The tibiofibular syndesmosis is a fibrous joint essential for ankle stability, whence the classical comparison with a mortise. Syndesmosis lesions are quite frequent in ankle trauma. This is a key ...element in ankle stability and lesions may cause pain or instability and, in the longer term, osteoarthritis. The lesions are often overlooked due to diagnostic difficulties, but collision sport with strong contact is the main culprit. Diagnosis, whether in the acute or the chronic phase, is founded on an association of clinical and paraclinical signs. Cross-sectional imaging such as MRI is fundamental to confirming clinical suspicion. Absence of tibiofibular diastasis no longer rules out the diagnosis. Stress CT and the introduction of weight-bearing CT are promising future diagnostic tools. Exhaustive osteo-ligamentous ankle assessment is necessary, as syndesmosis lesions may be just one component in more complex rotational instability. Therapeutically, arthroscopy and new fixation techniques, such as suture buttons, are opening up new perspectives, especially for chronic lesions (>6months). The present anatomic, epidemiological, diagnostic and therapeutic review does not preclude further clinical studies of rotational ankle instability with its strong risk of osteoarthritis.
Background:

Methods:
The primary posterior malleolar fracture fragments were characterized into 3 groups. A type 1 fracture was described as a small extra-articular posterior malleolar primary fragment. Type 2 fractures consisted of a primary fragment of the posterolateral triangle of the tibia (Volkmann area). A type 3 primary fragment was characterized by a coronal plane fracture line involving the whole posterior plafond.
Results:
In type 1 fractures, the syndesmosis was disrupted in 100% of cases, although a proportion only involved the posterior syndesmosis. In type 2 posterior malleolar fractures, there was a variable medial injury with mixed avulsion/impaction etiology. In type 3 posterior malleolar fractures, most fibular fractures were either a high fracture or a long oblique fracture in the same fracture alignment as the posterior shear tibia fragment. Most medial injuries were Y-type or posterior oblique fractures. This fracture pattern had a low incidence of syndesmotic injury.
Conclusion:
The value of this approach was that by following the pathomechanism through the ankle, it demonstrated which other structures were likely to be damaged by the path of the kinetic energy. With an understanding of the pattern of associated injuries for each category, a surgeon may be able to avoid some pitfalls in treatment of these injuries.

In advanced stages of ankle osteoarthritis (OA), ankle arthrodesis (AA) or total ankle arthroplasty (TAR) may be necessary. Our purpose is to compare AA and total ankle replacement for the surgical ...management of end stage ankle OA.
We conducted a literature search of PubMed, Medline, CINAHL, Cochrane, Embase and Google Scholar databases using the terms 'ankle' in combination with 'OA', 'arthrodesis', 'arthroplasty', 'joint fusion', 'joint replacement'. Studies where treatment was exclusively total ankle replacement or AA were excluded. Treatment characteristics and outcome parameters (overall postoperative outcome and complication rate) were reviewed.
When counseling patients who are considering their options with regard to ankle arthritis treatment, surgeons should determine on an individual basis which procedure is more suitable.
TAR has become an accepted treatment for end-stage OA, but revision rates for TAR are significant higher than for AA (odds ratio 2.28 95% confidence interval CI, 1.63-3.19; P < 0.0001).
The results of TAA are gradually improving, but the procedure cannot yet be recommended for the routine management of ankle OA.
Although there is some evidence to support TAR to conserve ankle motion and offer improved function and decreased pain with high satisfaction rates, revision rates for TAR are significantly higher than revision rates for AA. Proper patient selection should be better addressed in future studies for successful treatment of end-stage ankle OA.

