Intensitätsmodulierte Strahlenfelder bieten eine wesentliche Verbesserung der Strahlentherapie im Hinblick auf Konformität der Dosisverteilung mit dem Zielvolumen und der gleichzeitigen Schonung von ...Risikobereichen. Die Bestimmung der Fluenzprofile ausgehend von einer gewünschten Dosisverteilung ist ein Optimierungsproblem. Von der Formulierung dieses Problems in Form einer Zielfunktion hängt es ab, ob eine sinnvolle Lösung gefunden werden kann. Die notwendigen Parameter, um die komplikationsfreie Tumorkontrollwahrscheinlichkeit zu maximieren, sind oft nicht genau genug bekannt. In dieser Arbeit wird die übliche Methode der Minimierung der quadratischen Abweichung von einer Solldosisverteilung mit Methoden, welche die direkte Eingabe von Dosisschranken ermöglichen, verglichen. Die Methoden werden auf ein Testmodell angewandt. Bei der konventionellen Zielfunktion ist es notwendig, Wichtungsparameter interaktiv anzupassen, bis eine brauchbare Dosisverteilung erzielt wird. Die auf Dosisschranken basierenden Algorithmen sind schneller, und es lassen sich leichter klinisch gebräuchliche Strategien wie palliative bzw. kurative Therapie simulieren. Die Unterschiede in den Dosisverteilungen bei diesen Strategien werden in Abhängigkeit von der Felderanzahl dargestellt.
Intensity modulated irradiation beams provide a substantial progress for radiotherapy. Both, the conformity of the dose distribution with the target volume and sparing of organs at risk are improved. The determination of the fluence profiles is an optimization problem. It depends on the formulation of the objective function, whether a meaningful solution can be achieved. The parameters to maximize uncomplicated tumor control directly, are not yet known with sufficient accuracy. In this work, the usual method of minimizing the quadratic deviation between the calculated and desired dose distributions is compared to methods where clinically proven dose constraints are used directly. For the first approach it is necessary to adapt weighting factors interactively in order to achieve the desired result. Algorithms based on the use of dose limits are faster and allow to simulate usual clinical strategies like palliation or curative treatment very easily. The differences of the dose distributions for this strategies are described as a function of the number of beam portals.
Das Zielvolumen bei der Bestrahlung der endokrinen Orbitopathie umfaßt den Peri- und Retrobulbärraum mit den extraokulären Muskeln. Eine Konformations-Bestrahlungstechnik für dieses Zielvolumen wird ...vorgestellt. Die Bestrahlungstechnik besteht aus zwei Rotationsfeldern mit zentralem Linsenblock und zwei seitlichen Stehfeldern mit dorsaler Ausblockung. Die Linsen werden in jeder Gantryposition durch den zentralen Linsenblock ausgeblendet. Das Isozentrum der zwei Rotationsfelder befindet sich in den Linsen. Die Verifikation der Dosisverteilung zur Qualitätsicherung der Bestrahlungstechnik wurde mit Hilfe der Filmdosimetrie am Alderson-Phantom durchgeführt.
Mit Hilfe dieser Bestrahlungstechnik wurde eine zielvolumenkonforme Dosisverteilung, die den Peri- und Retrobulbärraum und die extraokulären Muskeln eifaßt, ermöglicht. Die filmdosimetrische Messung am Alderson-Phantom zeigt eine maximale Abweichung zwischen gemessener und berechneter Dosis von 5 %. Eine Dosisbelastung an den Augenlinsen von 25 % der applizierten Bestrahlungsdosis wurde gemessen.
The clinical target volume (CTV) for the irradiation of the endocrine orbitopathy (Graves ophthalmopathy) includes the peri- und retrobulbar space with the extraocular muscles. We present here a conformal irradiation technique aimed at optimal coverage of the CTV. The irradiation technique consists of two rotation fields with a central lens block and two lateral fixed fields with dorsal blocking. In each Gantry position, the lenses are faded out through the central lens block. The isocenter of the two rotation fields is located in the lenses. For quality assurance of this irradiation technique, verification of the dose distribution was performed by film dosimetry using the humanoid Alderson phantom.
The use of this irradiation technique yielded a dose distribution with conformal CTV coverage of the peri- and retrobulbar space and of the extraocular muscles. The film dosimetry of the Alderson phantom showed a maximal deviation of 5 % between the measured and the calculated dose distribution. The radiation load to the eye lenses was 25 % of the applied total dose.
