Institutional Review Board sometimes struggles with the application of the regulatory criterion that for clinical research to be approvable, risks should be minimized. We have an IRB staff member who insists on documenting every action, conversation, email, and decision that is in any way related to a protocol that came to the IRB for review, even when the ...information is not significant (e.g., a Principal Investigator calls to confirm his deadline for continuing review). She says that she does this based on her memory of a former Office of Human Research Protections director who said, “If it wasn’t documented, it didn’t happen.” While we all agree that documentation is important and we want to be compliant, the files are becoming stuffed with contact reports and notes that make it hard to find the substantive documents. Is this necessary?