Anterior ankle impingement syndrome (AAIS) has been reported to account for a high percentage of complications following ankle fracture surgery. The soft tissue etiology of AAIS is thought to be ...thickening and inflammation of the anterior ankle soft tissues intervening anteriorly at the tibiotalar joint, causing pain and functional limitation during dorsiflexion. However, the effects of anterior ankle soft tissue dynamics and stiffness on AAIS have yet to be clarified. This study aimed to determine the relationship between AAIS and the anterior ankle soft tissue thickness change ratio and shear modulus using ultrasonography (US). The participants were 20 patients with ankle joint fractures (AO classification A, B) who had undergone open reduction and internal fixation and 20 healthy adults. The evaluation periods were 3 months and 6 months postoperatively. US was used to delineate the tibialis anterior tendon, extensor hallucis longus tendon, and the extensor digitorum longus tendon over the talus and tibia on a long-axis image. Anterior ankle soft tissue thickness was measured as the shortest distance from the most convex part of the talus to the tendon directly above it. The Anterior ankle soft tissue thickness change ratio was determined by dividing the value at 0° dorsiflexion by the value at 10° plantarflexion. The same images as for the anterior soft tissue thickness measurement were drawn for the shear modulus measurement, and the average shear modulus (kPa) was calculated using shear-wave elastography. There was no significant difference in the thickness change ratio between the postoperative and healthy groups. Compared with the healthy group, the shear modulus was significantly higher at 3 and 6 months in the postoperative group (p < 0.01). The shear elastic modulus at 6-month postoperative group was significantly lower than at 3-month postoperative group (p < 0.01). Anterior ankle joint soft tissue stiffness may increase after surgery for an ankle fracture.
Purpose
Chronic pain can affect up to 40% of patients after ankle inversion sprains. The current hypothesis to explain this high percentage of chronic pain is a partial/total rupture of anterior ...talofibular ligament (ATFL) superior fascicle, a structure that has recently been described as intra‐articular and as having a different function than ATFL's inferior fascicle. This has created the need for diagnosing ATFL superior and inferior fascicles independently. Therefore, the objective of this study is to investigate if the ATFL's superior fascicle can be visualized on ultrasound, and to describe its ultrasonographic appearance.
Methods
Twenty fresh‐frozen ankle specimens were used in this 4‐phases study. First, the specimens were scanned on US to identify what was believed to be ATFL's superior fascicle. Second, ATFL's superior fascicle was sutured under direct arthroscopic vision. Next, the specimens were scanned on US to obtain an image of the sutured structure. Finally, the specimens were dissected to confirm that the suture was indeed placed on ATFL's superior fascicle.
Results
On the 20 specimens studied, full correlation was obtained between US, arthroscopic suture and specimen dissection. ATFL's superior fascicle US appearance is provided.
Conclusion
ATFL's superior fascicle can be visualized on US, which will allow to undergo diagnosis of isolated injuries to that fascicle, a common finding in ankle microinstability. The results of this study will facilitate the diagnosis of partial or complete rupture of ATFL's superior fascicle, likely increasing the amount of ankle microinstability diagnosis, impacting clinical management of ankle sprain consequences.

Introduction/Purpose:
Diagnosing syndesmotic instability, especially when subtle, remains challenging. Weight bearing computed tomography (WBCT) offers a unique opportunity to ...evaluate the distal syndesmosis under physiologic load while simultaneously comparing the injured and uninjured side. We hypothesized that WBCT volumetric measurements of the distal syndesmosis were increased on the injured side as compared to the contralateral uninjured side among patients with syndesmotic instability. Our secondary hypothesis was that these 3-dimensional calculations were an even more sensitive determinant of instability as compared to 2-dimensional methodology.
Methods:
Twelve patients with unilateral syndesmotic instability requiring operative fixation who underwent preoperative bilateral foot and ankle WBCT were included in the study group. The control group comprised of 24 patients without ankle injury who underwent similar imaging. For each WBCT scan, 2-dimensional measurements of the interspace between the distal fibula and tibia were measured 1cm above the joint line in the axial plane, namely the syndesmosis area and the direct anterior, middle and posterior difference. Furthermore, three volumetric measurements of the interspace between the distal fibula and tibia were evaluated: 1) from the tibial plafond extending until 3cm proximally, 2) 5cm proximally, and 3) 10cm proximally from the joint line.
Results:
In patients with unilateral syndesmotic instability, all weightbearing volumetric measurements were significantly larger on the injured side as compared to the contralateral, uninjured side (p<0.001). In the control group, there was no difference between syndesmotic volumes at any level. Of the three anatomic reference points, the volumetric measurement spanning from the tibial plafond to a level 5cm proximally had the highest relative volumetric ratio between the injured and uninjured side, suggesting it is more sensitive in distinguishing stable and unstable syndesmotic injuries (median ratios (IQR) 1.3(1.2-1.4), 1.8(1.6-2.1), 1.4(1.3-1.5), respectively; p<0.001). Additionally, this relative volumetric ratio was also more sensitive than 2-dimensional measurements (p=0.001).
Conclusion:
Volumetric measurement of the distal tibiofibular interspace using WBCT appears to be the most effective way to diagnose syndesmotic instability. The measurement from the tibial plafond extending until 5cm proximally is more sensitive to detect syndesmotic instability than using either more traditional 2D WBCT syndesmosis measurements or using more distal (3cm) or proximal (10cm) 3D volumetric measurements. This does not seem surprising given the overall spectrum and 3D nature of the syndesmotic injury across the injured population.

