Smoking is still one of the most dangerous and avoidable health risks. This study "Healthy air at work" analysed smoking habits, state of change, the influence of the diagnosis F.17.0 in patient ...treatment and estimation of subjective workloads and personal resources in health-care workers. Almost 2 000 questionnaires were analysed. 19.9% of this study population were smokers, while 26.4% were considered to be heavy or very heavy smokers. Half of the current smokers were willing to change, while the majority had already tried to quit multiple times. The most important motive to stop smoking was fear of consequences (44.4%), followed by other reasons (42.3%) (e. g., pregnancy) and expenses (33.9%). Protection against second-hand smoke was estimated mostly as very relevant, especially for patients. Being a role model in terms of tobacco consumption seems to be important for health-care workers. 61.3% of all health-care workers stated that patients' nicotine dependency had been diagnosed and out of these 46.5% say it is a relevant factor in therapy. 60% of all interviewed employees evaluated themselves as working quantitatively under heavy and very heavy workloads, while 20% had to deal with high qualitative challenges. In terms of future work ability and personal resources 75% were considerably optimistic. We did not find any relation in terms of workloads and smoking habits. Rather few health-care workers used nicotine replacement therapy during former cessation trials. Health-care workers could play an important role in the treatment and prevention of smoking dependency. This potential is not used to its full extent up to now.
The alpha-hemolysin (alpha HL) from Staphylococcus aureus causes the lysis of susceptible cells such as rabbit erythrocytes (rRBCs). Lysis is associated with the formation of a hexameric pore in the ...plasma membrane. Here we show that truncation mutants of alpha HL missing 2 to 22 N-terminal amino acids form oligomers on the surfaces of rRBCs but fail to lyse the cells. By contrast, mutants missing 3 or 5 amino acids at the C terminus are very inefficient at oligomerization but do lyse rRBCs, albeit extremely slowly. The C-terminal truncation mutants, retarded as monomers on the cell surface, undergo a conformational change in which the protease-sensitive loop located near the midpoint of the polypeptide chain becomes occluded. Judged by this criterion, polypeptides truncated at the N terminus, frozen as nonlytic oligomers, are in a similar conformation. A second proteolytic site near the N terminus of the polypeptide becomes inaccessible in the lytic pore formed by the wild-type polypeptide, supporting the idea that a second conformational change occurs upon pore formation. These findings suggest a pathway for assembly of the lytic pore in which alpha HL first binds to target cells as a monomer, which is converted to a nonlytic oligomeric intermediate before formation of the pore. In keeping with this model, an N-terminal truncation mutant blocks the slow lysis induced by a C-terminal truncation mutant, presumably by diverting the weakly lytic subunits into inactive oligomers.
Objectives To assess the efficacy, reliability, and performance of the LapraTy clip (LTc) as a substitute for knot tying during reconstructive surgery in a porcine model. Methods Twenty-four farm ...pigs were divided in two groups, each undergoing two surgical procedures: group A, transperitoneal laparoscopic cavotomy and small-bowel enterotomy with repair, and group B, laparoscopic partial nephrectomy and cystotomy with repair. In all animals LTc were used to replace knot tying. Tissue specimens were harvested and examined at 2, 4, and 8 weeks postoperatively to assess success of reconstruction and tissue reaction. Results There were no major complications. Animals in group A showed no clinical signs of caval obstruction. No intraperitoneal collections or significant narrowing were noted at the enterotomy sites. Animals in group B showed no evidence of fluid collections around the partially resected kidneys or the cystotomy sites, and all repairs were intact. Pathologic examination revealed that all LTc were encapsulated by fibroblasts and giant cells typical of a foreign body–type granulomatous reaction. No evidence of clip migration into the epithelium was noted in any of the tissues examined. Conclusions In an animal model, the LTc is a safe and efficient alternative to knot tying during laparoscopic reconstructive surgery. We are currently evaluating the clinical applicability of the LTc in a variety of urologic conditions. We believe that other surgical specialties should evaluate this device as well.
Children who have been severely maltreated tend to perform significantly below their non-maltreated peers in standardized tests, earn lower grades and have the most discipline issues in the school ...setting. There is evidence that self-esteem (SE) may be a protective factor for youth with regard to negative emotional outcomes. The role of self-esteem needs to be explored further in more collectivistic cultures. The purpose of this study was to explore the relationship between physical abuse, SE and school attitudes. Participants included 14 females rescued from a city landfill in Nicaragua (ages 7-17 years, M=12.44), half of whom had reported being beaten. SE and attitude toward school (ATS) were assessed using the Spanish Behavior Assessment System for Children. Grade point average (GPA) was obtained from their schools. Females who were beaten had significantly lower SE and a more negative ATS. Their GPA was lower, but not significantly. When SE and abuse were considered together, the effect of abuse on school attitudes was non-significant, suggesting that SE is a protective factor for the effects of abuse on ATS.

