Japan became the first country in the world to enter the super-aged society in 2007, and reform of the healthcare system and welfare policies is now an urgent issue. The elderly with geriatric ...syndromes and numerous chronic diseases can easily have serious and complicated conditions. In general, medical care has been focused on 'cure-seeking medical care' for single diseases, but a paradigm shift is needed to 'cure- and support-seeking medical care' for the elderly, with an emphasis on quality of life. In addition to 'preventive medicine', it will also be necessary to develop 'preemptive medicine', which starts at a stage before a disease or disorder occurs. This will require the development of new predictive measures and diagnostic methods that integrate novel biomarkers, genomic, proteomic and metabolomic analysis, and analysis of other clinical information, etc. Although ageing care research has progressed dramatically in recent years, it has not been sufficient in the field of Pancreatology. In the future, new knowledge about ageing will be accumulated through various approaches, so that evidence-based medical care can be provided to elderly patients with pancreatic diseases.

Objective: to understand the knowledge and preventive practices of elderly people about COVID-19. Methods: qualitative research developed with 11 elderly people living in a condominium for the ...elderly. Data were obtained using the technique of individual interviews and then submitted to lexicographic analysis by means of the Descending Hierarchical Classification using the software IRaMuTeQ®; and discussion in the light of the literature on COVID-19. Results: six classes emerged: Building knowledge and preventive practices about COVID-19; Emotional consequences; Preventive and control practices; Recognition and importance of prevention strategies; Physical, social, and economic impacts; and Individual and collective risk factors. Conclusion: the elderly understands the negative consequences of COVID-19, identifying the individual and collective risk factors, learned based on experiences during the pandemic, previous experiences, dialogues, and sources of information. They recognize the importance of preventive practices, carrying out care in the home and urban environment. Contributions to practice: the study contributes positively to the care of the elderly with the construction of knowledge about COVID-19, especially in the context of elderly residents in a condominium for the elderly, sensitizing and grounding health professionals to plan and develop problematizing educational actions, impacting on professional practice.

Family members are the primary source of support for older adults with chronic illness and disability. Thousands of published empirical studies and dozens of reviews have documented the psychological ...and physical health effects of caregiving, identified caregivers at risk for adverse outcomes, and evaluated a wide range of intervention strategies to support caregivers. Caregiving as chronic stress exposure is the conceptual driver for much of this research. We review and synthesize the literature on the impact of caregiving and intervention strategies for supporting caregivers. The impact of caregiving is highly variable, driven largely by the intensity of care provided and the suffering of the care recipient. The intervention literature is littered with many failures and some successes. Successful interventions address both the pragmatics of care and the emotional toll of caregiving. We conclude with both research and policy recommendations that address a national agenda for caregiving.
An 81-year-old man was being treated with oral medication for chronic heart failure and epilepsy. He had no history of diabetes, cirrhosis, or gastric surgery. He was admitted to our hospital due to ...disturbance of consciousness. His blood glucose level was 6 mg/dl, with a relatively high insulin level (14.4 μU/ml). Computed tomography and a 48 h fasting test showed no signs of insulinoma. There were no signs of reactive hypoglycemia, insulin autoimmune syndrome, or adrenal insufficiency. His wife had been taking medication for diabetes, including sulfonylurea. She had dementia, and he managed her medication. Since his medication was found in his wife's medicine box, we considered the possibility that he might have taken sulfonylurea by mistake. We asked his daughter to manage their medicine. However, one month later, he was admitted to our hospital again with severe hypoglycemia. His wife's HbA1c value and estimated glomerular filtration rate were 6.9% and 30 ml/min/1.73 m2. We asked his wife's home doctor to stop sulfonylurea prescription, and the hypoglycemia did not recur, with his wife's level of HbA1c remaining stable.Elderly individuals and patients with an impaired renal function are prone to hypoglycemia from sulfonylurea. In elderly households, there is a possibility of accidental ingestion of oral hypoglycemic agents by other family members living with the patient. It is therefore necessary to understand and manage the medications of family members living together. It is also important to avoid prescribing medications with a high risk of hypoglycemia to elderly patients.
The present study aimed to understand the meaning of work and leisure among the elderly. Participants were 10 elderly people over 65 years old who had work experiences followed by leisurely lives. ...Data was collected between the winter of 2014 and the summer of 2015 and analyzed using the descriptive phenomenological psychological approach of Giorgi . The results are as follows. First, 're-establishing daily life between work taking a sideline and leisure taking the centre in life' emerged as the essential meaning of the work and leisure for the elderly. Second, it could be understood that the essence of work and leisure for the elderly people, in addition to activities, had various meanings. Third, the relationship between work and leisure for the elderly was both mixed and conflicted. This study suggests that work and leisure for the elderly are two important axes in their lives, and neither can be ignored.
Introduction: In this study, the clinical results and prognostic factors after chemonucleolysis with condoliase for lumbar disc herniation in patients over 70 years 70 years of age were ...investigated.Methods: The study included 20 patients over 70 years of age who presented primarily for evaluation of leg pain and underwent chemonucleolysis with condoliase for lumbar disc herniation. Preoperative and postoperative low back pain, leg pain, Japanese Orthopaedic Association (JOA) scores, plain radiographs, magnetic resonance imaging, and computerized tomography were compared between patients who improved (improvement group) and those without improvement (non-improvement group). Results: Leg pain improved in 30.0% of the patients (6 and 14 in the improvement and non-improvement groups, respectively), and seven cases (35.0%) underwent surgery within one year. In patients who did not undergo surgery, leg pain improved significantly after treatment. Comparing the effective and ineffective groups, Pfirrmann's grades were significantly higher in the effective group.Conclusions: Chemonucleolysis with condoliase for lumbar disc herniation in elderly patients was found to be effective in patients with advanced discwas found to be effective in patients with advanced disc degeneration, although less effective than in younger patients. These results suggest that chemonucleolysis with condoliase may be a useful treatment with appropriate indications.