Introduction/Purpose:
Recent literature has established the middle facet of the subtalar joint as a reliable and accurate marker of pronounced peritalar ...subluxation (PTS) in Adult Acquired Flatfoot Deformity (AAFD) patients. The Foot and Ankle Offset (FAO) has also been accepted as a more complete and three-dimensional (3D) measurement of foot collapse and hindfoot valgus. To date, no assessment has been made regarding the relationship between the severity of the AAFD and the amount PTS measured at the middle facet. The objective of this study was to investigate the correlation between these variables. We hypothesized that direct positive linear correlation would be found, with significantly increased middle facet joint incongruence and subluxation in patients with more pronounced hindfoot valgus and foot collapse.
Methods:
In this IRB-approved retrospective comparative study, we included 76 feet (42 left, 35 right) with symptomatic AAFD who underwent standing weightbearing CT (WBCT) as standard baseline assessment of their foot deformity. One blinded fellowship-trained foot and ankle surgeon with more than 10 years of experience performed measurements using multiplanar reconstruction (MPR) WBCT images. Manual measurements of subluxation and incongruence of the middle facet were performed at the anteroposterior midpoint of the articular facet. Semiautomatic calculation of the FAO was performed after 3D coordinate harvesting of the weightbearing points of the first, fifth metatarsal and calcaneus as well as the center of the ankle. Correlation of FAO severity and subluxation/incongruence of the middle facet was assessed by Spearman’s correlation and bivariate analysis. Paired Wilcoxon was utilized to compare FAO values in congruent (0% subluxation), subluxated (1-99% subluxation) and dislocated middle facets. P-values <0.05 were considered significant.
Results:
No significant direct correlation was observed between FAO and Middle Facet Incongruence angle (p=0.12). However, positive linear correlation was found between middle facet subluxation and FAO (Spearman’s 0.54, R2 0.29, p<0.0001). The subluxation of the middle facet was found to increase by 3.5% for every one-point FAO increase (Middle Facet Subluxation (%) = 6.903202 + 3.5452074*Foot and Ankle Offset). Mean values and confidence intervals (CI) of FAO were significantly different depending on the congruency of the middle facet (p=0.0003): congruent, 5.2 (CI, 3.4 to 7.0), subluxated, 8.7 (CI, 6.8 to 9.0) and dislocated, 12.9 (CI, 9.8 to 15.9). Similarly, middle facet subluxation was also found to progressively and significantly increase when FAO values were staged from zero (normal alignment) to four (FAO>20) (p<0.0001).
Conclusion:
This study is the first to compare and correlate measurements regarding the amount of subluxation of middle facet with the severity of AAFD, when measured by the foot and ankle offset (FAO). We found a significant, positive and linear correlation between the amount of subluxation of the middle facet and FAO measurements. No correlation was found between the incongruence angle of the facet and FAO. Our results support the concept of using the subluxation of the middle facet of the subtalar joint as a key marker of early and progressive deformity in patients with AAFD.