Our group is developing a scanner that combines x-ray, single gamma, and optical imaging on the same rotating gantry. Two functional modalities (SPECT and optical) are included because they have ...different strengths and weaknesses in terms of spatial and temporal decay lengths in the context of in vivo imaging, and because of the recent advent of multiple reporter gene constructs. The effect of attenuation by biological tissue on the detected intensity of the emitted signal was measured for both gamma and optical imaging. Attenuation by biological tissue was quantified for both the bioluminescent emission of luciferace and for the emission light of the near infrared fluorophore cyanine 5.5, using a fixed excitation light intensity. Experiments were performed to test the feasibility of using either single gamma or x-ray imaging to make depth-dependent corrections to the measured optical signal. Our results suggest that significant improvements in quantitation of optical emission are possible using straightforward correction techniques based on information from other modalities. Development of an integrated scanner in which data from each modality are obtained with the animal in a common configuration will greatly simplify this process.
This study had two objectives. Firstly we assessed the effects of multifetal pregnancy reduction on the mothers' emotional well-being and the relationship with the children during the 2 years ...following intervention. Secondly at 2 years we compared mothers who had a reduction with mothers who had not and had delivered triplets. The comparisons focused on the mothers' health and their relationship with the children. Women having had a reduction in two hospitals in Paris, between May 1992 and June 1993, were contacted just after intervention for a prospective study. In all, 18 women were included. At 1 and 2 years, 10 women participated. At 2 years, 10 additional women were included. The answers of these 20 mothers were compared to those of 11 consecutive mothers of 2 year old triplets, assessed by the same psychologist in a previous prospective study. Semi-structured interviews were conducted at home. The mothers' social characteristics, their parity, the children's condition at birth and 4 months were very similar between the reduction and triplet groups. One year after birth one-third of the women in the reduction group reported persistent depressive symptoms related to the reduction, mainly sadness and guilt. The others made medical and rational comments expressing no emotion. At 2 years all but two women seemed to have overcome the emotional pain associated with the reduction. The comparison with mothers of triplets indicated that the mothers' anxiety and depression, and difficult relationship with the children were less acute in the reduction group. These results presented some limitations, since a high number of women who miscarried or refused to participate in the follow-up were not assessed at 1 and 2 years. However, a majority of women who participated in the study 2 years after intervention seemed able to accept a multifetal pregnancy reduction to achieve parental goals.
The objective of this study was to compare the efficacy and safety of a recombinant follicle stimulating hormone (FSH) preparation (Org 32489, Puregon) with a urinary FSH preparation (Metrodin) in ...infertile women undergoing in-vitro fertilization (IVF and embryo transfer and who were pituitary-suppressed with triptorelin. In an assessor-blind, group-comparative, multicentre study, 60 women were randomized to Org 32489 and 39 to urinary FSH. An evaluation of the main parameter, the mean total number of oocytes recovered, indicated a similar efficacy for the two preparations: 9.7 with Org 32489 versus 8.9 with urinary FSH. In addition, there were no significant between-group differences with respect to other efficacy variables such as the total dose used, the duration of the treatment, the number of follicles > or = 17 mm in diameter and embryo quality. The ongoing pregnancy rates per attempt (30.2 versus 17.4%) and per transfer (34.0 versus 18.8%) were higher with Org 32489, but this difference was not statistically significant. No clinically relevant differences between Org 32489 and urinary FSH were seen with respect to safety variables. Serum antibodies were not detected in any of the subjects. It is concluded that Org 32489 compares favourably with urinary FSH in the treatment of infertile pituitary-suppressed women undergoing IVF and embryo transfer.
Purpose We assessed the safety and efficacy of microparticulate ice slurry for laparoscopic hypothermia during renal ischemia in a single kidney porcine model. Materials and Methods A total of 18 ...farm pigs were randomized to 3 groups of 6 each. All groups underwent initial right laparoscopic nephrectomy, followed by 1 of 3 procedures on the left kidney. Group 1 underwent 90-minute hilar clamping under warm ischemia, group 2 underwent 90-minute hilar clamping under cold ischemia using laparoscopically delivered microparticulate ice slurry and control group 3 underwent hilar dissection, no clamping and no microparticulate ice slurry. Body and renal cortical temperatures were measured. Serum creatinine and the glomerular filtration rate were assessed preoperatively, and on postoperative days 1, 3, 8 and 15. Results Average time to achieve a renal temperature of 20C or less was 9.7 minutes and it remained constant during the 90-minute cold ischemia time. Mean serum creatinine was significantly higher in the warm ischemia group than in the cold ischemia and control groups on postoperative days 1 and 3. Additionally, mean serum creatinine in the cold ischemia and control groups was similar at all time points. The mean glomerular filtration rate was significantly lower in the warm ischemia group than in the cold ischemia and control groups on postoperative days 1, 3 and 8. The mean glomerular filtration rate in the cold ischemia group was lower than in the control group on postoperative day 1, while it was similar on postoperative days 3, 8 and 15. Conclusions In the porcine model laparoscopic renal hypothermia achieved with microparticulate ice slurry was safe and efficient. It significantly decreased renal dysfunction secondary to an ischemic insult with no adverse effects or complications associated with microparticulate ice slurry use.
