The treatments for synchronous head and neck cancer (HNC) and esophageal cancer (ESC) are toxic and difficult to employ. The aim of this study was to identify the feasibility of a protracted, less ...toxic treatment course and prognostic factor of synchronous HNC and ESC.
Cancer registry data from 2004 to 2012 were reviewed. The inclusion criteria were two cancer diagnoses within 30 days, and Stage III/IV HNC or Stage II-IV ESC that chemoradiation therapy was indicated. Evident metastasis, Eastern Cooperative Oncology Group performance score >2, a history of prior cancer, or palliative treatment were excluded. Survival rates and patient and treatment characteristics were analyzed.
There were 51 eligible cases. The 2 year overall survival rate was 25.1%. Univariate analysis found that anemia, larynx/hypopharynx HNC, and no esophagectomy correlated with poor overall survival. Multivariate analysis demonstrated that anemia and no esophagectomy were independent poor prognostic factors. The 2 year progression-free survival rate was 14.8%. Univariate analysis found only no esophagectomy correlated with poor progression-free survival.
The outcomes are poor for patients with advanced synchronous HNC and ESC. Radiotherapy with a split or protracted course does not result in inferior treatment result and can be considered when the aim is to avoid adverse events. Esophagectomy correlated with good prognosis and should be performed for patients if possible.
The treatment results of synchronous HNC and ESC is poor. A protracted chemoradiation course for synchronous HNC and ESC did not result in inferior survival and should be applied to patients with a poor prognosis. Esophagectomy correlates with good outcomes and should be encouraged if the patient has a good prognosis.
To investigate motion artifacts on kV CBCT and MV CBCT images with metal localization devices for image-guided radiation therapy.
The 8 μ pelvis CBCT template for the Siemens Artiste MVision and ...Pelvis template for the Varian IX on-board Exact Arms kV were used to acquire CBCT images in this study. Images from both CBCT modalities were compared in CNRs, metal landmark absolute positions, and image volume distortion on three different planes of view. The images were taken on a breathing-simulated thoracic phantom in which several typical metal localization devices were implanted, including clips and wires for breast patients, gold seeds for prostate patients, and BBs as skin markers. To magnify the artifacts, a 4 cm diameter metal ball was also implanted into the thoracic phantom to mimic the metal artifacts.
For MV CBCT, the CNR at a 4 sec breathing cycle with 1 cm breathing amplitude was 5.0, 3.4 and 4.6 for clips, gold seeds and BBs, respectively while it was 1.5, 2.0 and 1.6 for the kV CBCT. On the images, the kV CBCT showed symmetric streaking artifacts both in the transverse and longitudinal directions relative to the motion direction. The kV CBCT images predicted 89 % of the expected volume, while the MV CBCT images predicted 95 % of the expected volume. The simulated soft tissue observed in the MVCT could not be detected in the kV CBCT.
The MV CBCT images showed better volume prediction, less streaking effects and better CNRs of a moving metal target,
. clips, BBs, gold seeds and metal balls than on the kV CBCT images. The MV CBCT was more advantageous compared to the kV CBCT with less motion artifacts for metal localization devices.
This study would benefit clinicians to prescribe MV CBCT as localization modality for radiation treatment with moving target when metal markers are implanted.
The purpose of this study was to assess the utility of CT-based renal cortex volume to estimate split renal function (SRF) in pre-transplant living kidney donors and to evaluate its reliability to ...predict graft function in the recipients.
Our study recruited all adult potential donors who had both Tc-99m-diethylenetriamine pentacetate (DTPA) scintigraphy and CT angiography of the abdominal aorta done before donating their kidney. We compared the correlation between CT-based renal cortex volume combined with kidney function and DTPA scan as well as post-donation kidney function in the recipients.
The correlation between CT-based split cortex volume and DTPA-measured SRF before transplantation was strong (intraclass correlation coefficient = 0.954-0.968). The inter-rater reliability of two radiologists also showed substantial agreement (intraclass correlation coefficient = 0.97,
< 0.001). In contrast, the correlations between renal cortical volume of donated kidney adjusted to recipient body weight and recipient kidney function was poor at both 2 week and 2 year follow-up.
CT-based renal cortex volume combined with pre-operative kidney function appears to be precise and reproducible to evaluate pre-transplant SRF. Nevertheless, the prediction of recipient graft function needs to be further investigated to ensure a good outcome.
This method is practicable for all potential donors who undergo kidney transplantation in terms of streamline donor workup without compromising information.
The aim of this study was to investigate low-dose kilovoltage cone-beam CT (kV-CBCT) for image-guided radiotherapy, with a particular focus on the accuracy of image registration with low-dose ...protocols.
Imaging doses were measured with a NOMEX semiconductor detector positioned at the front of head, thorax, and pelvis human body phantoms while kV-CBCT scans were acquired at different tube currents. Aspects of image quality (spatial resolution, noise, uniformity, contrast, geometric distortion, and Hounsfield unit sensitivity) and image registration accuracy using bone and soft tissue were evaluated.
With preset and the lowest tube currents, the imaging doses were 0.16 and 0.08 mGy, 5.29 and 2.80 mGy, and 18.23 and 2.69 mGy for head, thorax, and pelvis, respectively. Noise was the only quality aspect directly dependent on tube current, being increased by 1.5 times with a tube current half that of the preset in head and thorax, and by 2.2 times with a tube current 1/8 of the preset in the pelvis. Accurate auto-bone matching was performed within 1 mm at the lowest tube current. The auto-soft tissue matching could not be performed with the lowest tube current; however, manual-soft tissue matching could still be performed within 2 mm or less.
Noise was the only image quality aspect dependent on the imaging dose. Auto-bone and manual-soft tissue matching could still be performed at the lowest imaging dose.
When optimizing kV-CBCT imaging dose, the impact on bone and soft tissue image registration accuracy should be evaluated.