Introduction/Purpose:
The current scientific evidence for the clinical and sports-related outcomes of the surgical treatment of osteochondral lesions of the talus (OLTs) ...with arthroscopic microfracture (MF) in elite athletes is scarce. The purpose of curretn study is to evaluate the clinical outcomes and return to sports activity after arthroscopic MF for OLTs in elite athletes.
Methods:
The files of 41 elite athletes (mean age 19.34 +- 3.76 years) who had undergone arthroscopic MF for OLTs at our institution between January 2011 and September 2015 were retrospectively reviewed. The Foot and Ankle Outcome Score (FAOS), American Orthopaedic Foot & Ankle Society (AOFAS) hindfoot and visual analog pain (VAS) score, time and rate of ‘return-to-competition’ (RTC, defined as return to an official match for at least one minute after treatment), and rate of ‘return- to-play’ (RTP, defined as participation in at least two entire seasons after treatment) were used to evaluate the outcomes. We compared athletes who were able to RTP (RTP-group) with those who were not (No-RTP group).
Results:
The mean follow-up was 54.9 +- 13.72 months. All five subscales of preoperative FAOS were significantly improved at the final follow-up. The mean preoperative AOFAS hindfoot score of 74.46 +- 8.10 improved to 91.62 +- 2.99 (P < 0.001) at the final follow-up. The mean preoperative VAS score of 5.44 +- 1.57 improved to 2.66 +- 1.04 (P < 0.001). All patients achieved RTC (100%) at mean time of 5.45 +- 3.18 months, and 74.4% of patients were able to RTP. The RTP-group showed significantly smaller lesions compared to the No-RTP group (71.52 +- 43.29 vs. 107.00 +- 45.28 mm2, P = 0.009). The cut-off OLT size for predicting RTP was 84.0 mm2, with a sensitivity of 90.0% and specificity of 75.9%.
Conclusion:
All athletes were able to RTC at average of 5.45 months after MF for OLTs, and 74.4% were able to RTP. The only prognostic variable for RTP was lesion size, and its predictive cut-off was 84 mm2.

Introduction/Purpose:
Assessment of mortise stability is paramount in determining appropriate management of ankle fractures. While instability is readily apparent in ...bimalleolar or trimalleolar ankle fractures, determination of instability in the apparent isolated Weber B fibula fracture often requires further investigations such as stress radiography. While several authors have previously demonstrated poor predictive value of physical examination findings such as tenderness, ecchymosis and swelling with instability, no previous investigation has examined the ability of a translational test to determine deltoid ligament injury. The goal of this study is to test the validity of a novel clinical test, the lateral drawer test, in determining mortise instability in a cohort of patients with Weber B fibula fractures.
Methods:
Patients presenting with isolated fibula fractures were prospectively identified by an orthopaedic nurse practitioner or resident. Three views of the ankle as well as gravity stress views (GSV) were obtained. Prior to examining radiographs, two foot & ankle fellowship-trained orthopaedic surgeons (JYK, CPM) performed and graded the lateral drawer test. (Grade 0: no instability/symmetric to contralateral ankle, Grade I: translation < 5mm, Grade II: translation >= 5mm) While the surgeons were made aware of the presence of a Weber B fibula fracture, radiographs were not visualized prior to conducting the drawer test.
Medial clear space (MCS) was measured. VAS pain scores were obtained before and after testing. The results of the lateral drawer test results were compared with radiographic measurements of MCS.
Results:
The study has thus far prospectively enrolled 14 consecutive patients with Weber B ankle fractures. Six (43%) patients demonstrated radiographic instability with widening of the medial clear space >= 5 mm on GSV. When correlated with MCS measurement, the lateral drawer test demonstrated a sensitivity of 83.3%, specificity of 100%, positive predictive value (PPV) of 100% and negative predictive value (NPV) of 88.9% for determining instability. There was a strong correlation between the lateral drawer test grade and amount of MCS widening (⍴=0.82, p=0.0003). Patients tolerated the lateral drawer test well with an average increase of 1.6 on the VAS pain scale after testing.
Conclusion:
The lateral drawer test demonstrates high sensitivity, specificity, PPV and NPV for detecting instability in patients presenting with Weber B fibula fractures. While further validation is required in a larger cohort of patients, the lateral drawer test may offer an adjunct tool via physical examination to predict mortise stability.
