These data shows the need for affordable and good follow-up care especially for patients without any cancer treatment due to irreversible progression of tumor. Currently the outpatient palliative cancer care gets more into the forefront. Prerequisite for a well working outpatient palliative care is cooperation with general practitioners and home health care agencies. The purpose of the so called program of palliative cancer care is to guide a patient in palliative cancer care and to improve the cooperation among health care providers. METHODS: During the period from January 2008 to October 2010 we evaluated in patient without any oncology treatment due to irreversible progression of tumor. RESULTS: In palliative outpatient clinic we treated 446 patients, 119 of them received home care services with average length of 27.8 days. 77 patients died at home, 51 in health facilities and 41 in inpatient hospice care. CONCLUSION: We present pilot study focusing on outpatient palliative cancer care which shows the real benefit from early indication of palliative cancer care. Kurativní multimodální léčba je postupně limitována stavem nemocných a pokročilostí procesu. Existují různá řešení péče o starší nemocné s pokročilou malignitou. Při rozhodování jde o citlivý problém a potřebu sladit objektivní možnosti terapie s osobním postojem nemocného k jeho onemocněním. Konečným cílem je dosazení optimální kvality života a zásada eubiosie (dobrý život do posledního dne).- Achieving the optimal care in patients, suffering from advanced malignancies represents a difficult ethical and economical problem of the health care in developed countries. Increasing part of elderly population with growing incidence of malignant diseases is the main reason of that. There are different attempts for solving this problem. Selection of the optimal solution depends on the possibilities of anticancer therapy, regarding the CGA (Comprehensive gariatric assesment) and individual attempt of the patient to different solutions. Přitom stoná-li nemocný delší dobu a jeho nemoc má nepříznivý vývoj, přemýšlí o konci života a vytvoří si určitou představu, jak a kde by se měl konec jeho života odehrát. Je-li skutečnost podstatným způsobem odlišná, vyvolává to pocit dyskomfortu. Základní složkou péče v terminálni fázi by tedy měla být informovanost o přáních nemocného, o odmítaných procedurách apod. Například 60 % nemocných starších 65 let odmítá „život na přístrojích", 33 % odmítá život se stálou bolestí, 24 % umělou výživu apod. Podstatnou roli hraje předpokládané místo úmrtí, i když ne každý si přeje zemřít doma, ale ani těm, kteří si to přejí, se toto přání mnohdy nesplní. Dále jsou v článku rozebírány rysy nemocniční péče o terminálne nemocného a péče mimo nemocnici. Je obava, že faktory, které ovlivňují konečnou podobu terminálni fáze ze strany nemocného, nebude možno v domácím prostředí zvládat, a vzniknou tak komplikace, a nemocný bude na obtíž svým blízkým. Rodina a pečující tým má obavu, že nerozpozná ještě řešitelnou komplikaci a způsobí předčasný odchod svého blízkého. Z právního hlediska je podstatné týmem konstatované a do dokumentace nemocného zaznamenané ukončení kauzálni léčby a vyjasnění situace v podávání opioidů. V závěru článku je uvedena kazuistika términální fáze nemocného s bronchogenním karcinomem plic.- The situation of dying people during the time immediately preceding the death is not satisfactory solved until now. The patient creates his vision of the end of his life, if he is ill for longer time and the course of his disease is unfavourable. He feels quite uncomfortably, if the reality is rather different. The information concerning end of life decisions should be one of the basic steps in the terminal care. For example - 60% of seniors above 65 years of age consider the life supporting activities „worse as a death", 33% the life with permanent pain, 24% the arteficial nutrition etc. Substantial is the supposed place of the expected death - not everybody wants to die at home, but even these, they want, freíuently die at the hospital. Factors how the dying patient influences the final course of the end of life are the fear from complications, which could not be settled et home and the wish not to be the burden for family members. The fami¬ ly members and caregivers are affraid of causing the untimely death by unrecognized still solvable complication considered as the final one. From the law point of view there is substantial by team to establish and into patient's documents to sign the end of causal therapy. The opioid administration from the lawers point of view is another important i issue in terminal care.
