Department of Fixed Prosthodontics. - Data provided by Europeana Collections- Iako se implantati dugo vremena upotrebljavaju u svakodnevnoj praksi nema dovoljno podataka o tome kako tip fiksacije (vijkom pričvršćeni u odnosu na cementirane) utječe na prisutnost određenih bakterija. Cilj ovog istraživanja je bio odrediti prisutnost A.actinomycetemcommitans, P.gingivalis, T. forsythia, P. intermedia, T. denticola i F.nucleatum u ispitanika koji imaju sa vijkom pričvršćene u odnosu na cementirane implantate. Ujedno cilj je ovog istraživanja bio odrediti utjecaj lokalnih i sistemskih čimbenika na prisutnost bakterija s obzirom na tip fiksacije (vijkom pričvršćeni u odnosu na cementirane). Praćeni su lokalni čimbenici poput pušenja i stanja parodonta mjereni indeksima Svjetske Zdravstvene Organizacije, a sistemski čimbenici poput sistemskih bolesti i upotrebe lijekova su također zabilježeni. Nadalje je određivano stanje temporomandibularnih zglobova na temelju anamnestičkih podataka ali i pregleda navedenih zglobova. Ukupan broj od 136 implantata je postavljen u 51 osobe, od čega su u 32 osobe oni bili cementirani, a u 19 osoba su bili pričvršćeni pomoću vijka. Uzorci su uzeti s pomoću sterilnih papirnatih štapića i analizirani uz pomoć real-time reakcije lančane polimeraze (Carpagen® GmbH, Münster, Germany) prije i nakon postave implantata. Statistička analiza je uključivala parametrijske i neparametrijske testove i p vrijednost manja od 0,05 je smatrana znakovitom. Rezultati ovog istraživanja pokazuju kako nije bilo znakovitih razlika u broju A.actinomycetemcommitans, P.gingivalis, T. forsythia, P. intermedia, T. denticola i F.nucleatum s obzirom na tip fiksacije bataljka. Nadalje, pokazano je znakovito povećanje broja bakterija u osoba koje su bolovale od kardiovaskularnih i reumatskih bolesti kao i u ispitanika koji su bili stariji te su uzimali lijekove. Doduše rezultati ovog istraživanja nisu pokazali znakovite razlike vezano uz spol ispitanika i prisutnost parodontalnih ispitivanih patogena. Rezultati ovog istraživanja pokazuju kako je utvrđena statistički znakovita razlika u broju različitih bakterija s obzirom na broj nadomjestaka (jedan ili više implantata). Rezultati ovog istraživanja pokazuju kako se nakon postave implantata povećala patološka dubina džepova te da je u njima nađena povećana količina bakterija. Nadalje, rezultati ovog istraživanja pokazuju kako su sudionici koji su imali pozitivnu anamnezu vezano uz temporomandibularni zglob, imali veći broj prisutnih bakterija. Na temelju rezultata ovog istraživanja možemo zaključiti kako prevalencija istraživanih bakterija nije različita s obzirom na tip fiksacije bataljka (pričvršćeni vijkom u odnosu na cementirane).- Background and objectives: Although implants are widely used in everyday practice there are insufficient data upon type of abutment fixation (screw versus cement) and presence of microbial species. The aim of this study was to determine presence of A. actinomycetemcommitans, P. gingivalis, T. forsythia, P. intermedia, T. denticola and F. nucleatum in patients with screw or cemented implants. Furthermore, the aim of this study was to evaluate influence of local and systemic factors on the presence of investigated bacteria regarding the type of fixation (screw versus cement). Local factors such as smoking and periodontal status measured by World Health Organisation were analysed as well as systemic factors such as systemic diseases and medication intake. On the basis of detailed medical history together with clinical assessment, condition of temporomandibular joint disturbances was assessed. Methods: In fifty one patient, 136 implants were inserted, out of which in 32 patients they were cemented and in 19 patients they were screw retained. Samples were taken with sterile paper points and analyzed with real-time polymerase chain reaction (Carpagen® GmbH, Münster, Germany) before and after implant placement. Statistical analysis included parametric and nonparametric test and p<0.05 was considered significant. Results: No differences in the presence of A. actinomycetemcommitans, P. gingivalis, T. forsythia, P. intermedia, T. denticola and F. nucleatum with regard to the abutment fixation. Furthermore, there was a significant increase in bacterial count in persons suffering from cardiovascular and rheumatic diseases as well as in those patients who took medications and who were older. However, there were no significant differences between genders regarding investigated periodontal pathogens. The results of this study show significant differences in the number of investigated bacteria regarding the number of inserted implants (one implant in comparison to the more implants). The results of this study show that after implant placement the pathological pocket depth increased and that more bacteria were found within.