H) MR spectroscopy (MRS) has evolved as a non-invasive tool for diagnosis and for biochemical characterization of breast ...cancer. Water-to-fat ratio, fat and water fractions and choline containing compounds (tCho) have been identified as diagnostic biomarkers of malignancy. Detection of tCho in normal breast tissue of volunteers and in lactating females limits the use of tCho as a diagnostic marker. Technological developments like high-field scanners, multi channel coils, pulse sequences with water and fat suppression facilitated easy detection of tCho. Also, quantification of tCho and its cut-off for objective assessment of malignancy have been reported. Meta-analysis of
H MRS studies have documented the pooled sensitivities and the specificities in the range of 71-74% and 78-88%, respectively. Inclusion of MRS has been shown to enhance the diagnostic specificity of MRI, however, detection of tCho in small sized lesions (≤1 cm) is challenging even at high magnetic fields. Potential of MRS in monitoring the effect of chemotherapy in breast cancer has also been reported. This review briefly presents the potential clinical role of

Junior doctors routinely request radiological investigations for patients. Prior studies have noted that among this group there is a lack of knowledge on radiation legislation and radiation exposure ...in common radiological investigations. However, no studies have compared this against radiology trainees and radiographers. We compared knowledge of radiation legislation and radiation exposure in common radiological investigations among final year medical students (FYMS), foundation year doctors (FY1, FY2) against specialist radiology trainees (SRT) and radiographers (RG).
A 12-question multiple choice questionnaire (MCQ) was distributed to FYMS, FY1, FY2, SRT and RG at a UK teaching hospital. Questions assessed knowledge of radiation legislation and radiation-dose estimates of common radiological investigations. Mean MCQ scores were compared using one-way ANOVA and Tukey post-test to determine statistical significance (
-value < 0.05).
127 participants were included in the study. Mean scores (%) for FYMS (49.3%), FY1 (52.6%) and FY2 (51.1%) were significantly lower compared to SRT (64.4%) and RG (66.3%) (
-value < 0.05). Mean test scores between FYMS, FY1 and FY2 did not significantly differ (
-value > 0.05).
FYMS, FY1 and FY2 knowledge of radiation legislation and radiation exposure in common radiological investigations was poor compared to SRT and RG. Patients require knowledge of radiation risk to provide informed consent as per IRMER regulations, thus we propose formal teaching on the subject matter to promote radiation safety culture among medical undergraduates and postgraduates.

Machine learning promises much in the field of radiology, both in terms of software that can directly analyse patient data and algorithms that can automatically perform other processes in the ...reporting pipeline. However, clinical practice remains largely untouched by such technology. This article highlights what we consider to be the major obstacles to widespread clinical adoption of machine learning software, namely: representative data and evidence, regulations, health economics, heterogeneity of the clinical environment and support and promotion. We argue that these issues are currently so substantial that machine learning will struggle to find acceptance beyond the narrow group of applications where the potential benefits are readily evident. In order that machine learning can fulfil its potential in radiology, a radical new approach is needed, where significant resources are directed at reducing impediments to translation rather than always being focused solely on development of the technology itself.
Post-mortem CT (PMCT) can replace autopsy in many cases of non-suspicious death. A purely NHS-based service to replace autopsy with PMCT was launched, with the cost met by the family from 2015 to ...2017, and subsequently "free at the point of delivery" after local authority funding was secured. The aim of the service was to improve the experience for the families. This report describes and evaluates the service against local standards of (1) less than four day turn around, (2) cause of death given in >90% and (3) less than 10% require autopsy.
A retrospective review of reports, records and emails was undertaken to collate demographics, times of different stages of the process, the outcome and comments from service users.
Between July 2015 and July 2018, 279 patients had PMCT scans, 67 (24.0%) in the family-funded service and 212 (76%) in the current service. 97.1% (
= 271/279) of cases had the radiology report issued by day 3 (96.8%
98.6% for the family funded and local authority-funded services respectively). A cause of death was given in 97.2% of scans. 2.8% of patients required autopsy. Feedback from families, coroner's officers and undertakers has been overwhelmingly positive.
The services exceeded local standards and met the needs of the Coroner and the families based on the feedback received. This model could be employed for similar services, but the change to the logistics and financial structures required to initiate such services remains a significant hurdle.
This is the first report of a fully NHS-based PMCT service.
In the following pictorial review, common and uncommon neurovascular complications associated with a spectrum of viral, bacterial and fungal infections involving the central nervous system will be ...illustrated. These complications include cerebral vascular insult, venous thrombosis, vasculitis and aneurysm formation. They can occur as separate entities but are often inter-related. The imaging features of neurovascular complication related to infections can provide clues and aid diagnosis when considering the potential mode of infectious spread and the type of potential infectious organism involved. The radiological appearances vary from common features that are shared by several types of pathogens to typical characteristics of a type of infectious organism.
Despite the thousands of articles discussing appendicitis in the literature, the dilemma of how to best diagnosis and manage pediatric appendicitis remains unsettled. Over the past decade, evidence ...has been mounting about the use of antibiotics as the sole therapy in uncomplicated appendicitis in the adult population. This debate has even recently bled over into the lay press. While this change in practice pattern is still in its infancy for the pediatric population, radiologists should be aware of this change in therapy and how it can impact the imaging work-up and relevant findings. This article concisely summarizes the imaging findings and various imaging pathways to arrive at the diagnose of appendicitis with an emphasis of how to best be of use to our surgical colleagues in this evolving paradigm. It also highlights venues for further research, namely increasing accuracy of differentiating complicated from uncomplicated appendicitis.
