between the years 2003 and 2005, 113 patients underwent laparotomy appendectomy for acute appendicitis. The appendiceal stump was treated by simple ligation in 49 cases and by ligation and burial in 64 cases. The two groups were similar in terms of average age, gender, preoperative signs and symptoms, time of disease progression and stage of appendicitis diagnosed histopathological examination. RESULTS: There was no statistically significant difference between the two groups regarding the duration of operation, complications (wound infection, wound dehiscence, abscess formation, intestinal obstruction, fistula and seroma ) or hospital stay. CONCLUSIONS: Both techniques are equally safe for treatment of appendicular stump, and so, it is possible to recommend simple ligation because it is easier to be done.
